However, lack of knowledge and information-processing limitations may limit the ability to make informed decisions. Decision aids address these issues, and may increase patients' ability to make informed choices. However, little research has examined decision aid effectiveness. One difficult decision faced by many women is whether to use hormone replacement therapy (HRT), which is associated with both potential risks and potential benefits. A tool designed to enhance HRT decision making could benefit many women. This paper presents the evaluation of an HRT videotape decision aid. The paper's primary goals are to (1) examine the impact of the decision aid on treatment-relevant knowledge, perceived knowledge, and perceived ability to make an informed choice about HRT; (2) determine the impact of the decision aid on HRT attitude; (3) examine whether the decision aid is perceived as useful; and (4) evaluate the influence of specific decision aid components on these issues. There were three phases to this project. First, several focus groups were convened to gain better understanding of women's knowledge and attitudes about HRT. Next, two pilot studies examined how altering the presentation of HRT-type information affects preference for hypothetical treatments. The third phase involved the evaluation of an HRT decision aid videotape. Female college students (n = 153) were randomly assigned to one of four conditions: (1) complete HRT video; (2) HRT video with patient testimonials excluded; (3) HRT video with decision aid data excluded; or (4) a control (non-HRT) video. All subjects completed pre- and post-video questionnaires examining HRT knowledge and attitudes. The HRT video was associated with increases in knowledge, perceived knowledge, and perceived ability to make an informed HRT decision. Actual knowledge and perceived knowledge were not related, and perceived ability was predicted by perceived knowledge rather than actual knowledge. The video was perceived as a useful tool for HRT decision making, although it did not increase positive attitude towards HRT. Results were similar for each version of the video, except that the version without patient testimonials was perceived more negatively. Implications of these findings are discussed.

Available in PDF, EPUB and Kindle. Book excerpt: Originally published in 1982, this book examines the current status of expectancy-value models in psychology. The focus is upon cognitive models that relate action to the perceived attractiveness or aversiveness of expected consequences. A person’s behavior is seen to bear some relation to the expectations the person holds and the subjective value of the consequences that might occur following the action. Despite widespread interest in the expectancy-value (valence) approach at the time, there was no book that looked at its current status and discussed its strengths and its weaknesses, using contributions from some of the theorists who were involved in its original and subsequent development and from others who were influenced by it or had cause to examine the approach closely. This book was planned to meet this need. The chapters in this book relate to such areas as achievement motivation, attribution theory, information feedback, organizational psychology, the psychology of values and attitudes, and decision theory and in some cases they advance the expectancy-value approach further and, in other cases, point to some of its deficiencies.

Available in PDF, EPUB and Kindle. Book excerpt: Over the past decade health care systems around the world have placed increasing importance on the relationship between patient choice and clinical decision-making. In the years since the publication of the second edition of Shared Decision Making in Health Care, there have been significant new developments in the field, most notably in the US where 'Obamacare' puts shared decision making (SDM) at the centre of the 2009 Affordable Care Act. This new edition explores shared decision making by examining, from practical and theoretical perspectives, what should comprise an effective decision-making process. It also looks at the benefits and potential difficulties that arise when patients and clinicians share health care decisions. Written by leading experts from around the world and utilizing high quality evidence, the book provides an up-to-date reference with real-word context to the topics discussed, and in-depth coverage of the practicalities of implementing and teaching SDM. The breadth of information in Shared Decision Making in Health Care makes it an essential resource for policy-makers and health care workers. As health care systems adapt to increasingly collaborative patient-clinician care frameworks, this will also prove a useful guide to SDM for clinicians of all disciplines.

Book excerpt: A compelling, “fascinating” (Robert Cialdini) defense of hormone replacement therapy, exposing the faulty science behind its fall from prominence and giving women the evidence they need to make informed decisions about their health. Now fully revised and updated. "Estrogen Matters was my antidote to the misinformation surrounding menopause. This book should be the bible for every single person going through menopause.”―Naomi Watts For years, hormone replacement therapy (HRT) was the medically approved way to alleviate menopausal symptoms (ranging from hot flushes to brain fog) and reduce the risk of heart disease, Alzheimer's, and osteoporosis. But when a large study by the Women's Health Initiative (WHI) announced, with national fanfare, that women taking HRT had an increased risk of breast cancer, women were scared off, and the treatment was abandoned. Now, Dr. Bluming, a medical oncologist, and Dr. Tavris, a social psychologist, reveal the true story of the WHI’s efforts to distort their data to exaggerate unsupported claims of estrogen’s harms. Important updates in this edition include: Evidence that demolishes the WHI’s claim that HRT causes breast cancer. A list of the WHI’s retractions of their original scare stories. Updated findings on estrogen’s benefits on heart, brain, bones, and longevity. A critical review of the alternative products and medications being marketed to treat symptoms of menopause. A sobering and revelatory read, Estrogen Matters sets the record straight on estrogen’s benefits, providing a light to guide women through this inevitable phase of life.

Book excerpt: It has been known for over 40 years that GH-deficient-children benefit from replacement with the hormone. But GH, essential for longitudinal growth, also plays a role after completion of final height. With the introduction of biosynthetic human GH 20 years ago, the use of GH was no longer restricted to severe growth retardation in hypopituitary children. This book will take the reader behind the myths of GH and into the real world of clinical endocrinology. The contributions stem from recognized clinicians and scientists who have been working in the field for decades. The contents encompass traditional end points of GH therapy such as body composition, bone biology and physical performance. Attention is also devoted to diagnostic aspects and side effects. Additional features range from clinical epidemiology to quality of life, and novel areas such as the impact of traumatic brain injury on pituitary function are also covered. The present volume of Frontiers of Hormone Research is essential reading for health care professionals interested in clinical endocrinology and GH.
