More than 1,200 miles to the south, a similar sense of optimism pervaded the public discourse around the relationship between health care and the future of Houston's economy. Taking Pittsburgh and Houston as case studies, The Medical Metropolis offers the first comparative, historical account of how big medicine transformed American cities in the postindustrial era. Andrew T. Simpson explores how the hospital-civic relationship, in which medical centers embraced a business-oriented model, remade the deindustrialized city into the "medical metropolis." From the 1940s to the present, the changing business of American health care reshaped American cities into sites for cutting-edge biomedical and clinical research, medical education, and innovative health business practices. This transformation relied on local policy and economic decisions as well as broad and homogenizing national forces, including HMOs, biotechnology programs, and hospital privatization. Today, the medical metropolis is considered by some as a triumph of innovation and revitalization and by others as a symbol of the excesses of capitalism and the inequality still pervading American society.

Book excerpt: The concept of the metropolitan area, as best exemplified by Los Angeles, has highlighted two contradictory characteristics of the current urban scene: the dispersion of political power among a number of centers, and the presence of issues and problems whose impact transcends the jurisdiction of any one local government. In this book the author have focused their attention of the process by which organized groups have sought to identify public issues and to reach decision on them within one of the most rapidly developing and most complex metropolitan areas of the United States: Los Angeles. Beginning with a discussion of the setting and framework of the Los Angeles metropolitan area, the authors attempt to clarify the nature of the legal, political, social, and economic forces tha have shaped the present system. The second part of this work is concerned with the contenders for leadership within the area: the central city, the urban county, and the suburbs. On the basis of the collected information, the authors next pose the hypothesis that democratic ideology and group interests have combined to produce competing power centers from which groups operate while at the same time lacking sufficient resources to dominate decision making. In the final section of a number of possible alternatives that might produce decision on area-wide issues are examined, and suggestions for bringing together the various political groupings are given. Research for this work was carried out under a grant from the John Randolph Haynes and Dora Haynes Foundation. Book excerpt: In Disciplining Statistics Libby Schweber compares the science of population statistics in England and France during the nineteenth century, demonstrating radical differences in the interpretation and use of statistical knowledge. Through a comparison of vital statistics and demography, Schweber describes how the English government embraced statistics, using probabilistic interpretations of statistical data to analyze issues related to poverty and public health. The French were far less enthusiastic. Political and scientific élites in France struggled with the “reality” of statistical populations, wrestling with concerns about the accuracy of figures that aggregated heterogeneous groups such as the rich and poor and rejecting probabilistic interpretations. Tracing the introduction and promotion of vital statistics and demography, Schweber identifies the institutional conditions that account for the contrasting styles of reasoning. She shows that the different reactions to statistics stemmed from different criteria for what counted as scientific knowledge. The French wanted certain knowledge, a one-to-one correspondence between observations and numbers. The English adopted an instrumental approach, using the numbers to influence public opinion and evaluate and justify legislation. Schweber recounts numerous attempts by vital statisticians and demographers to have their work recognized as legitimate scientific pursuits. While the British scientists had greater access to government policy makers, and were able to influence policy in a way that their French counterparts were not, ultimately neither the vital statisticians nor the demographers were able to institutionalize their endeavors. By 1885, both fields had been superseded by new forms of knowledge. Disciplining Statistics highlights how the development of “scientific” knowledge was shaped by interrelated epistemological, political, and institutional considerations.
