Les enfants présentent des niveaux extrêmes d'hyperactivité, d'impulsivité et d'inattention. Ils ont des difficultés à l'école et dans leur relation avec les autres et ont une faible estime d'eux même. Le trouble va souvent persister à l'adolescence et à l'âge adulte. Le méthylphénidate (psycho-stimulant) est le traitement le plus souvent utilisé. Le nombre de prescriptions ne cesse d'augmenter et ce depuis les années 90. Dans cette étude nous allons d'abord passer en revue les critères diagnostiques, les facteurs étiologiques et les traitements de ce trouble. Nous présenterons ensuite nos résultats concernant l'étude de facteurs génétiques et environnementaux associés à la consommation du méthylphénidate, obtenus sur une population d'enfants âgés de 6 à 12 ans et présentant le trouble.

Book excerpt: Objectif : Etudier les facteurs associés à l’indication de méthylphénidate dans une population trouble déficit de l’attention/hyperactivité (TDAH) en faisant l’hypothèse qu’ils sont liés à la sévérité du trouble. Evaluer la pratique professionnelle au sein de notre unité de pédopsychiatrie selon l’evidence based medecine. Méthodes : Nous avons recueilli pour notre étude les données de 151 patients évalués sur l’unité de soin et de diagnostic 6-12 ans du service de médecine psychologique pour enfant et adolescent secteur 1 (MPEA-1) St Eloi à Montpellier. Ils ont bénéficié d’une évaluation avec entretien diagnostic Kiddie-SADS, échelle de sévérité des symptômes ADHD-RS, questionnaire SDQ parents et enseignants, bilan cognitif avec le WISC-IV et bilan attentionnel TEA-Ch. Parmi les 141 patients étudiés, nous avons comparé les patients avec indication de traitement par méthylphénidate (88 patients) et les patients sans indication de traitement par méthylphénidate (53 patients). Résultats : Ainsi nous avons déterminé les caractéristiques cliniques et neuropsychologiques des patients qui interviennent dans la décision de traiter par méthylphénidate. Nos résultats correspondent aux recommandations internationales qui préconisent de traiter par méthylphénidate quand le trouble est sévère. Nos résultats montrent une association significative entre l’indication de prescription du méthylphénidate et la sévérité du trouble : présentation mixte du TDAH, comorbidités TOP et trouble des apprentissages type mixte, nombre total de comorbidités, nombre d’items à la Kiddie-SADS, scores à l’ADHD-RS, les scores totaux de difficultés, les scores de trouble du comportement et d’hyperactivité inattention et le retentissement à la SDQ parents/enseignants, et l’indice de raisonnement perceptif au WISC-IV. L’analyse multivariée montre que plusieurs variables ont un impact sur la décision de traiter, la présentation mixte, la présence d’un TOP et/ou d’un trouble de l’acquisition des coordinations et également la sévérité du trouble avec le score anormal d’inattention à l’ADHD-RS. Conclusion : L’augmentation de prescription de méthylphénidate s’inscrit dans une réalité clinique et thérapeutique d’amélioration du dépistage et de la prise en charge du TDAH de l’enfant.

Cette étude explore l'impact du traitement par méthylphénidate sur la progression sur un an dans les apprentissages des enfants ayant un TDA/H et des troubles spécifiques des apprentissages avec déficit en lecture et expression écrite graves intégrés dans l'Unité de Rééducation Neurologique Infantile de l'hôpital Bicêtre. Les enfants intégrés dans l'Unité ces 4 dernières années ayant un TDA/H ont été inclus et séparés en 2 groupes : traités et non traités par méthylphénidate. La progression est évaluée sur un an par le gain en mois d'âge développemental entre le début et la fin de l'année (évaluation par l'EDA, l'ALOUETTE, la BATELEM). Dans le groupe traité, la progression en compréhension de texte est significativement plus importante (significativité persistante en sous-groupe chez les femmes, les enfants sans hyperactivité, sans trouble du langage oral) ; il n'est pas trouvé de différence en leximétrie, transcription, mathématiques, par rapport au groupe non traité. Cette étude présente plusieurs limites, dont un petit effectif ayant pu impacter nos résultats, mais il apparait nécessaire de dépister systématiquement un TDA/H devant des troubles spécifiques des apprentissages, afin de proposer rapidement un traitement médicamenteux pour favoriser la progression, sans banaliser les difficultés attentionnelles présentes.

Book excerpt: Le Trouble du Déficit de l'Attention avec Hyperactivité (TDAH) est l'un des troubles neurodéveloppementaux les plus répandus chez l'enfant et l'adolescent et peut persister à l'âge adulte dans environ un tiers des cas. Le méthylphénidate, stimulant couramment utilisé dans le traitement du TDAH, a reçu l'Autorisation de Mise sur le Marché (AMM) pour les adultes en France en 2021. Cette thèse se concentre sur les risques potentiels associés à l'exposition au méthylphénidate pendant la grossesse et l'allaitement, ainsi que sur les implications d'un traitement pharmacologique parental sur la dynamique parent-enfant. Concernant l'exposition in utero, les résultats indiquent un risque accru de prématurité et une corrélation faible avec une augmentation des risques d'anomalies cardiaques, sans qu'il y ait une hausse significative du risque global de malformations congénitales ou de mortalité périnatale. Ces découvertes soulignent l'importance d'une prise en charge interdisciplinaire, intégrant un soutien psychologique et un suivi obstétrical approfondi pour garantir le bien-être de la mère et du fœtus. Les études analysées montrent que l'exposition du nourrisson au méthylphénidate via le lait maternel est minimale. Cependant, les données restent insuffisantes pour déterminer le passage lacté de ce médicament et d'autres recherches sont nécessaires pour évaluer ses impacts à long terme durant l'allaitement. La dernière partie de cette thèse examine les travaux du Dr Chronis-Tuscano sur les effets du traitement du TDAH sur les interactions parent-enfant, et l'importance des interventions comportementales adaptées pour améliorer la gestion des symptômes du TDAH. Bien que cette revue de la littérature suggère que les risques associés à l'usage du méthylphénidate chez les patientes enceintes ou allaitantes sont généralement faibles, ces résultats doivent être interprétés avec prudence. D'un point de vue clinique, chaque cas doit être considéré individuellement, avec des dispositifs de surveillance et d'accompagnement mis en place tout au long de la grossesse et du post-partum. D'un point de vue pharmacologique et épidémiologique, une vigilance continue est nécessaire.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
