Significant challenges remain, however, in terms of improving equity with continuing low rates of enrollment. Ensuring financial protection also remains an elusive goal. The Master Plan for Universal Coverage approved in 2012 by the Prime Minister directly addresses both these deficiencies in coverage. The objective of this report is to assess the implementation of Vietnam SHI and provide options for moving towards UC. This is a joint assessment with development partners, World Health Organization, United Nations Children's Fund (Unicef) and Rockefeller Foundation. Expanding breadth of coverage, particularly for those hard to reach groups such as the near-poor and informal sector would require substantially increasing general revenue subsidies and fully subsidizing the premiums for the near-poor. High enrollment rates would, however, have little impact on financial protection and equity if OOP costs remain high. Achieving UC will require sustained efforts to improve efficiency in the system, and gain better value for money from available budgetary resources; without these efforts, any further progress towards UC would be financially unsustainable. There is considerable scope for improving efficiency in Vietnam. Fragmentation in the pooling of funds gives rise to unnecessary costs. Inefficiencies in resource allocation and purchasing arrangements include: (i) an overly generous benefits package; (ii) provider payment mechanisms and the mix of incentives facing providers which result in an oversupply of services; (iii) high prices, overconsumption and inappropriate use of pharmaceuticals; and (iv) the structure and incentives embedded within the delivery system. The organization, management and governance of SHI are fragmented and often dysfunctional. The present institutional setting for SHI needs to be assessed and changed.

Starting in the late 1980s, these elements were successfully fused with the embrace of markets and the global economy. Economic growth since then has been rapid, stable, and inclusive, translating into strong welfare gains for the vast majority of the population. But three decades of success from reforms raises expectations for the future, as aptly captured in the Vietnamese constitution, which sets the goal of “a prosperous people and a strong, democratic, equitable, and civilized country.†? There is a firm aspiration that by 2035, Vietnam will be a modern and industrialized nation moving toward becoming a prosperous, creative, equitable, and democratic society. The Vietnam 2035 report, a joint undertaking of the Government of Vietnam and the World Bank Group, seeks to better comprehend the challenges and opportunities that lie ahead. It shows that the country’s aspirations and the supporting policy and institutional agenda stand on three pillars: balancing economic prosperity with environmental sustainability; promoting equity and social inclusion to develop a harmonious middle- class society; and enhancing the capacity and accountability of the state to establish a rule of law state and a democratic society. Vietnam 2035 further argues that the rapid growth needed to achieve the bold aspirations will be sustained only if it stands on faster productivity growth and reflects the costs of environmental degradation. Productivity growth, in turn, will benefit from measures to enhance the competitiveness of domestic enterprises, scale up the benefits of urban agglomeration, and build national technological and innovative capacity. Maintaining the record on equity and social inclusion will require lifting marginalized groups and delivering services to an aging and urbanizing middle-class society. And to fulfill the country’s aspirations, the institutions of governance will need to become modern, transparent, and fully rooted in the rule of law.

Book excerpt: The Vietnam War interrupted the lives of the men and women who served and affected the direction of their lives in a multitude of ways. This study examines disparities in health, cognition, and health literacy among Vietnam-era military Veterans during their late middle-adulthood years, ages 50 to 64, guided by the paradigms of life-course theory. The study attempts to remove selection bias inherent in analyzing Veteran populations by limiting the sample to Veterans. The sample was limited to those who completed high school or its equivalency, generally a requirement for military induction. Little literature exists that examines this age period in general, let alone for Veterans. The data used to help fill this gap are from the 2003 National Assessment of Adult Literacy, Health Literacy Component, and the RAND harmonized Health and Retirement Study, version N, and the individual HRS longitudinal files from 1996 through 2010. The main conclusion is that education is essential in these each three analyses in that health literacy levels, early cognitive dysfunction (CD), and self-report of health/access to health are all associated with educational attainment, e.g., education increases health literacy, reduces the odds of early CD, and increases odds of reporting good SRH. Other conclusions are: (1) Veterans in the 50 to 64-year age group have lower Mean Health Literacy Scores than Non-Veterans, (2) Veterans and civilians 65 and older have inadequate health literacy unless health is self-reported as very good or excellent, (3) CD can be detected prior to age 65 using the four questions in the HRS survey, (4) Vietnam Veterans do not have higher odds of transitioning into early CD when compared to those who did not serve, (5) Good/very good/excellent self-report of health is associated with decreased odds of transitioning into early CD, (6) No association was found between Veteran status and self-report of health (SRH) when controlling for the various indicators of socio-economic position (wealth, income, or health insurance), (7) Race was not significantly associated with good SRH in the Veteran population, and (8) Income was significantly associated with increased odds of reporting good health for Blacks, while wealth did the same for Whites. These results suggest that practitioners should be aware of potential issues relating the low health literacy levels, cognitive dysfunction, and the effects of socio-economic position and racial disparities in treating Vietnam Veterans.

Book excerpt: Abstract: "While there is a great deal of anecdotal evidence on the economic effects of adverse health shocks, there is relatively little hard empirical evidence. The author builds on recent empirical work to explore in the context of postreform Vietnam two related issues: (1) how far household income and medical care spending responds to health shocks, and (2) how far household consumption is protected against health shocks. The results suggest that adverse health shocks - captured by negative changes in body mass index (BMI) - are associated with reductions in earned income. This appears to be only partly - if at all - due to a reverse feedback from income changes to BMI changes. By contrast, there is a hint - the relevant coefficient is not significant - that adverse BMI shocks may result in increases in unearned income. This may reflect additional gifts, remittances, and so on, from family and friends following the health shock. Medical spending is found to increase following an adverse health shock, but not among those with health insurance. The impact for the uninsured is large, equal in absolute size to the income loss associated with a BMI shock. The lack of impact for the insured points to complete insurance against the medical care costs associated with health shocks, and is consistent with the very generous coverage of Vietnam's health insurance program in this period. The question arises: have Vietnamese households been able to hold their food and nonfood consumption constant in the face of these income reductions and extra medical care outlays? The results suggest not. For the sample as a whole, both food and nonfood consumption are found to be responsive to health shocks, indicating an inability to smooth nonmedical consumption in the face of health shocks. Further analysis reveals some interesting differences across different groups within the sample. Households with insurance come no closer to smoothing nonmedical consumption than uninsured households. Furthermore, and somewhat counterintuitively, better-off households - including insured households - fare worse than poorer households in smoothing their nonmedical consumption in the face of health shocks, despite the fact that in the case of insured households there are no medical bills associated with an adverse health event. Why the poor rely on dissaving and borrowing to such an extent, and do not apparently reduce their food and nonfood consumption following an adverse health shock while the better-off do, may be because the levels of food and nonfood consumption of the poor are simply too low relative to basic needs to enable them to cut back in the face of an adverse BMI shock."--World Bank web site.

The program, which started in 2003, did not as of 2004 include all these groups, but those who were included (about 15 percent of the population) were disproportionately poor. Estimates of the program's impact-obtained using single differences and propensity score matching on a trimmed sample-suggest that HCFP has substantially increased service utilization, especially in-patient care, and has reduced the risk of catastrophic spending. It has not, however, reduced average out-of-pocket spending, and appears to have had negligible impacts on utilization among the poorest decile.

Bartecchi served in the US Army as a Flight Surgeon during the Vietnam War and in addition to caring for wounded troops, organized medical forays by helicopter to towns throughout the Mekong Delta. During his tour in Vietnam he developed an admiration and respect for the resilient and proud Vietnamese people. In the mid-90s, he became involved with assisting the hospital improve their facility and providing training for doctors, nurses and staff there, including exchanges of medical personnel between Bach Mai and American hospitals, as assistance in the area of medical equipment and supplies, donated by organizations and corporations.
