Available in PDF, EPUB and Kindle. Book excerpt: The microcirculation of the gastrointestinal tract is under the control of both myogenic and metabolic regulatory systems. The myogenic mechanism contributes to basal vascular tone and the regulation of transmural pressure, while the metabolic mechanism is responsible for maintaining an appropriate balance between O2 demand and O2 delivery. In the postprandial state, hydrolytic products of food digestion elicit a hyperemia, which serves to meet the increased O2 demand of nutrient assimilation. Metabolically linked factors (e.g., tissue pO2, adenosine) are primarily responsible for this functional hyperemia. The fenestrated capillaries of the gastrointestinal mucosa are relatively permeable to small hydrolytic products of food digestion (e.g., glucose), yet restrict the transcapillary movement of larger molecules (e.g., albumin). This allows for the absorption of hydrolytic products of food digestion without compromising the oncotic pressure gradient governing transcapillary fluid movement and edema formation. The gastrointestinal microcirculation is also an important component of the mucosal defense system whose function is to prevent (and rapidly repair) inadvertent epithelial injury by potentially noxious constituents of chyme. Two pathological conditions in which the gastrointestinal circulation plays an important role are ischemia/reperfusion and chronic portal hypertension. Ischemia/reperfusion results in mucosal edema and disruption of the epithelium due, in part, to an inflammatory response (e.g., increase in capillary permeability to macromolecules and neutrophil infiltration). Chronic portal hypertension results in an increase in gastrointestinal blood flow due to an imbalance in vasodilator and vasoconstrictor influences on the microcirculation. Available in PDF, EPUB and Kindle. Book excerpt: These notes were developed to assist in the review of the basic science of pathology for those students preparing to sit for various national proficiency examinations which include, as part of their content, material dealing with general and systemic pathology. As such, the notes are not intended to replace the many fine textbooks of pathology whose scope, detail, and mission are more suited to the in depth study of pathology. Rather, these notes are intended to serve as "memory joggers" to rekindle and refresh the mind of some of the salient features of various disease processes and to point out to the student possible areas of weakness which they may need to address in more detail by referring to one of the standard textbooks of pathology. In this regard, these notes are presented in an organized and sensible (we hope!) outline format which is intended to allow systematic review of the material in a reasonable amount of time. In an effort to contain cost and avoid undue repetition with other volumes of the Oklahoma Notes series, some topics that are generally covered in most standard pathology courses as well as illustrative material have been omitted from the present edition. The authors would like to extend our sincere gratitude to the faculty and staff of the Department of Pathology for their ideas, support, and assistance during the preparation of these notes. In particular, we would like to thank Drs. K.

Available in PDF, EPUB and Kindle. Book excerpt: This text comprehensively reviews the current state of the art in Laryngopharyngeal Reflux (LPR) together with a comprehensive explanation and description of the known gastroesophageal reflux (GERD) literature. With chapters written by experts from around the world, this text aims to deliver what is current in reflux recognition, diagnosis, reflux related complications, and the various treatment modalities. This is the first textbook to combine the most up to date knowledge of both LPR and GERD meant for both specialties and the general medicine population. Completely unique to the reflux literature is a section detailing the substantial benefits of a mostly plant based, Mediterranean style diet in the treatment of reflux disease. Encouraging patients to read and learn about diet and health is likely the most important step in improving their disease. The text provides direction to the caregiver on how to transition to a mostly plant-based diet. Review of myths, effects of diet in the setting of other disease states, and dietary consequences are explained. Guidelines on how to transition diet, dining out while maintaining a plant-based diet, and how to wean off medication, such as PPI, are also provided. Laryngopharyngeal and Gastroesophageal Reflux: A Comprehensive Guide to Diagnosis Treatment, and Diet-Based Approaches will provide the medical community with a resource to understand, teach, and provide the latest in LPR and GERD information to the caregiver and subsequently the patient.
