Book excerpt: Work, so fundamental to well-being, has its darker and more costly side. Work can adversely affect our health, well beyond the usual counts of injuries that we think of as 'occupational health'. The ways in which work is organized - its pace and intensity, degree of control over the work process, sense of justice, and employment security, among other things - can be as toxic to the health of workers as the chemicals in the air. These work characteristics can be detrimental not only to mental well-being but to physical health. Scientists refer to these features of work as 'hazards' of the 'psychosocial' work environment. One key pathway from the work environment to illness is through the mechanism of stress; thus we speak of 'stressors' in the work environment, or 'work stress'. This is in contrast to the popular psychological understandings of 'stress', which locate many of the problems with the individual rather than the environment. In this book we advance a social environmental understanding of the workplace and health. The book addresses this topic in three parts: the important changes taking place in the world of work in the context of the global economy (Part I); scientific findings on the effects of particular forms of work organization and work stressors on employees' health, 'unhealthy work' as a major public health problem, and estimates of costs to employers and society (Part II); and, case studies and various approaches to improve working conditions, prevent disease, and improve health (Part III).

Some 7 per cent of the working population is workless and receiving benefits because of long-term health conditions or disabilities. This represents a significant cost to the economy - in cost of benefits, healthcare, forgone taxes, lost production, sickness absence, informal care - estimated at between £103 and £129 billion. The review's vision for health and work in Britain is based on three principal objectives: prevention of illness and promotion of health and well-being; early intervention for those who develop a health condition; an improvement in the health of those out of work. The review establishes the first baseline for the health of the working population. It then examines the role of the workplace in health and well-being. Work is good for both physical and mental health (Waddell & Burton, 2006, "Is work good for your health and well-being?" TSO, ISBN 9780117036949). Employers, trade unions, employees, safety and health practitioners should all promote the benefits of investment in health and well-being. The review calls of a fundamental shift in the perception of fitness for work, to move away from it being inappropriate to be at work if not 100 per cent fit. Early intervention can prevent short-tem sickness becoming more serious, and pilot trials of a new Fit for Work service are proposed. More health support for workless people on incapacity benefits is recommended. Professional expertise for working age health is needed, and occupational health should be in the mainstream of healthcare provision. To safeguard the future health of the working population, young people should understand the benefits of a life in work. The review closes with proposals for taking the agenda forward.

For over a century, governments and voluntary groups have run educational campaigns encouraging Canadians to adopt healthy habits in order to prolong lives, cost the state less, and produce more efficient workers. Be Wise! Be Healthy! explores the history of public health in Canada from the 1920s to the 1970s. Through the Health League of Canada, people were urged to drink pasteurized milk, immunize their children, and avoid extramarital sex. Health was presented as a responsibility of citizenship – and doctors and dentists as expert guides. Public health campaigns have reduced preventable deaths. But such campaigns can also stigmatize marginalized populations by implying that poor health is due to inadequate self-care, despite clear links between health and external factors such as poverty and trauma. This clear-eyed study demonstrates that while we may well celebrate the successes of public health campaigns, they are not without controversy.
