The purpose of this evidence-based practice (EBP) change project was to implement Purpose: The purpose of this evidence-based change project was to provide diabetes self-management education (DSME) to participants in a federally qualified health clinic (FQHC) to increase patient knowledge of diabetes self-management behaviors and decrease fasting blood glucose levels. Methods: Thirteen adults with uncontrolled Type 2 Diabetes; able to read, write, and speak English; and who had access to a telephone and a computer or tablet with a stable internet connection were voluntarily recruited at a FQHC. Four bi-weekly DSME one-on-one virtual sessions lasting one hour occurred over a nine-week period, with a 30-minute follow-up phone session during weeks when DSME was not scheduled. Using a pretest/posttest approach, participants' diabetes self-management knowledge, using the Revised Michigan Diabetes Knowledge Scale (RMDKS), and fasting blood glucose were measured before and after the educational intervention session. Statistical analysis utilized a paired t-test for comparison. Results: The data showed a statistically significant post-intervention improvement in both patients' diabetes knowledge with a mean pretest knowledge score of 60% and mean posttest score of 77% (p

According to copious amounts of research studies, it has been found that for the patient to be successful in diabetic self-care management and to achieve within normal limit HgA1c levels frequent diabetic education and follow-up is necessary. In order for nurses to provide competent education to the diabetic patient, the nurse must first self-assess their knowledge and seek out education on diabetes management in areas lacking. The proposed implementation plan will include required education staff meetings and online continuing education credit courses for the specialized diabetic nurse. Establishing and implementing an education plan for nurses and the patient is key in achieving quality patient outcomes. The education implementation plan includes a preassessment of diabetic knowledge of the nurse and patient, education provided to staff nurses, an outlined diabetic teaching plan for patients with monthly follow-ups over a six-month period of time, monitoring of HgA1c levels throughout teachings, and post-assessment of the patient and nurse. A six-month pilot study prior to policy and procedure change will support the need for frequent teachings and follow-up. By providing the patient with current evidence-based education, the diabetic patient can ultimately be more successful in managing their type 2 diabetes.

This research reviews the impact of diabetes education for patients with type II diabetes and then presents a project for the presentation of diabetic education and importance of adherence to a volunteer group of type II diabetic participants from randomly selected churches in a central Louisianaa community. Sixteen people participated in the educational intervention. Ager each participant signed a consent form, and completed a survey for demographical information, a pretest was given before a sixty minute lecture/discussion on diabetes type II and importance of adherence to medical treatment. After completion of the discussion, a posttest was given to evaluate the effectiveness of the program. Eight weeks ofter the program, a telephone survey was completed. The intervention showed that diabetes education in successful in helping patients adhere to their medical regimen.
