The concept of ICPC-2 has not been changed but the main body of the actual classification (chapter 10) has been completely revised to reflect the adequate use for an individual patient record and for research purposes. This now brings the printed version of ICPC-2 completely in-line with the electronic version previously only available through the Oxford Website for the journal Family Practice. ICPC-2 is patient-oriented rather than disease or provider-oriented. It encompasses both the patient's reason for encounter and the patient's problem. ICPC-2 is extensively used internationally and includes a detailed conversion system for linking the ICPC and ICD-10 codes published by WHO, additional inclusion criteria, and cross-referencing rubrics. ICPC-2 It has been developed based on the recognition that building the appropriate primary care systems to allow the assessment and implementation of health care priorities is possible only if the right information is available to health care planners.

Book excerpt: This book reports changes in GP ordering of imaging tests in Australia from 2002-03 to 2011-12, and evaluates alignment between guidelines and recent GP test ordering for selected problems. Over the decade, 9,802 GPs participated in BEACH, providing details of 980,200 GP-patient encounters. The likelihood of GPs ordering imaging in the management of a problem increased over time. In recent practice, at least one imaging test was ordered at 9% of encounters, at a rate of 10 imaging tests per 100 encounters. Diagnostic radiology was the most commonly ordered type of imaging test, but the order rate decreased over time, with a shift toward orders for ultrasound, CT and MRI, which all significantly increased. Eight selected problems accounted for one-third of all imaging orders. Imaging ordering behaviour suggests broad compliance with published guidelines in the management of osteoarthritis, shoulder problems, bursitis/tendonitis/synovitis, abdominal pain and other musculoskeletal injuries. Current ordering patterns for knee problems and some sprains/strains have potential for improvement. The ordering pattern for new presentations of back problems was inconsistent with all established guidelines for management of back problems.

For an 'average' 100 problems managed, GPs recorded: 65 medications (including 53 prescribed, seven supplied to the patient and six advised for over-the-counter purchase); 12 procedures; 24 clinical treatments (advice and counselling); six referrals to specialists and three to allied health services; orders for 31 pathology tests and seven imaging tests. A subsample study of more than 31,000 patients suggests prevalence of measured risk factors in the adult (18 years and over) population who attended general practice at least once in 2013-14 were: obesity-27%; overweight-35%; daily smoking-17%; at-risk alcohol consumption-26%. Book excerpt: Foundational Knowledge for the Practice of Family Medicine in West Africa is a selection of introductory concepts from literature using literature search and review; a collection of lectures from eminent family physicians from South Africa, Canada, United Kingdom, and United States; lecture notes; seminal essays; and journal articles that guided the author through the transition from general practice to family medicine in the 1980s and 1990s. The concept of family medicine in West Africa was a paradigm shift from a specialty conception of a conglomeration of independent specialties in general practice to a unique specialty with its own distinct identity with a fundamental principle based on the biopsychosocial model. The specialty draws a distinctive identity from its approach to care by placing emphasis on the contextual setting of the patient, starting with the family and extending to the sociocultural and economic environment. This composite approach to health care was new and confusing to the uninitiated medical graduate undergoing a residency training taught in the traditional biomedical reductionist model and to other medical specialists who asked what new knowledge we were bringing to the table that other specialists were not already teaching. As for those teachers of family medicine in the subregion, it was quite clear as to the need for this conceptual framework in family medicine as opposed to the limitation of general practice. This book documents the struggle to position family medicine as a separate specialty of medicine in West Africa.
