Book excerpt: The change in R-R intervals between adjacent heartbeats is referred to as Heart Rate Variability (HRV). HRV data provides information regarding an individual's Autonomic Nervous System (ANS), specifically the ANS's two branches, the Sympathetic Nervous System (SNS) and the Parasympathetic Nervous System (PNS). The HRV of a healthy, well-conditioned heart is large at rest, while low HRV is associated with adverse health outcomes such diabetes, heart disease and early mortality. There has been a substantial amount of HRV research conducted with typically developing individuals. One group who is greatly underrepresented in research is individuals with intellectual disabilities. Currently, no studies have been undertaken with Special Olympics athletes. Therefore, the purpose of this study was to create HRV profiles at rest, during submaximal exercise, and at recovery of adult Special Olympic athletes. The study also sought to examine the impact that Down syndrome, age, sex, and medication on HRV profiles. The current study found that although heart rate responded appropriately during the three testing conditions (rest, exercise, recovery) the athletes were sympathetically dominated across all three conditions, indicating an imbalance between the SNS and the PNS. In addition, male and female athletes were significantly different with regards to low frequency and high frequency power. It is possible that anxiety or excitement about the testing influenced some athletes, and future research should examine how additional protocol familiarization could impact the HRV profiles within this population. Additionally, more research with larger sample sizes is needed to more fully understand the impact that age, etiology of intellectual disability, and medication use may be having on HRV profiles.

Book excerpt: Heart Rate Variability (Atlaoui et al.) is greater in aerobically trained athletes due to an increase in parasympathetic tone. While high levels of aerobic training are generally associated with good cardiovascular responsiveness, it sometimes leads to suppressed responsiveness. To examine the cardiovascular and autonomic changes that occur in trained and untrained subjects exposed to hypoxic (H) and normoxic (N) conditions. Seven aerobically trained men (6 trained cyclists, 1 runner; age: 29.14 + 4.37 yrs, height: 72.1 + 2.1 in, weight: 77.9 + 7.2 kg, average training hours/week 14.6 + 4.35) and 6 untrained men (age: 28.3 + 4.8 yrs, height: 69.41 + 3.0 in, weight 81.43 + 16.9 kg) were recruited for this study. This study used a double-blind crossover design and subjects were randomly assigned treatment order. Subjects rested for one hour and EKG (Hexoskin) was recorded as well as SaO2. After one hour BP was taken. Subjects then completed a submaximal exercise bout (1.5 watts/Kg BW) for 10 minutes. Subject then reported back to lab for a second test to receive the opposite treatment. Standard deviation of normal-to-normal interval (SDNN) was calculated for each subject during rest and exercise for each environmental treatment. HRV, BP, HR, and SaO2 were analyzed using a Wilcoxon test of Correlated Groups and a Mann-Whitney U test for the difference between two independent samples. An alpha level of 0.05 was used for significance. Trained subjects had a trend (significant at 0.1 level) towards an ANS mediated response (N: 17.57 + 16.6 ms, H: 8.14 + 6.2 ms), while UT subjects did not change during exercise in different environments. Untrained subjects had a significant increase in HR during the exercise bout in hypoxia when compared to that of the normoxic bout (N: 139.2 + 26.1bpm, H: 151.5 + 28.4 bpm) when the trained group did not significantly differ between environments (N: 101.6 + 23.1 bpm, H: 112.8 + 20.9 bpm). Trained subjects had a significant increase in BP in the hypoxic bout of exercise (N: 143.3 + 10.8 mmHg, H: 150.3 +15.8 mmHg) while the untrained subjects did not (N: 155.3 + 9.4 mmHg, H: 159 + 15.7 mmHg). Trained subjects may be more sensitive to small changes in autonomic stimuli whereas Untrained subjects are not as sensitive to change. Both groups were found to be equally responsive to strong autonomic stimuli.

Heart rate variability (HRV), or the beat-to-beat alterations of an individual's heart rate can quantify the body's physiologic response to chronic physical activity. High Frequency (HF) and low frequency (LF) heart rate variability, as well as , pre-ejection period (PEP) of the heart, and Root Mean Square of the Standard Deviation (RMSSD) were recorded and analyzed to calculate heart rate variability. The hypothesis tested was that physically active individuals would have higher parasympathetic and lower sympathetic activation at rest, which would be associated with a reduced risk for injury during physical exercise. Measurement instruments included the Vivometrics LifeShirt© ambulatory physiologic monitoring system and VivoLogic Software (Version 3.2) for the determination of heart rate variability. Data collection equipment was worn by each subject at rest in a controlled environment. The results demonstrated that physically active individuals participating in athletics and performing arts, with lower resting values of RMSSD, reported a significantly higher number of injuries. RMSSD explained 3.8% of the variance in injury occurrence. The application of this finding has yet to be investigated, however is suggestive of at least a minor role of heart rate variability in injury incidence. The use of heart rate variability monitoring during athletic training and competition has not been well elucidated. Heart rate variability monitors are non-invasive, and with advances in technology, can be worn by athletes easily, without interference of athletic ability. Further research is needed and should be geared toward the investigation of the utility of heart rate variability assessment in monitoring the physiologic function in athletes. Heart rate variability assessment may provide information regarding injury risk.

Participants: five male varsity athletes four rugby, one basketball (mean age 19.6 ± 1.52 years), number of previous concussion 1.6 ± 0.55. Measurements: HR & HRV frequency domain (LF n.u., HF n.u., LF/HF ratio, Total Power) & Heart rate (bpm) during both seated rest and steady state exercise using a stationary cycle. Results: Repeated measures ANOVA revealed a significant difference between baseline (pre- injury) resting heart rate and first post-injury assessment resting heart rate (p=0.037). Resting Total Power was significantly different between baseline (pre-injury) and first post-injury assessment (p=0.044) and between first post-injury and second post-injury assessment (p=0.010). No statistical significant differences in any variables were found during exercise, however the trends in the changes of HRV were similar to other research studies and could be of clinical importance. Conclusion: Athletes display dysfunction in neuroautonomic cardiovascular regulation post-concussion as seen with changes in HRV. Findings of this study warrant further investigation into the use of HRV as a marker of concussion and concussion recovery.

Previous research has examined the effect of training on HRV and ANS control of heart rate in males, there is a lack of any comprehensive studies that address adaptations in female athletes. Therefore, the purpose of this study was to investigate the changes in HRV and ANS fluctuations in female swimmer athletes throughout an entire collegiate swim season. 9 Division I female swimmers (Age: 20.6±1.01) were used to determine HRV at three different points in their competitive training: pre-season, mid-season, and post-season. During each testing session, HRV was measured both at rest and during a maximal 400 yd freestyle swim. Heart rate values were determined using PolarTM heart rate monitors, and the HRV analyses was conducted using Kubios 2.0 HRV analysis software. Global ANS balance was shown to significantly shift towards Sympathetic (SNS) predominance during the mid-season testing and significantly shift towards parasympathetic (PNS) predominance during post-season testing. HRV analysis appears to be an appropriate tool to monitor the effects of physical training loads on performance and fitness in female athletes, and it can be used to help identify and prevent overtraining states.
