Available in PDF, EPUB and Kindle. Book excerpt: Infectious disease is the most common cause of illness and death in Africa, yet health practitioners routinely fail to identify causative microorganisms in most patients. As a result, patients often do not receive the right medicine in time to cure them promptly even when such medicine is available, outbreaks are larger and more devastating than they should be, and the impact of control interventions is difficult to measure. Wrong prescriptions and prolonged infections amount to needless costs for patients and for health systems. In Divining without Seeds, Iruka N. Okeke forcefully argues that laboratory diagnostics are essential to the effective practice of medicine in Africa. The diversity of endemic life-threatening infections and limited public health resources in tropical Africa make the need for basic laboratory diagnostic support even more acute than in other parts of the world. This book gathers compelling case studies of inadequate diagnoses of diseases ranging from fevers—including malaria—to respiratory infections and sexually transmitted diseases. The inherited and widely prevalent health clinic model, which excludes or diminishes the hospital laboratory, is flawed, to often devastating effect. Fortunately, there are new technologies that make it possible to inexpensively implement testing at the primary care level. Divining without Seeds makes clear that routine use of appropriate diagnostic support should be part of every drug delivery plan in Africa and that diagnostic development should be given high priority.

Book excerpt: Addressing the management of genetic resources, this book offers a new assessment of the contemporary Access and Benefit Sharing (ABS) regime. Debates about ABS have moved on. The initial focus on the legal obligations established by international agreements like the United Nations Convention on Biological Diversity and the form of obligations for collecting physical biological materials have now shifted into a far more complex series of disputes and challenges about the ways ABS should be implemented and enforced. These now cover a wide range of issues, including: digital sequence information, the repatriation of resources, technology transfer, traditional knowledge and cultural expressions, open access to information and knowledge, naming conventions, farmers’ rights, new schemes for accessing pandemic viruses sharing DNA sequences, and so on. Drawing together perspectives from an interdisciplinary range of leading and emerging international scholars, this book offers a new approach to the ABS landscape; as it breaks from the standard regulatory analyses in order to explore alternative solutions to the intractable issues for the Access and Benefit Sharing of genetic resources. Addressing these modern legal debates from a perspective that will appeal to both ABS scholars and those with broader legal concerns in the areas of intellectual property, food, governance, Indigenous issues, and so on, this book will be a useful resource for scholars and students as well as those in government and in international institutions working in relevant areas.

Book excerpt: This encyclopedia provides the most complete treatment to date of the accomplishments of African American scientists—and the struggles of African Americans to find their place in the scientific community. This comprehensive reference work sheds new light on an aspect of African American life that is often overlooked. More than a summary of individuals and accomplishments, African Americans in Science: An Encyclopedia of People and Progress explores the entire experience of African Americans seeking a place in the scientific community—not just the triumphs but the frustrations, discriminations, and the efforts to support (and sometimes impede) African American scientists. African Americans in Science offers alphabetically organized entries in three areas: the contributions of African Americans in over 30 different fields of science and medicine, schools and organizations that played a role in the development of African American scientists, and additional topics related to African American scientists. No other reference offers such a complete and up-to-date portrait of the pivotal work of African Americans across the spectrum of scientific research and what it took to achieve it.

Book excerpt: Sharing research data on public health issues can promote expanded scientific inquiry and has the potential to advance improvements in public health. Although sharing data is the norm in some research fields, sharing of data in public health is not as firmly established. In March 2015, the National Research Council organized an international conference in Stellenbosch, South Africa, to explore the benefits of and barriers to sharing research data within the African context. The workshop brought together public health researchers and epidemiologists primarily from the African continent, along with selected international experts, to talk about the benefits and challenges of sharing data to improve public health, and to discuss potential actions to guide future work related to public health research data sharing. Sharing Research Data to Improve Public Health in Africa summarizes the presentations and discussions from this workshop.

Book excerpt: A Choice Outstanding Academic Title of the Year ÒA critical, poignant postmortem of the epidemic.Ó ÑWashington Post ÒForceful and instructive...Sabeti and Salahi uncover competition, sabotage, fear, blame, and disorganization bordering on chaos, features that are seen in just about any lethal epidemic.Ó ÑPaul Farmer, cofounder of Partners in Health ÒThe central theme of the book...is that common threads of dysfunction run through responses to epidemics...The power of Outbreak Culture is its universality.Ó ÑNature ÒSabeti and Salahi present a wealth of evidence supporting the imperative that outbreak response must operate in a coordinated, real-time manner.Ó ÑScience As we saw with the Ebola outbreakÑand the disastrous early handling of the COVID-19 coronavirus pandemicÑa lack of preparedness, delays, and system-wide problems with the distribution of critical medical supplies can have deadly consequences. Yet after every outbreak, the systems put in place to coordinate emergency responses are generally dismantled. One of AmericaÕs top biomedical researchers, Dr. Pardis Sabeti, and her Pulitzer PrizeÐwinning collaborator, Lara Salahi, argue that these problems are built into the ecosystem of our emergency responses. With an understanding of the path of disease and insight into political psychology, they show how secrecy, competition, and poor coordination plague nearly every major public health crisis and reveal how much more could be done to safeguard the well-being of caregivers, patients, and vulnerable communities. A work of fearless integrity and unassailable authority, Outbreak Culture seeks to ensure that we make some urgently needed changes before the next pandemic.

Available in PDF, EPUB and Kindle. Book excerpt: Can today's innovative practices and molecular tools tame this ancient disease? One third of the world's population is infected with tuberculosis (TB), with about 10 million new cases annually. To combat TB and its agent, Mycobacterium tuberculosis, the World Health Organization launched The End TB Strategy, which aims to slash the suffering and cost of TB by 2035. This makes the second edition of Tuberculosis and the Tubercle Bacillus, edited by Jacobs, McShane, Mizrahi, and Orme, an extremely valuable resource for scientists and clinicians. The editors have gathered their colleagues from around the world to present the latest on the molecular biology of M. tuberculosis and related species, the host-pathogen interactions that enable invasion, and the host's immune response to M. tuberculosis infection. The basic, clinical, and translational research presented in this book supports the goals of WHO's End TB Strategy by driving toward the development of effective vaccines, rapid molecular diagnostics, and anti-TB drugs. Creating an effective tuberculosis vaccine. Understand the innate and adaptive immune response to M. tuberculosis infection, its study in established animal models, and how this information is being used to develop new vaccines against TB. Formulating new antituberculosis drugs. Learn the challenges and methods for evaluating new drugs in preclinical trials with a focus on drugs that work against "persisters" and those that act on the electron transport complex and ATP synthase of M. tuberculosis. Overcoming the challenges of diagnosing tuberculosis. Review new diagnostic tools that are simple, rapid, affordable, specific, sensitive, and safe, including molecular-based diagnostic methods such as GeneXpert MTB/RIF. Using molecular, genomic, and bioinformatics tools to understand the biology and evolution of Mycobacterium. Explore current research on the molecular mechanisms that M. tuberculosis uses to evade the immune system, enter a state of nonreplicating persistence, and become reactivated. The second edition of Tuberculosis and the Tubercle Bacillus presents the latest research on a microorganism that is exquisitely well adapted to its human host. This pathogen continues to confound scientists, clinicians, and public health specialists, who will all find much valuable information in this comprehensive set of reviews.

Early and reliable diagnosis and subsequent rapid initiation of appropriate antifungal therapy has shown to improve survival significantly, at least for invasive Aspergillosis. Early diagnosis of Aspergillus associated infections, therefore, represents a cornerstone in successful management of these diseases. Current state of the art diagnostic approaches, new insights in epidemiology and established biomarkers as well as an outlook for future diagnostic options in the armamentarium of diagnostics tools are highlighted in this eBook.
