Book excerpt: This volume of the Italian conference summarizes and integrates a substantial body of knowledge that has arisen in recognition of the discovery that the inflamed middle ear and the normal inner ear are immunocompetent. Its theme - that the immune system in the inner and middle ear can respond to infectious agents or to self antigens to either resolve disease or prolong it - underlies the comprehensive reviews provided on such subjects as: humoral and cellular immune response in the nasopharynx, the middle ear and inner ear; immune mechanisms in several inner ear disorders; the genetics of inner ear immunologic disorders; and the role of viral infection in middle and inner ear diseases. Also discussed are the homing of lymphocytes into the middle and inner ear and potential sources of these immunocompetent cells. This volume seeks to show the importance of understanding the immunologic processes involved in otitis media and inner ear disease and for developing vaccines against these diseases.

Book excerpt: It is with great pleasure that I write this Foreword to the Proceedings of the International Conference on Behçet’s Disease which was held in Berlin in June 2002. This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.

Available in PDF, EPUB and Kindle. Book excerpt: Cholesteatomas are bone destructive expansions of keratinizing squamous epithelium in the middle ear and temporal bone. Today, surgery is the only treatment. There are several controversies regarding cholesteatomas, including the definition, the pathogenesis and the surgical method. Intense efforts have been made searching for a comprehension of the cholesteatoma process at a cellular and molecular level. Recurrent infections and inflammation seem to be contributing factors for the cholesteatomas to expand. The innate immunity, essential to keep a healthy middle ear environment and to protect the middle ear from intruding pathogens, is therefore a matter of interest. In this thesis, results are presented from a cohort of cholesteatoma surgeries in Östergötland from a 16-year period. A group of patients also filled in a questionnaire to assess changes in health-related quality of life (HRQoL) after surgery. According to the findings in this thesis, the residual and recurrence frequencies are low, and the hearing and HRQoL are improved in the majority of cases. This thesis also presents an investigation of the innate immunity in ears with acquired cholesteatoma, in comparison with healthy controls. The expression of mRNA of toll-like receptors 2 and 4, participants of the Janus kinase/signal transducer and activator of transcription pathway, and nitric oxide synthases in middle ear mucosa, were investigated with quantitative polymerase chain reaction. An investigation of nitric oxide (NO) in the middle ear, with chemiluminescence measurements, is also presented. A derangement of the innate immune system is seen in ears with cholesteatoma, which supports the idea that the innate immunity participates in the cholesteatoma process, though the underlying mechanisms are still unclear. The suggestion of NO production in the middle ear sheds light on NOs possible participation in the healthy middle ear environment.

Available in PDF, EPUB and Kindle. Book excerpt: Until recently, the contribution of immunological knowledge to the under standingand management ofENTdisorders was slight, being largely confined to the appreciation that many rhinitic patients were allergic. Happily, this situation is rapidly changing: the immunological basis of many disorders of the ears, nose and throat is becoming recognized and the mechanisms of the reactions involved are being elucidated. From this, rational therapy should evolve. This book aims to highlight some of the areas in which immunological mechanisms are involved in otorhinolaryngology. It is written by experts in their respective fields of immunology and allergy, otology, rhinology and pathology. It opens with an overview ofthe pathways ofthe immune response and the cells and molecules involved, leading to an appreciation of the normal defence mechanisms of the upper respiratory tract and possible areas offailure. There is then a chapter on HIV infection and how this may present to otorhinolaryngologists. The normal function of the tonsil and the immunological effects oftonsillectomy are then considered. The varying roles of fungi in ENT disorders ranging from commensal through allergen to invasive organisms is assessed by Professor R. J. Hay. Perhaps the most obvious immunological contribution to management thus far lies in the immunocytochemical diagnosis of pathological conditions of the ears, nose and throat and this is covered in a chapter by Professor Leslie Michaels.

Book excerpt: The immunology of mucosal surfaces is one of the most exciting and relevant areas of medical veterinary and dental research since it applies basic research to tissues in volved in everyday defence against microbes and against environmental and food antigens. This book is based on the contributions presented at the International Con gress of Mucosal Immunology, held in London in July 1989 and organised by the Mu cosal Immunology Affinity Group of the British Society for Immunology. The meet ing was attended by over 500 delegates from 27 countries, including virtually all of the leading investigators in the field. The contents give comprehensive and up-to date information on such topics as antigen presentation and processing in the gut, mucosal vaccines in man and animals, HIV infection in the gut, the role of yo T cells in the gut epithelium, recent advances in inflammatory bowel disease and coeliac dis ease, the role of cytokines in the regulation of the IgA response, mucosal mast cells and cell migration. The contributions reflect the rapid pace of research in mucosal immunology, and the great strides which are taking place in the understanding of the immunology, molecular biology and biochemistry of host response at mucosal sur faces.
