Health professionals often overestimate the health literacy of patients, thereby affecting patient outcomes. In a hospital environment, patients rate the ability of the nurse to explain things in a way patients understand by completing the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) after discharge. The HCAHPS results are converted to a score which is used in a formula to determine hospital reimbursement; the lower the score, the lower the reimbursement. The purpose of this project was to improve nurse communication skills and facilitate patient understanding of care. A literature review revealed the teach-back method as a best practice strategy. Teach-back is a communication technique designed to improve patient understanding about what was said by healthcare providers. Teach-back is a way of presenting information, then asking patients to repeat what was said in their own words. An education module designed to improve nurse communication skills was implemented on a 14-bed hospitalist medical unit for the registered nurse staff (N=13). The education included content on health literacy and the teach-back method of communication. Nursing staff was surveyed before (61.5% response rate) and six weeks following (50% response rate) the educational offering, using the Health Professionals Communication Skills Scale (HP CSS), an 18-item tool measuring empathy, informative communication, respect, and social skills. In addition, patient responses to the HCAHPS question, the nurse explained things in a way you could understand were compared before and after the educational offering. -- Overall, the post-survey responses were lower than the pre-survey responses in each domain, indicating a decreased perception of communication by the nursing staff. The purpose of this Doctor of Nursing Practice (DNP) project was to reeducate the nurses on the effective use of Teach-back on a cardiac unit at a community hospital in Maryland. The aim of promoting Teach-back (TB) is to assist nurses in becoming comfortable and competent in using Teach-back effectively. The goal is to improve patient education documentation, increase the use of Teach-back for patient education, close the loop on nurse-patient communication, encourage patient's engagement, and enhance health literacy, and improve patient outcomes. This quality improvement (QI) project took place over a four-week period. Pre-and post-intervention questionnaires were given to cardiac nurses at the hospital before and after the implementation of the educational intervention. A pre- and -post-test design was used to evaluate the increase use of Teach-back. Descriptive analysis was performed to determine if the Teach-back reeducation of the nurses would improve the use of Teach-back. The findings indicated a considerable improvement in the nurses' competence and confidence in using Teach-back for patient education after the intervention. The findings can contribute to positive change by improving the quality of cardiac care, preventing readmissions, and reducing healthcare costs in cardiac facilities. Book excerpt: Improving Nurse-patient Communication about New Medicines Abstract The project aimed at improving nursing communication with patients and families about medication in order to increase patient satisfaction in terms of safety, quality of care, and patient-centeredness in an acute care setting. The selected patient cohort was at a 42-bed medical-surgical unit in a trauma center in Oakland, California. The gap of nurse-patent communication about medicines was identified based on the HCAHPs scores that had been found to be lower than average (44.1% in March 2015; 40.9% in April 2015). The visibility board was provided to show the staff the HCAHPS scores, the pre-survey scores, the information about the teach-back method, and how the "ask3/teach3" approach would be implemented during medication administration with the easy access to the information of the medicines through the EHR during the medication administration. The compared results of 40 surveys from the pre-intervention and post-intervention revealed the increased number of the patients who knew the names of new medicines they had been taking from 25% to 75%, knew the purposes from 50% to 90%, and knew the side effects from 15% to 50%. The teaching materials and strategies included the flyers about the ask3/teach3 methods, the sheets of common side effects of medications list, the communication board on the medicine section, and the role-play on ask3/teach3 intervention. Based on the project, to ensure the effectiveness of the engagement in medication communication process, it is recommended that sustained efforts in collecting data and evaluating the effectiveness of the intervention be encouraged as an ongoing process with staff collaboration. Patient satisfaction with discharge education is a notable component of hospital reimbursement by national standards, as measured by the Hospital Consumer Assessment of Healthcare Providers and Systems survey (HCAHPS) results. Following education of nursing staff participating in use of discharge teaching, patient satisfaction with discharge instructions taught using the teach-back method was measured via HCAHPS for a period of two months. Results suggested potential correlation to improved HCAHPS scores for the question “When I left the hospital, I had a good understanding of the things I was responsible for in managing my health,” associated with use of teach-back at time of discharge.

Book excerpt: The setting for this Clinical Nurse Leader (CNL) project was the Post Anesthesia Care Unit (PACU) at a level I trauma center in the Bay Area. The goal was to improve the discharge education performed by the PACU nurses to improve patient safety and decrease the chance of complications or readmissions to this hospital. With no clear instructions for how discharge teaching should be done, the nurses have many differing styles which leaves room for gaps in discharge planning. This influenced the implementation of the teach-back method during discharge planning in order to ensure proper education and increased patient understanding. A literature review revealed that the teach-back method has helped to decrease hospital readmissions and prevents complications patients may experience during home self-care. The Joint Commission, Agency for Healthcare Research and Quality, and the Institute for Healthcare Improvement all state that the teach-back method for discharge planning is best practice. Project data was collected through observation, microsystem assessments and through nurse and patient surveys. Interventions included nurse education at staff meetings, one on one in-service educations and teach-back handouts. The pre-intervention nurse survey (n=26) showed that 80% of the nurses were familiar with the teach-back method while only 36% of nurses used teach-back during discharge planning regularly. The patient survey (n=25) revealed that patients were able to retain 68% of the information presented during discharge education. One on one in-service education on the teach-back method is still being conducted at this time, with high nurse satisfaction. Follow-up surveys will commence when 100% of the staff have completed the in-service. The expected final outcome of this project is the implementation of the teach-back method during discharge planning by all nurses and increased patient understanding of discharge education.

Education in pain management has been part for standard care for nurses. The purpose of this study was to reinforce teach-back techniques with nurses when delivering pain management education with non-verbal patients in order to improve patient outcomes in the adult ICU. By using the teach-back method, the nurse will be able to hone a more collaborative approach in dealing with pain, encourage autonomy and include the patient/family in making informed decisions regarding treatment for pain. Reinforcement regarding teach-back was focused on teaching points, and rewording teach-back cues to better engage with the patient/family and acquire more data, not just for the education but for their routing cares as well. A family guide was used as a visual aid for the teaching. A survey was conducted, pre-/post-reinforcement whether or not the nurses do teach-back and observations and family interviews were done after the reinforcement of teach-back technique. The findings show that four out of five family interviews found the guide to be useful, and four out of five nurses found the guide to be useful.
