The risk for long-term health problems such as blindness, renal failure, heart attack, lower limb amputations, and stroke are exacerbated if healthy monitoring of the condition is not adhered to consistently, and early after diagnosis. The present study sought to examine theoretical constructs which are important to overall treatment adherence among adolescents with Type 1 diabetes and their maternal caregivers. Self-efficacy and perceptions of diabetes self care between child and mother were disseminated through the inclusion of three separate measures designed for adult caregivers, and their children with Type 1 diabetes. Despite the literature which has established significant findings in the treatment domains separately, the present study did not yield significant results across the domains, but was able to offer new insights into previously held findings in the Type 1 literature. The author concludes with an analysis of the barriers in conducting research with chronically ill populations, and offers suggestions for future research in this area.

Book excerpt: Type 1 diabetes (T1D) is one of the most common chronic diseases in youth and it has been shown that adolescents have the worst glycemic control of any age group. The objective of this study was to develop, test and evaluate the feasibility of an online intervention (Can-Do-Tude) that uses the principles of motivational interviewing (MI) to deliver tailored diabetes self-management education to adolescents with T1D. Bandura's efficacy belief system was used to guide the design of this study. The study used a multi-phase, multi-method approach. The first phase (alpha) of this study was a qualitative descriptive design to examine the intervention's fidelity. Evaluation of performance was conducted by experts in the fields of MI, T1D, adolescence and/or online education. The second phase (beta) was a quantitative descriptive design conducted in order to evaluate feasibility by examining the acceptability (recruitment, retention and satisfaction) and implementation (diabetes self-management self-efficacy) to determine whether the intervention was appropriate for further testing. First phase findings showed that the intervention passed all measures with the content experts (n = 6): it was functional, accurate, usable and secure. Improvements to the intervention were made based on reviewer recommendations. For the second phase 5 adolescents between 14 and 17 were enrolled. Three adolescents completed all 4 weeks of the intervention while 2 completed only 3 weeks. Participants (n = 3) rated satisfaction on a 5-point Likert-type scale ranging from "not at all" satisfied (1) to "very much" satisfied (5). There was a positive response to the intervention (M = 4.28, SD = 0.55). Implementation was measured by a pre- and post-test for diabetes self-management self-efficacy. Participants (n = 3) demonstrated overall improvements in diabetes self-management self-efficacy (Z = -2.952, p = .007). Implications for further Can-Do-Tude research are planned at a metropolitan diabetes center using updated technology including an application platform. Although the sample was small, findings indicate that the intervention can be conducted using a web-based format and there is initial evidence of improvement in self-efficacy for diabetes self-management.

Book excerpt: Adolescents living with type 1 diabetes (T1D) begin to adopt a more collaborative approach to managing T1D with their parents before becoming independent in self-management. However, adolescents can encounter difficulties with T1D management and ambiguity in relation to the division of responsibilities for T1D management with their parents. This study aimed to explore: (1) how adolescents living with T1D experience self-management of T1D and share self-management responsibilities with parents and (2) the relationship between adolescent communication with parents about T1D self-management and demographic, clinical and psychosocial variables. A meta-synthesis of qualitative literature on child and adolescent experiences of self-management of T1D was conducted. The findings of the meta-synthesis indicated that balancing the demands of self-management with other aspects of life and sense of control over T1D are important for self-management of T1D. Following this a mixed methods study involving two phases was conducted; semi-structured interviews with adolescents aged 11 to 17 years (n = 28) and a cross-sectional survey with adolescents aged 11 to 17 years (n = 113). In interviews, adolescents highlighted their experiences of gaining independence in self-management in the overarching theme 'Navigating increasing responsibility for self-management during adolescence: "it changed gradually... and I am almost fully doing it [self-managing] now". The following themes contributed to the overarching theme: (1) Changing levels of involvement in self-management, (2) Talking about self-management with parents, (3) Taking ownership of self-management and (4) Environmental and contextual influences affecting self-management. The findings from the quantitative phase of this study indicated that parent-adolescent communication and T1D-specific family conflict are associated with T1D self-efficacy, division of family responsibility for T1D management, activation and self-management of T1D. However, the findings also indicated that T1D-specific family conflict and parent-adolescent communication are differentially related to distinct aspects of self-management and QOL. Overall, this study identified that parent-adolescent communication and family context characteristics relate to and contribute to self-management and T1D-specific QOL in adolescents living with T1D. This research identified what factors contribute to helping or hindering adolescent engagement with self-management of T1D and specifically that adolescents' perceptions of their communication with their parents about T1D management are important. For the first time, the findings provide a nuanced understanding of adolescent perspectives on self-management of T1D and communication with parents about T1D self-management. Promoting self-efficacy through targeting communication strategies employed by adolescents and their parents may result in more optimal sharing of responsibilities, improved self-management and better QOL for adolescents living with T1D.
