Chapter 55 of Title 10 U.S. Code, entitles certain health benefits to military personnel, retirees, and their families. These health benefits are administered by a Military Health System (MHS). The primary objectives of the MHS, which includes the Defense Department's hospitals, clinics, and medical personnel, are (1) to maintain the health of military personnel so they can carry out their military missions and (2) to be prepared to deliver health care during wartime. Health care services are delivered through either Department of Defense (DOD) medical facilities, known as military treatment facilities (MTFs) as space is available, or through civilian health care providers. As of 2017, the MHS operates 681 MTFs, employs nearly 63,000 civilians and 84,000 military personnel, and serves 9.4 million beneficiaries across the United States and in overseas locations. Since 1966, civilian care for millions of retirees, as well as dependents of active duty military personnel and retirees, has been provided through a program still known in law as the Civilian Health and Medical Program of the Uniformed Services (CHAMPUS), more commonly known as TRICARE. TRICARE has three main benefit plans: a health maintenance organization option (TRICARE Prime), a preferred provider option (TRICARE Select), and a Medicare supplement option (TRICARE for Life) for Medicare-eligible retirees. Other TRICARE plans include TRICARE Young Adult, TRICARE Reserve Select, and TRICARE Retired Reserve. TRICARE also includes a pharmacy program and optional dental and vision plans. Options available to beneficiaries vary by the sponsor's duty status and geographic location. This report answers selected frequently asked questions about military health care, including How is the Military Health System structured? What is TRICARE? National trends indicate substantial increases in the abuse of prescription medications. The problem of substance abuse within the military has come under new scrutiny in the context of the two concurrent wars in which the United States has been engaged during the past decade-in Afghanistan (Operation Enduring Freedom) and Iraq (Operation Iraqi Freedom and Operation New Dawn). Increasing rates of alcohol and other drug misuse adversely affect military readiness, family readiness, and safety, thereby posing a significant public health problem for the Department of Defense (DoD). To better understand this problem, DoD requested that the Institute of Medicine (IOM) assess the adequacy of current protocols in place across DoD and the different branches of the military pertaining to the prevention, screening, diagnosis, and treatment of substance use disorders (SUDs). Substance Use Disorders in the U.S. Armed Forces reviews the IOM's task of assessing access to SUD care for service members, members of the National Guard and Reserves, and military dependents, as well as the education and credentialing of SUD care providers, and offers specific recommendations to DoD on where and how improvements in these areas could be made.

To help alleviate this problem, a three- year demonstration project known as Medicare Subvention has been implemented creating a Department of Defense Health Maintenance Organization called TRICARE Senior Prime (TSP). This research determined the financial impact of TSP at Naval Medical Center, San Diego (NMCSD). The financial analysis includes an examination of inpatient and outpatient costs and revenues for Fiscal Year (FY) 1999. The results of this research show that TSP indirectly caused a net reduction. in operating costs at NMCSD in FY 1999 of $ 1.5 million or one percent of the operating budget. The program caused revenues to decline by causing a 20 percent reduction in admissions for non-TSP beneficiaries with other health insurance. Proportionally larger cost reductions resulted from this decrease in non-TSP admissions that outweighed increases in TSP admissions.

This report evaluates the early implementation of TRICARE Senior Prime at six MTFs, financed by subvention (the transfer of funds from HCFA to DoD). It finds that enrollment rates in Senior Prime were faster than in many private health plans and beneficiaries were generally pleased with their care. However, sites are having difficulties caused by the complexity of payment methods and by uncertainty over whether they will ever see Senior Prime revenues--even if DoD obtains net payments from HCFA after each year's reconciliation. If a larger Senior Prime program becomes a permanent part of TRICARE, the following policy issues must be addressed: (1) balancing interactions between the readiness mission and Senior Prime; (2) structuring and managing Senior Prime effectively; (3) creating the desired financial incentives; and (4) achieving effective clinical and cost performance. Lessons learned include the importance of effective introduction of the program and orientation of beneficiaries; early identification of enrollees with existing treatment requirements; the importance of coordination of intake activities and follow-up visits; and the difficulty many beneficiaries have with automated appointment and customer service systems. In addition, DoD's health data system capabilities will need to be improved.

The goal of the demonstration is to implement a cost-neutral alternative to delivering health care to Medicare-eligible military beneficiaries, while increasing the total federal cost to neither the Department of Defense (DOD) nor Department of Health and Human Services. The purpose of this research effort is to determine if MAMC can achieve cost-neutrality and contribute to the achievement of the DOD Level of Ef fort (LOE). Two supporting objectives are examined in this research effort. The first seeks to determine if the cost of treating Medicare dual-eligible beneficiaries is significantly greater than the cost of treating all other beneficiaries. The second examines whether MAMC will meet its assigned LOR based on accrued costs for care rendered to dual-eligible beneficiaries. Cost-neutrality is achieved if the LOR is met but not exceeded. Results indicate MAMC will meet and exceed its assigned LOE, but will not achieve cost-neutrality. Results also revealed that costs for Medicare patients are greater (significantly) . Information and methods presented in this study may assist health care decision-makers in innovating methods for controlling the associated costs.
