Book excerpt: Maintaining marginal bone level is a key factor that contributes to the preservation of peri-implant soft tissues and to the prosthetic success. The aim of this study was to systematically review the combined effect of platform switching with implant placement depth on marginal bone loss.An electronic search was conducted up to March 2017. One-piece implants and immediate post-extractive implants were excluded. The mean differences between groups were calculated using the inverse variance method with a random effects model.Twenty studies were included in the meta-analysis. More marginal bone loss was observed around platform-matched implants at 12 months after prosthetic loading (-0.38mm (-0.49; -0.27)). Among platform-switched implants, more marginal bone loss was observed around implants placed subcrestally (-0.67mm (-1.31; -0.03)).Further studies are needed to compare marginal bone loss around (1) platform-matched implants placed at different depths and (2) platform-switched implants placed at crestal versus supracrestal levels.

Book excerpt: Background: Functional and dimensional maintenance of the soft tissue seal in dental implants can determinate better long-term condition, and an adequate amount of bone around the implant reduces the risk of the abutment loosening from the implant. Aim: The present animal study aimed to analyze the possible relationship between the combination of different platform switching and vertical implant depth and remodeling of the crestal bone. Material and Methods: Sixty conical Morse taper dental implants were placed in the tibia of 15 rabbits, hence 2 implants per tibia. Three different implant diameters were used, 3.5-mm, 4.0-mm and 4.5-mm (n=3x20 implants). The implants were placed either 1-mm or 2.0-mm subcortically, based on a randomized allocation. A transmucosal healing abutment with a diameter of 3.3 mm and height of 3.5 mm was secured to the implants. Eight weeks after the implantation surgery, histological samples were prepared and analyzed quantitatively. The extent of new bone formation was appraised by two measurements. I.e., the relative area of new bone formed between the osteotomy wall and the vertical surface of the abutment (%), and the height from the implant shoulder to the most cervical crest of the new bone (mm). Parametric and nonparametric statistical tests were applied to assess the significance of observed group differences (n+6x10 implants). Results; New bone formation between the osteotomy wall and the vertical surface of the abutment cervically to the implant shoulder was observed in all 6 groups. The highest and lowest new bone formation measured by the bone height was observed respectively for the 3.5 mm implants placed 2 mm subcrestally (1.2 mm) versus the 4.0 and 4.5 mm implants placed 1 mm subcrestally (0.6 mm) (p120%) and the 4 and 4.5 mm (~55%) implants. Conclusions: In this animal model, both the implant diameter and vertical placement level influence the crestal bone behavior during the healing process.

Otherwise, subcrestal placement of short implants is advised since it is considered as a crucial factor in preservation of crestal bone. For such scenario, bone biomechanical state is directly dependent on implant insertion depth. The aim of the study was to compare the impact of crestal or subcrestal short plateau implant placement in different bone quality conditions on peri-implant bone stresses and to assess implant prognosis under 120.92 N mean maximal oblique functional loading.5.0u00d75.0 mm Bicon Integra-CPu2122 implant was selected for this comparative study. Its 3D models were placed in four posterior maxilla models with types III and IV bone with 1.0 mm cortical bone thickness. Different insertion depths were simulated: the implant neck was in crestal (C) and -1, -2 and -3 mm subcrestal (S1, S2, S3) positions. All materials were assumed to be linear elastic and isotropic. Elastic moduli of cortical, type III/IV cancellous bone and implants were set to 13.7, 1.37/0.69 and 114 GPa, and Poissonu2019s ratio was 0.3 for all materials. Finite element (FE) models were analyzed in Solidworks Simulation software. 4-node 3D FEs were generated with a total number of up to 3,570,000. 120.92 N mean maximal oblique load (molar area) was applied to the center of 7.0 mm abutment. Von Mises equivalent stress (MES) distributions in surrounding bone were studied to determine the areas of bone overload with magnitude >100 MPa in cortical and >5 MPa in cancellous bone.Maximal MES magnitudes for all scenarios were found at the implant neck. For C scenario, maximal MES magnitudes were found in crestal cortical bone: 21/28 MPa (type III/IV bone). Maximal MES magnitudes in cancellous bone were approximately 5 MPa for both bone types. For S1, S2 and S3 scenarios, since there was no contact between implant and cortical bone, maximal MES magnitudes were located in cancellous bone at the implant neck. For S1 scenarios they were 18u202620 MPa, for S2 and S3 scenarios they were 13u202614 MPa, for both bone quality types. Another critical MES area was located at the implant root: for C scenario, maximal MES magnitudes were 2 MPa for both bone quality types, for S1 scenario they were 2.5/3.5 MPa, for S2 u2013 3.5/4.0 MPa and for S3 u2013 7/10 MPa (for type III/IV bone).It was found that 5.0u00d75.0 mm Bicon Integra-CPu2122 implant in crestal placement generated safe bone MESs and offered favorable clinical prospect. For all subcrestal scenarios, the implant caused exceeding MESs in cancellous bone due to absence of interface between the implant and cortical bone. However, the tested Bicon implant showed low susceptibility to bone quality worsening at studied levels of subcrestal placement. This finding confirms positive clinical experience of Bicon plateau implants.

Book excerpt: Abstract:Background: The aim of this study was to assess the clinical and radiographic outcome of a new, bone level tapered dental implant with a hydrophilic surface when inserted either following an immediate or early placement protocol. Furthermore, the behavior of the implant stability quotient (ISQ) at different time-points and the aesthetic outcome was analyzed.Material and methods: Patients in need of tooth extraction and replacement by a single implant in the anterior to premolar areas of their maxillae or mandibles were recruited. The patients were randomly assigned to either the immediate or the early placement group. 4-6 weeks after implant placement, the impression was performed. 8-10 weeks after surgery, the implants were loaded with the final crown. The clinical and radiological outcomes were assessed. Furthermore, the behavior of the implant stability quotient at different time-points and the aesthetic outcome was analyzed.Results: All 20 implants could be loaded according to the protocol within 8-10 weeks after implant placement. At the 12 months follow-up visit, all 20 implants were osseo-integrated and the survival rate was therefore 100%. The ISQ values of all 20 implants showed a trend towards a gradual increase from 69.6 (buccal) and 72.7 (mesial) directly after implant placement to 73.55 (buccal) and 76.2 (mesial) at impression taking and 75.95 (buccal) and 78.4 (mesial) at loading. During the follow-up visits, the mean distance from implant shoulder to the first bone-to-implant contact was 0.32 mm after 6 months and 0.35 mm after 12 months, respectively. At the 12-months examination, the mean bone loss in the immediate group was 0.55 mm and 0.18 mm in the early placement group.Conclusions: The short-term, preliminary data of this clinical study demonstrate high survival rates and excellent clinical outcomes. Bone level tapered implants placed by means of both immediate and early implant placement protocols exhibit similar survival and success rates, changes in the crestal bone level and esthetic outcomes 12 months after loading.

Another method suggests to increase soft tissue thickness by placing implant at the bone level with 2 mm height healing abutment and suture soft tissues over.The aim of the study was fourfold - to register crestal bone stability around (1) 1.5 mm subcrestally placed implants; and (2) epicrestally placed implants with 2 mm healing abutments; to (3) register bone remodelling in subcrestal group; (4) determine how vertical soft tissue has increased in epicrestal group, compared to initial situation.24 patients with vertically thin tissues of 2 mm or less received 24 submerged bone level platform-switched implants (Conelog, Camlog, Basel, Switzerland). Implants were divided into 2 equal groups u2013 (1) 1.5 mm subcrestally placed implants and (2) epicrestal implants with 2 mm healing abutments. At II stage surgery, all implants received 4 mm healing screws and later restored with Zr screw-retained single crowns, radiological images taken at II stage surgery and after 1 y follow-up. Bone loss was calculated as a distance between implant neck and first radiographically visible bone-to-implant contact and measured in both groups. Additionally, in subcrestal group bone remodelling was calculated as a distance between bone crest and implant neck. Mann-Whitney U-test was used for statistical analysis, significance set to 0.05.After 1 y follow-up, 1 (subcrestal) group had 0.06 (0.07 SD) mm of bone loss, while 2 group (epicrestal with 2 mm healing abutment) had 0.11 (0.14 SD) mm of bone loss, with statistically significant difference (P=0.005). Bone remodelling in group 2 (subcrestal) was 0.55 (0.3 SD) and was significantly larger compared to bone loss of subcrestal (P=0.001) and epicrestal groups (P=0.003). Vertical tissue thickness in epicrestal group before intervention was 2.30 (0.26 SD) and 3.65 (0.41 SD) after use of 2 mm healing abutment, with statistical difference (P=0.005).The novelty of the study is that crestal bone loss and bone remodelling as discussed as separate outcomes. Bone loss was calculated as a distance between implant neck and first radiographically visible bone-to-implant contact, while bone remodelling was calculated as a distance between bone crest and implant neck. Both groups had only minor crestal bone loss and could be used as a method to reduce bone loss in vertically thin tissues. However, it is interesting to note that bone remodelling in subcrestal group was larger than bone loss in both groups. In addition, the use of 2 mm healing abutment, as a method to increase soft tissue thickness and reduce bone loss was established.

Radiographic bone loss stratified by restorative status changed by -1.2 ± 1.0 for the restored group, and -1.9 ± 1.5 for the not restored group. Book excerpt: Statement of problem: The peri-implant bone level has been used as one of the criteria to assess the success of dental implants. Bone resorption was accepted as a physiologic response to second-stage implant uncovering An average of 1.2 mm of bone loss around submerged two-piece implants in the first year following restoration was noted. The concept of platform switching was proposed to reduce crestal bone loss around implants. Purpose: This study was attempted to study the influence of platform switching on the marginal bone level alterations around implantsMaterials and Methods: 60 implant sites were divided into two groups i.e a test group and a control group. The test group comprised of platform switched implants and the control group comprised of platform matched implants Radiologic assessment was done at varying time intervals after uncovering of the implant.Results: A Comparison of mean bone loss between platform matched and platform switched abutment as noted on the mesial side showed a non-significant difference (p value=0.92) while on the distal side statistically significant difference was noted. Comparison of the results between 2 groups at 1,2 4,6 months of post abutment placement on the distal side showed a statistically significant difference. (p value=0.024).Conclusion: It was noted that bone loss is more in platform matched abutments as compared to platform switched abutments. It can thus be concluded that platform switching results in peri-implant bone preservation and also has esthetic benefits.

The amount of bone gain in area of jumping distance and the changes in buccopalatal bone width was assessed . Measurements of the peri-implant mucosa was done at implant placement, abutment placement and 1 month, 3 month, 6 month post abutment placement Results: Statistical analysis showed a bone gain at the time of stage II surgery (P = .048). 97.19% bone gain at the time of stage II surgery and was statistically significant. Platform matched abutments elicits statistically insignificant change in mesial and distal papilla level and buccal peri-implant mucosa level at different time interval.(p u2265 0.05). Platform switched abutment elicits statistically significant change in mesial and distal papilla level and buccal peri-implant mucosa level at different time interval. Conclusion: This study suggests that platform switching can provide increase in mesial and distal papilla and buccal peri-implant mucosa level along with bone remodeling around implant.
