Book excerpt: Excerpt from Hip Disease in Childhood: With Special Reference to Its Treatment by Excision The greater part of the present work has been already published in America, in the Archives of Pediatrics; and most of the cases in Appendix I., with some of those in Appendix II., have appeared in the annual numbers of Abstracts, edited by the staff of the General Hospital for Sick Children, Manchester. The text has, however, been much revised and expanded from the American issue, into which many mistakes crept from my having no opportunity of correcting proof sheets. The list of excisions has been enlarged, and later records of the condition of many of the cases obtained. That this record is not more complete is due to the practical impossibility of tracing many patients after their discharge from hospital. Most of the cases, however, have been followed up for considerable periods. I have thought it worth while to publish the work in its present form since hip disease is one of the subjects on which the most directly conflicting views are still held, especially as to the question of excision, and the results of an experience of several hundred cases cannot be without interest. The present uncertainty in the matter is, in my opinion, to no small extent due to the habit of assuming that all joints of the body are affected by disease in the same way, while, in fact, the effects of disease are as different as is the construction of the several joints, and the hip stands much by itself in its pathology. Book excerpt: This book is concerned with the effect that displacement, whether minimal or severe, may have on the hip joint. Although it is concerned with the changes which take place in childhood and during growth, when they are most common and most severe, it is also to a lesser extent concerned with the way they will continue or even start long after growth has ceased. It is based on a series of about 450 cases of congenital displacement of the hip treated when the deformity was established, together with unstable hips drawn from 82000 children whose hips were examined at the time of birth. This study was carried out at the Nuffield Orthopaedic Centre in Oxford. It was started in early 1949 by the author with Mr. J. C. Scott and continued until mid-1977, since when it has continued in the capable hands of Mr. J. W. Goodfellow and Mr. M. K. Benson. The study was started at a time when the generally accepted view was still that the displacement was part of the primary failure of development of the acetabulum, which could not adequately contain the femoral head. Conservative treatment with manipulative reduc tion followed by a prolonged period of plaster immobilisation was the method of choice. Few attempts had been made with surgery as a primary procedure and these had not met with continuing success.
