Book excerpt: While addressing the Medicare financing problem, Congress wrestled with rising health care costs in the private sector and the need to balance the budget. Between 1994 and 1998 Medical Savings Accounts (MSAs) emerged as a controversial policy option. This thesis examines the evolution of MSAs from their inception to legislation introducing them to the private sector and then to Medicare. Data was obtained from congressional reports, periodicals and telephone conversations with a Health Care and Finance Administration (HOFA) analyst. The thesis explains how MSAs became available as an option to Medicare beneficiaries via the Balanced Budget Act of 1997. It details the legislative history of MSAs, identifies the key players involved and explains their incentives. The primary findings were that MSAs are supported by congressional Republicans as a way to reduce Medicare costs by allowing beneficiaries to manage their own health care, financed by a fixed annual contribution from Medicare. The President and many congressional Democrats opposed them because of lost tax revenues and possible damage to the existing health care system. The critical first step to Medicare MSAs was congressional approval of tax free NSAs for small employers via a four year demonstration project in the Health Insurance Portability and Accountability Act of 1996. Medicare MSAs were delayed because insurers failed to file applications to provide them in time. In 1998, MSA supporters failed in their attempt to extend the small empoyer MSA demonstration and to expand MSAs to the Federal Employees Health Benefits Program.

Book excerpt: Excerpt from The Balanced Budget Act of 1997: A Look at the Current Impact on Providers and Patients: Hearing Before the Subcommittee on Health and Environment of the Committee on Commerce, House of Representatives, One Hundred Sixth Congress, Second Session; July 19, 2000 Hospitals have been among the most vocal providers in seeking relief from the bba, m part because so many of their operations were affected by provisions of the law. For inpatient services covered by the prospective payment system, the bea froze base payments in fiscal year 1998 and reduced updates in subsequent years, instituted a new policy for transfer cases, lowered the a 'ustment for indirect med ical education (ime) to teaching hos itals, lowered the a justment received by hos pitals that treat a disproportionate s are (bsh) of low-income patients, and reduced capital payment rates. For outpatient services, the bba eliminated the so-called for mula driven overpayment, extended reductions in payments for capital and services paid on a cost basis, and directed the Secretary to implement a prospective pa ent system for services still being paid at least partially on the basis of costs. Ollec t1vely, these provisions were intended to slow Medicare spending growth, brin in patient payments in line with costs, and move payments for outpatient services m a cost-based system to mmspective one. Book excerpt: The November 1994 midterm elections were a watershed event, making possible a Repbulican majority in Congress for the first time in forty years. Contract with America, by Newt Gingrich, the new Speaker of the House, Dick Armey, the new Majority Leader, and the House Republicans, charts a bold new political strategy for the entire country. The ten-point program, which forms the basis of this book, was announced in late September. It received the signed support of more than 300 GOP canditates. Their pledge: "If we break this contract, throw us out". Contract with America fleshes out the vision and provides the details of the program that swept the GOP to victory. Among the pressing issues addressed in this important book are: balancing the budget, stopping crime, reforming welfare, reinforcing families, enhancing fairness for seniors, strengthening national defense, cutting government regulations, promoting legal reform, considering term limits, and reducing taxes.

Book excerpt: Among the issues confronting America is long-term care for frail, older persons and others with chronic conditions and functional limitations that limit their ability to care for themselves. Improving the Quality of Long-Term Care takes a comprehensive look at the quality of care and quality of life in long-term care, including nursing homes, home health agencies, residential care facilities, family members and a variety of others. This book describes the current state of long-term care, identifying problem areas and offering recommendations for federal and state policymakers. Who uses long-term care? How have the characteristics of this population changed over time? What paths do people follow in long term care? The committee provides the latest information on these and other key questions. This book explores strengths and limitations of available data and research literature especially for settings other than nursing homes, on methods to measure, oversee, and improve the quality of long-term care. The committee makes recommendations on setting and enforcing standards of care, strengthening the caregiving workforce, reimbursement issues, and expanding the knowledge base to guide organizational and individual caregivers in improving the quality of care.

Available in PDF, EPUB and Kindle. Book excerpt: The balance between state and federal health care financing for low-income people has been a matter of considerable debate for the last 40 years. Some argue for a greater federal role, others for more devolution of responsibility to the states. Medicaid, the backbone of the system, has been plagued by an array of problems that have made it unpopular and difficult to use to extend health care coverage. In recent years, waivers have given the states the flexibility to change many features of their Medicaid programs; moreover, the states have considerable flexibility to in establishing State Children's Health Insurance Programs. This book examines the record on the changing health safety net. How well have states done in providing acute and long-term care services to low-income populations? How have they responded to financial incentives and federal regulatory requirements? How innovative have they been? Contributing authors include Donald J. Boyd, Randall R. Bovbjerg, Teresa A. Coughlin, Ian Hill, Michael Housman, Robert E. Hurley, Marilyn Moon, Mary Beth Pohl, Jane Tilly, and Stephen Zuckerman.
