L'objectif de la reconstruction mammaire est double. Il est d'abord et avant tout psychologique. Mais il est également morphologique en se fixant comme objectif la restauration d'un sein de volume, de forme et de consistance la plus proche possible du sein controlatéral. Deux alternatives s'offrent au chirurgien, la reconstruction mammaire par implants qui utilise des prothèses afin de restaurer le volume uniquement, et la reconstruction mammaire autologue qui utilise les tissus de la patiente pour assurer le volume et la couverture. Il s'agit le plus souvent des lambeaux de grand droit de l'abdomen (rectus abdominis) et de grand dorsal (latissimus dorsi). Nous avons analysé les résultats obtenues par reconstruction mammaire après mastectomie chez 116 patientes opérées par le même chirurgien entre 1996 et 2002 au sein de l'unité de chirurgie plastique du centre hospitalier universitaire de Grenoble .L'étude concerne uniquement les reconstructions par implants et celles par lambeau de grand dorsal sans prothèse. L'analyse a porté sur des critères morphologiques permettant ainsi de juger la qualité de la reconstruction et sur les principales complications de chacune des techniques ainsi que sur le taux global de réinterventions. La reconstruction de la plaque aréolo-mamelonnaire qui est réalisée selon la même technique n'a pas été évaluée. Les résultats ainsi obtenus montrent un net avantage à la reconstruction autologue principalement par absence de contractures périprothétiques, qui viennent détériorer les résultats des reconstructions par implants, et par une meilleure évolution dans le temps avec un taux d'asymétrie plus faible. Ces deux facteurs contribuent à un taux de réinterventions bien plus élevé en cas de reconstruction prothétique. Dans le cadre des reconstructions autologues, le lambeau de grand dorsal peut être considéré comme une alternative intéressante au lambeau de grand droit de l'abdomen de part sa fiabilité, sa relative simplicité d'exécution et ses suites opératoires moins contraignantes. Cette alternative thérapeutique permettra certainement en adaptant au mieux l'indication chirurgicale à chaque patiente d'obtenir de meilleurs résultats et à long terme.
