Available in PDF, EPUB and Kindle. Book excerpt: The hemodynamic significance of the flow properties of blood was put into perspective only during the past decade. Advances in modern technologies today allow the quantitative analy sis of the fluidity of blood and its components under conditions approximating the flow in vivo, particularly those in the microcirculation. The hematocrit is the most important of the determinants of blood fluidity (reciprocal value of blood viscosity); acute increases in the hematocrit exert deleterious effects on circulation and oxygen transport owing to impaired fluidity of blood. High viscosity of plasma due to hyper- or dysproteinemias initiates the microcirculatory dysfunctions in hyperviscosity syndromes. Furthermore, the fluidity or deformability of red cells might be critically diminished and therefore cause redistribution of blood elements and adversely affect the resistance to flow within the microvessels. In low flow states blood fluidity most likely becomes the key determinant for microvessel perfu sion, overriding the neural and local metabolic control mechanisms operative at physiological conditions to adjust blood supply to tissue demand. Microcirculatory disturbances are there fore encountered whenever driving pressures are reduced, as in shock or hypotension, and distal to stenoses of macrovessels, but also in hemoconcentration due to plasma volume con traction, polycythemia, leukemia, and dysproteinemia. Based on experimental studies exploring the possibilities and limitations, with regard to improving the fluidity of blood by reducing the hematocrit, the concept of intentional hemo dilution has been introduced to clinical medicine.

Book excerpt: Hemodilution is defined as the dilution of the concentration of red blood cells and plasma constituents by partially substituting the blood with colloids or crystalloids and it is a strategy to avoid exposure of patients to the hazards of homologous blood transfusions, Several mathematical models and computer simulations have been introduced to validate the efficacy of hemodilution; the amount of maximal allowable blood loss and final postoperative hematocrit or hemoglobin has been calculated, The basic prerequisite for preserving tissue oxygenation during hemodilution is adequate oxygen delivery, and therefore all determinants of oxygen transport should be monitored, Compensatory, mechanisms such as increased cardiac output and stroke volume occur following hemodilution, When debating the use of colloid and cnstalloid during fluid replacement, it is imperative to think about all of the components of the Starling equation, In order to better analyze the clinical outcome of hemodilution, more realistic mathematical models should be developed.

Book excerpt: Traditional cardiopulmonary bypass (CPB) techniques have suffered from a number of disadvantages including haemodilution, inflammation and post-operative bleeding. Minimised cardiopulmonary bypass techniques use developments in perfusion technology to significantly reduce foreign surface-blood interactions to make bypass simpler and safer. This important book reviews key developments and issues relating to this promising technology.Part one covers the broad range of CPB pathophysiology, including anticoagulant protocols, the impact of CPB circuit surfaces, optimal haemodilution levels, and the important issue of CPB-induced systemic inflammatory response syndrome. Part two focuses on the issues of the new equipment developed for mini-CPB, optimal myocardial protection protocols and CPB perfusate options. Part three discusses clinical issues, including patient selection, coronary and valve surgery protocols and, among others, paediatric patients. With its distinguished editors and international team of expert contributors, Minimized cardiopulmonary bypass techniques and technologies is a valuable reference for cardiac surgery teams and those researching this important technology. Covers a broad range of cardiopulmonary bypass (CPB) pathophysiology, including anticoagulant protocols, the impact of CPB circuit surfaces and optimal haemodilution levels Focuses on new equipment specially developed for minimized-CPB and myocardial protection protocols Discusses clinical issues, including patient selection
