La contamination croisée ou contamination d'un produit par un autre, est un risque qui génère des conséquences sur la sécurité du patient et pour l'entreprise mise en cause. La gestion du risque de contamination croisée a pris une importance croissante au cours des dernières années. Les BPF ont notamment été mises à jour en 2015 dans le but d'améliorer la prévention des contaminations croisée. Une démarche rigoureuse de gestion des risques Qualité et l'utilisation d'outils adaptés selon l'ICH Q9, sont alors nécessaires pour réduire ce risque. Les sources de contamination croisée sont multiples et peuvent survenir à toutes les étapes de la vie du médicament. Elles peuvent être liées à des défaillances d'origine technique ou organisationnelle : problèmes de nettoyage, de conception des locaux, de maîtrise des flux, de vide de ligne... Un outil de gestion des risques tel que l'AMDEC, ciblant un procédé en particulier, permet d'identifier efficacement les risques selon leur criticité. Suite à cette analyse du risque, des moyens de maîtrise de la contamination croisée sont mis en place par les industriels, tout en se conformant aux exigences des BPF. À travers cette étude, le risque de contamination croisée au sein d'un service de conditionnement sera abordé plus en détails.

Afin de contrôler cette contamination, il est nécessaire de mettre en place un système de gestion du risque qualité. Ce système a pour rôle d'identifier les risques de contamination et les évaluer afin que l'on puisse mettre en place des actions correctives ou préventives. Ces actions vont permettre de maîtriser ce risque en le réduisant, en le limitant ou en le supprimant. Il y a différents moyens de maîtriser ce risque. Ces moyens peuvent être techniques ou organisationnels. Il est possible de classer ces moyens en fonction de la méthode des 5M d'Ishikawa : on pourra ainsi distinguer les moyens concernant la gestion des matières premières, la gestion de la main d'oeuvre, l'organisation du milieu, l'utilisation des méthodes et du matériel appropriés. Malgré la mise en place de ces moyens de gestion du risque, le risque subsiste toujours. Deux cas concrets survenus dans l'industrie pharmaceutique sont décrits.

Le vide de ligne concerne toutes les phases de fabrication d'un médicament et les sources d'erreurs sont nombreuses. Une défaillance peut conduire à l'apparition d'une contamination croisée, contamination d'une matière ou d'un produit par un(e) autre, et engendrer des conséquences graves. On parle alors d'une non-conformité. Cette thèse est composée de trois parties. La première présente les généralités sur le vide de ligne, notamment ce qui entoure sa pratique en industrie pharmaceutique (équipement, documentation et personnel) et sur les contaminations croisées. La seconde aborde les difficultés liées au vide de ligne et ce qui est mis en place pour maîtriser l'apparition de non-conformités. Enfin, la dernière traite d'un cas concret d'investigation sur une contamination croisée liée à une défaillance du vide de ligne.
