Small symposia, where scientific discussion can be confined to limited groups, have lately become successful vehicles of information transfer. However, such symposia, although eminently suited to the exploration of specific problems, often have the llherent'disadvantage of not being able to successfully address themselves to larger themes. Accordingly, the planners of this symposium have solicited the parti cipation of internationally acclaimed investigators to consider current problems both in the physiological and clinical domains of nephrology. The observant reader will note that the variety of theoretical and experi mental approaches used by the physiologists are reflected in the clinical studies, while the clinicians, in turn, have provided the physiologists with useful insights. The part of the symposium which attempted to correlate metabolic characteristics of kidney cells with their transport functions has proven to be particularly fruitful. For example, the presentations dealing with aldosterone, angiotensin and diuretics have offered a multi-faceted approach that was previously unavailable. The contributions of investigators from many countries and their enthusiastic participation have led to the success of this symposium. To them we wish to express our deep gratitude. Numerous studies have been based on the concept that the elevation in blood pressure is mediated by several vasopressor substances. For this reason the renin-angiotensin system has been considered to be the essential endocrine mechanism involved in developing and sustaining arterial hyper tension. Other vasopressor factors, such as catecholamines, vasopressin, and serotonin, have been studied extensively and were assumed to play additional parts in blood pressure regulation. However, several other ap proaches have been initiated in recent years.

It is now more than 70 years and will continue to increase in the near future. Aging is a challenge to science and society. There are several theories of aging. The programmed aging hypothesis suggests that aging cannot be altered by any external influence, that aging is programmed from the beginning of life until the end of senescence. Another theory is based on genetic instability, with repair processes correcting errors in the cells that cause aging. Aging processes manifest themselves at the molecular and cellular level, in different organs, and in the whole organism. Most older persons acquire multiple diseases, primarily chronic ones. The multimorbidity of the elderly raises many questions as to nutrition, metabolism, immunity, drug therapy, and rehabilitation. Nutrition may be able to intervene in the aging process. Aging is associated with dec lining immunity and reduced resistance to many diseases, and it is pos sible that nutritional changes in old age contribute to the declining immunity in the elderly. The elderly take 40% of the prescription drugs in Germany and adverse drug reactions are more frequent in geriatric patients. There are not only problems in drug therapy for elderly pa tients, but also in their rehabilitation. The aim of rehabilitation is the restoration of geriatric patients to their former functional and environ mental status.

Public and even physician interest in the subject remained minimal until the early forties, and even then there were not more than a dozen "fulltime" investigators worldwide. The first organization devoted to communicating regularly the results of hy pertension research was created in Cleveland in 1945; it became the Council for High Blood Pressure Research of the American Heart Association in 1949. The early history of hypertension is not a story of progressive achievement. Richard Bright is given great credit, followed by Mahomed, Allbutt, Riva-Rocci, Bergman and Janeway, but it was not until Volhard, Fahr and Allen that some semblance of order emerged. They were followed by a younger, much more vigorous group of investigators who in fact initiated modern research in hypertension. Two more generations have fol lowed. In the course of some fifty years, we have seen emerge a magnificent body of evidence that has prescribed good treatment and contributed a considerable understanding of the many complex mechanisms involved in the hypertensions. Treatment is now actively pro moted by public health agencies, including the World Health Organization. This is an other case of treatment preceding full understanding of the nature of the disease.
