Book excerpt: Approximately 4 million U.S. service members took part in the wars in Afghanistan and Iraq. Shortly after troops started returning from their deployments, some active-duty service members and veterans began experiencing mental health problems. Given the stressors associated with war, it is not surprising that some service members developed such mental health conditions as posttraumatic stress disorder, depression, and substance use disorder. Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

High-quality, evidence-based behavioral health treatment is central to individual well-being and the overall health of the force. However, there has been little research to date on the behavioral health care that National Guard and reserve personnel-collectively known as the reserve component (RC)-receive from the MHS. The full-time personnel in the U.S. military's active component (AC) overwhelmingly receive behavioral health care in military treatment facilities and generally from behavioral health specialty providers, RC personnel rely much more heavily on private-sector contracted care (also referred to as purchased care) and primary care providers. MHS administrative data indicate that RC personnel are less likely to receive recommended treatment for PTSD, depression, and substance use disorders. Likewise, RC personnel who live in areas that are remote from a military treatment facility are less likely than who do not to receive recommended treatment for these conditions. It is well documented in the literature that there are high rates of psychological disorders among military personnel serving in Operation Enduring Freedom in Afghanistan and Operation Iraqi Freedom in Iraq as well as among the service members' families. For service members' families, the degree of hardship and negative consequences rises with the amount of the service members' exposure to traumatic or life-altering experiences. Adult and child members of the families of service members who experience wartime deployments have been found to be at increased risk for symptoms of psychological disorders and to be more likely to use mental health services. In an effort to provide early recognition and early intervention that meet the psychological health needs of service members and their families, DOD currently screens for many of these conditions at numerous points during the military life cycle, and it is implementing structural interventions that support the improved integration of military line personnel, non-medical caregivers, and clinicians, such as RESPECT-Mil (Re-engineering Systems of Primary Care Treatment in the Military), embedded mental health providers, and the Patient-Centered Medical Home. Preventing Psychological Disorders in Service Members and Their Families evaluates risk and protective factors in military and family populations and suggests that prevention strategies are needed at multiple levels - individual, interpersonal, institutional, community, and societal - in order to address the influence that these factors have on psychological health. This report reviews and critiques reintegration programs and prevention strategies for PTSD, depression, recovery support, and prevention of substance abuse, suicide, and interpersonal violence.

The Army has devoted an extraordinary amount of time, attention, and resources to care for Soldiers returning from deployments, especially those with behavioral health conditions. The Army continues to make great strides in changing the culture that stigmatized those with Post Traumatic Stress Disorder (PTSD) and to educate and encourage Soldiers and leaders to heal these invisible wounds of war. The Army has revised several policies to ensure Soldiers with PTSD are properly diagnosed, and if appropriate, considered for a medical discharge. Concerns regarding how the Army treats Soldiers with behavioral health conditions remain high. The ATFBH looked at the IDES process holistically, focusing on the needs of the Soldiers and their Families. The CAP provides 24 findings and 47 recommendations to improve behavioral health diagnosis and evaluation in the context of the IDES. Many of the recommendations seek to improve the IDES as a whole. The findings and recommendations are supported by detailed discussion sections and identify key actions assigned to specific actors required for implementation. The CAP findings and recommendations consist of five focus areas: 1) IDES oversight, training, and tracking; 2) BH issues within the IDES; 3) Reserve Component issues and improvements; 4) Personnel policies that impact the IDES; and 5) Enhancing Support for Soldiers and their Families.
