A screw-retained provisional crown was fabricated chairside immediately after implant placement. Objective evaluation of implant esthetics was performed after a mean follow-up of 1.2 years using the Pink Esthetic Score (PES) and compared to the baseline PES.Results: The mean PES of evaluations at the first assessment (n=22) was 11.41 (+/-1.81 SD), and 12.14 (+/-1.52 SD) at the second one. The difference between these two means was not significant statistically (P=0.6882). Conclusions: Immediate implant placement and restoration appear to be a valid treatment option when using guided surgery to obtain satisfactory esthetic results.

Book excerpt: Objective: To evaluate the aesthetic outcome of immediate dental implantation and immediate non-functional loading utilizing definitive abutments in the aesthetic zone, with and without bony substitutes filling the peri-implant gap.Methods: in this prospective clinical trial, 12 implants were placed in 11 subjects (6 males, 5 females) utilizing a flapless immediate post extraction approach in the esthetic area of the maxilla (15 to 25). All participants were above 21 years old and medically fit. Atraumatic extraction was performed for teeth that were indicated for extraction keeping an intact facial bone plate. Implants were placed free hand with a gap of at least 2 mm from the inner surface of the facial plate bucco-lingually, and in the range of 3u20134 mm from the future gingival margin in a corono-apical direction (Buser et al. 2004). The gap between the implant and the inner surface of the facial plate was either left without grafting or filled with natural bovine bone mineral granules based on a coin toss method regardless of the size of the gap. Upon achieving a torque of uf0b3 30 Ncm, immediate non-functional loading was performed utilizing a definitive abutment as in a one-abutment-at-one-time protocol (Canullo et.al 2010). Definitive crowns fabrication was performed 12 weeks post implant placement. Pink Esthetic Score (PES) (Furhauser et al. 2005) was assessed preoperatively (PES0), 1 year after delivery of definitive crowns (PES1), and at 2 years follow up (PES2). Dental casts were made at those points of time as well. Those casts were scanned using an imes-icore table-top scanner, and the scans were registered using surface to surface registration and then signed closest point distances were measured. Surface models were cropped to include one tooth mesial and distal to the region of interest (implant site) in an attempt to remove the outliers that may result from differences else were on that cast. All image analysis steps were done using Slicer CMF. Appropriate statistical analyses were performed.Results: The range of the gap left between the implant and the inner surface of the facial plate varied from 2mm to 4mm. All implants remained integrated and restorations were functional at the end of a two year follow up period. The means of PES score were as following: PES0= 9.5uf0b11.68, PES1=9.5uf0b11.98, PES2=9.83uf0b12.89. There were no statistically significant differences in PES scores at different points of time both collectively (P=0.08 for PES0 vs PES1, P=0.14 for PES0 vs PES2, and P=0.61 for PES1 vs PES2), and when compared for the grafting group versus non-grafting group (P=0.24 for PES2 graft vs. PES2 non-grafting). As for casts scans, the surface distances between the 2 time points for all cases were less than 1 mm in all reference planes and there were no statistically significant differences. This reflects a stable surgical outcome of this approach.Conclusions: Immediate placement and immediate non-functional loading utilizing a definitive abutment in a one-abutment-at one-time protocol in the esthetic area of the maxilla appears to result in a favorable and stable result as far as esthetic outcome, and alveolar process sufficiency are concerned. It seems grafting the horizontal gap has no additional benefit in promoting better clinical outcomes given that the provisional restoration is meticulously fabricated and strict selection criteria are applied for such cases. Of notice is the limitation present in this study as the sample number is small as this is a pilot clinical trial.

Both implants were immediately restored with non- functional, provisional restorations. A 10-year follow up of the case will be presented.Materials and MethodsA female patient, aged 19 years old, underwent orthodontic therapy, during which teeth 12 and 22 presented with root resorption. Both teeth were scheduled to be extracted and replaced with dental implants. Tooth #12 was extracted at a prior date by her general dentist 6 months prior to implant placement. Tooth #22 was extracted on the day of surgery, and two implants were placed. Bovine bone substitute (Bio-Oss) was placed to cover the gap between the body of the immediately placed implant and the buccal bone. Subsequently, the implants were immediately restored with temporary crowns, non-functionally loaded. ResultsFinal restorations were placed 4 months later, after successful osseointegration of the implants was confirmed. Clinical and radiographical examinations revealed that functional and esthetic results have withstood the test of time. No soft tissue recession was observed at the 10-year follow-up. The appropriate preoperative evaluation of the morphology of the alveolar process and the periodontal biotype, the surgical planning which provided a guide for proper implant placement, management of the peri-implant gap and the peri-implant soft tissues led to a successful result.ConclusionImmediate implant placement along with immediate provisional restoration in the maxillary anterior is a predictable approach with good long-term results, as long as proper treatment planning is performed.

Therefore, we compared esthetic outcomes of platform-switching implants to regular platform implants in anterior maxillae.AimTo compare the influence of platform switching or regular platform implants on aesthetic outcomes in the maxillary anterior zone by pink esthetic score and white esthetic score.Materials and MethodsPatients who had received single crown restoration with implant in the anterior maxilla from June 2014 to June 2016 were enrolled. At the time of final crown insertion, the standardized clinical images including a frontal full view image, an occlusal partial view image and a frontal partial view image were collected. Three clinicians who were not participated in the treatment procedures evaluated esthetic outcomes through the images using the pink esthetic score and white esthetic score (PES/WES).ResultsA total of 27 implants meeting the inclusion criteria were recruited and divided into two groups: platform-switching group (n=11) and regular platform group (n=16). According to the professional assessments, PES in platform-switching group was 6.33u00b10.78, while PES in regular platform group was 5.77u00b11.25, which was not statistically different (P=0.238). WES in platform-switching group was 6.56u00b11.32, while PES in regular platform group was 5.85u00b10.96, which was not statistically different either (P=0.139). In addition, PES/WES still showed no statistical difference when platform-switching group was 12.89u00b11.65 and regular platform group was 11.63u00b11.67 (P=0.082).Conclusions and Clinical ImplicationsWithin the limitations of this retrospective study, platform switching implants showed no statistically better esthetic outcomes than regular platform implants. However, PES/WES of platform-switching group was higher than regular platform group. Further study should be taken with larger samples and longer observations to gain strong evidence on comparing aesthetic outcomes of platform switching or regular platform implants in anterior maxillae.

Book excerpt: Introduction The results of immediate implant placement, with a flapless approach, graft of the residual gap and immediate tooth replacement (FIITR) has been extensively reviewed, showing efficacy in terms of the preservation of the alveolar ridge and good implant survival. However, very little information exists on the clinical results of the immediate implants in total or partial absence of the vestibular bone plate (FIITR-AC). 421Objective Analyse retrospectively the success and aesthetic outcomes of 60 consecutive cases of FIITR-AC with a follow-up of 1 to 7 years, evaluated by means of the Albrektsson criteria (Ac), interproximal bone level changes (IPBLC) and the Pink Esthetic Score (PES). 257Material and methods 59 consecutive patients received a FIITR-AC in the anterior region (60 implants), from 2008 to early 2015, were included in the study. Surgery was performed by only one experienced surgeon (AS) with the help of a surgical microscope and microsurgical instruments. All patients were undergoing periodontal maintenance therapy. Special care was taken during the tooth extraction to preserve as much of the cortical plate as possible, after the flapless placement of the implant, an autogenous tunnel soft tissue graft was made to cover internally the bone dehiscence defect, and a deproteinized bovine bone mineral graft was made in the remaining gap. After achieving adequate implant stability, impressions were taken and a lab-made screw-retained provisional prosthesis was fabricated and delivered to the patient within the following 2 hours.Success (Ac), IPBLC (calibrated periapical digital radiographs), and aesthetic aspects compared to the contralateral tooth (PES) were evaluated. Results 40 implants supported single tooth restorations, while 20 were splinted in multiple-implant supported prosthesis. One of the cases (operated in 1988) failed (1.7%) after 1 month of healing. Interproximal peri-implant bone levels (IPBL) remained stable during the years of follow up (mean 0.7 mm single vs 0.26 mm splinted). The success rate was 98.3% (95%cr= 91.1-99.7%). Good aesthetic results were observed, with even significant better aesthetic outcomes in the single tooth implants than in the contralateral teeth (PES: 12.4 vs 10.7, p=0.002), both at the level of the global PES and its components. 78.3% of the cases (95% cr= 62.8-88.6) showed no buccal soft tissue marginal recession (PES=2), while 21.6% showed a PES of 1 (less than 2 mm). Evaluation was made by a calibrated examiner (AP, Kappa> 0.70). 813Conclusions With the limitations of a retrospective study, it has been observed that FIITR-AC may be a reliable option of treatment for alveoli with partial loss of the buccal plate when adequate implant stability may be obtained, showing a good success rate, the stability of IPBL and good aesthetic results in comparison with contralateral teeth.

Immediately after surgery a full resin provisional immediate restoration was placed. At 6 months, the final restoration was performed. The clinical follow-up period was at least 60 months of functional loading.30 patients were treated with 300 implants. The clinical findings demonstrate an excellent clinical response of this type of protocol, free of postoperative complications in patients undergoing surgery. From an implantologic point of view, the results indicate a success rate of 98.4%. Five implants were lost during the healing period with the provisional prosthesis. After a mean functioning period of 77.8 months from the final restoration, there was no late complications.This study indicates that treatment with dental implants by guided surgery in the upper jaw is a high success rate therapeutic alternative.

However this concept is still controversial and some authors suggest delaying the placement and loading of the implant in the aesthetic area.The aim of this study is to evaluate and compare 2 study groups (immediate placement and loading- and delayed) of 20 single implants 10 for each group placed into the aesthetic area made by the same operator. Are included in this study only from incisors to canine area in upper and lower jaw and evaluated each one after 60 months.Materials & MethodsAfter a minimally traumatic extraction of the tooth, a different surgical approach was used.In the first DEALAYED study group (10 individual cases), depending on the presence of thin or thick gingiva and on the residual volume of buccal plate, the implant was placed with conventional protocol and delayed loading. In the second IMMEDIATE LOADING study group (10 individual cases), after the tooth extraction the implant was immediately placed and loaded without functional loading. In both study groups, if bone loss occurred in the buccal plate, an integration with GBR techniques and the use of biomaterials were necessary to fill the buccal gap. Good primary stability of the implant and a non-functional load were achieved in all loading cases.ConclusionsThe follow-up after 60 months, performed by clinical probing and radiographic evaluation and also supported by TC scan, shows that the results are comparable and that post-extractive implants with immediate non functional loading can be used in selected cases.The results obtained are good in both thin and thick biotype cases and it seems evident that it is possible to obtain predictable long term functional and aesthetic success. An accurate previous evaluation and study of each single case as biotype, hard and soft tissue and as well as residual buccal plate are always considered essential requirement criteria to obtain this data.

BLX Straumann design implants were placed. Implants insertion torque were measured using the surgical unit. A screwretained immediate provisional was placed if primary stability exceeded 35 N.cm. Three months after implant placement impressionwas taken for final restorations.Three consecutive patients were included in the study. Two maxillary central incisors and 1 mandibular canine were extracted. A totalof 3 implants were inserted, the average initial stability obtained 42.5 N.cm with a range of 40-45 N.cm, allowing in each cases theplacement of an immediate provisional restoration. At 6 months recall the implant survival rate was 100%.The results of this case series suggest that the use of the BLX Straumann implant design is predictable to achieve sufficient primarystability, which is necessary for immediate temporization and subsequent osseointegration for replacement of maxillary incisors.

Followed by a period of 10 months to 45 months after final restoration. The success rate and red-white esthetic outcomes were observed. X-ray examinations were performed to observe changes in the marginal bone level around the implant.ResultatsNone of the 7 implants loosen or falled off during the follow-up period. The implant function was normal and the implant success rate was 100%. After the permanent restoration, good aesthetic effects were obtained. The natural contour and the mesial-distal gingival papilla were completely preserved. The highest point of the labial margin was in harmony with the height of the adjacent natural teeth, and the alveolar process was full and natural. After 10 months to 45 months of follow-up, the stability of the implants was good. The X-ray showed that marginal bone level around the implants was stable.Conclusions and Clinical ImplicationsFor the Type II sockets, flapless immediate implantation and GBR were performed in the sockets at the same time. Various guided bone regeneration (GBR) techniques are used in order to regain adequate tissue amountfor implant treatment. Combined use of growth factors derived from human blood with grafting materials have been widely used andseen as a predictable treatment modality.Aim/HypothesisThe aim of this two case reports was to evaluate the effects of horizontovertical ridge augmentation on bone gain with combined useof xenograft bone material, Ti-Reinforced non-resorbable PTFE membrane, and CGF prior to implant placement and implant survivalrates.Materials and MethodsA 37-year-old female and a 57-year-old male patient registered with complaints of acute pain and mobility of maxillary lateral teeth.After the clinical and radiographic examination, the extraction of the tooth was planned. The extractions were made non-traumaticallywith periotome and extraction sockets were filled with CGF to enhance the wound healing. 4 weeks after extraction, flap surgery wasperformed for augmentation. The defect was grafted with xenograft bone material (Bio-Oss Collagen, Geistlich) and Ti-ReinforcedPTFE membrane (NeoGen) was fixated with titanium screws. Double layers of CGF were used over the membrane in order to obtainsoft tissue closure. Antibiotic therapy was prescribed for infection control. After the healing period of 6 months, re-entry was made andnon-resorbable membranes were removed before the implant placements. Prosthetic loading was performed after 3 months ofosseointegration period. Recalls were scheduled at 1, 3, 6, 12, and 24 months following.ResultsNo signs of inflammation or infection was observed post-surgically and at follow-up sessions. After the removal of Ti-Reinforced PTFEmembrane, horizontovertical bone gain was observed with CBCT evaluation and clinically. Healing was uneventful without anycomplications after the implant surgery and the implants were in good terms of success rate. The survival rates of implants are both%100 two years after implant placement. Combined use of grafting materials, PTFE membrane, and CGF obtained an adequate boneformation at horizontovertical bone defect sites.Conclusions and Clinical ImplicationsCombined use of xenograft bone substitute, PTFE membrane, and CGF may be an effective treatment modality to regenerate boneformation for horizontovertical bone defect cases prior to implant placement and can improve the implant success and survival rates.The present positive outcomes of these two cases can be considered promising but must be confirmed by long term controlledstudies with larger sample of patients.
