Book excerpt: Since the previous Committee of Public Accounts reported in 2012, the Department of Health and NHS England have made progress in improving outcomes for diabetes patients. International evidence now available also suggests that the UK performs well compared to other countries in terms of outcomes for diabetes patients. However, there are significant variations in the routine care and support that diabetes patients receive, and in outcomes for diabetes patients. We are concerned that the witnesses from the Department and NHS England painted an unduly healthy picture of the state of diabetes services in England. Although an individual diabetes patient's prospects are getting better, the number of people with diabetes is rising by 4.8% a year, and performance in delivering the nine care processes and achieving the three treatment standards, which help to minimise the risk of diabetes patients developing complications in the future, has stalled. In addition, very few new diabetes patients are taking up education that could help them manage their condition, and the number of diabetes patients experiencing complications (which account for over two-thirds of the cost of diabetes to the NHS) continues to rise. This all means that the costs of diabetes to the NHS will continue to rise. In order to control these costs, the Department and NHS must take significant action to improve prevention and treatment for diabetes patients in the next couple of years.

Book excerpt: Nearly one fifth of consultant posts in emergency departments were either vacant or filled by locums in 2012. Neither the Department nor NHS England have a clear strategy to tackle the shortage of A&E consultants and there is too much reliance on temporary staff to fill gaps. The Committee raised the possibility of paying consultants more to work at struggling hospitals. Greater use in A&E of consultants from other departments could also be made, or mandate that all trainee consultants spend time in A&E, or make A&E positions more attractive through improved terms and conditions. The slow introduction of round-the-clock consultant cover in hospitals - which will not be in place before the end of 2016-17 - is also having a negative impact. More people die as a result of being admitted at the weekend when fewer consultants are in A&E. Changing this relies on the British Medical Association and NHS Employers negotiating a more flexible consultants' contract, and neither the Department nor NHS England has direct control over the timescale or details of these negotiations. Hospitals, GPs and community health services all have a role to play in reducing emergency admissions - but financial incentives to make this happen are not in place. While hospitals get no money if patients are readmitted within 30 days, there are no financial incentives for community and social care services to reduce emergency admissions. Both the Department of Health and NHS England struggled to explain to us who is ultimately accountable for the efficient delivery of local A&E services

Book excerpt: The MoD's financial settlement in the next Comprehensive Spending Review must be made in the light of the need to retain a credible deterrent capacity in the country's Armed Forces. The Committee welcomes the emphasis that the Government places on the importance of cyber defence and the commitment of resources to a new cyber strike capability. But the difficulty in identifying actors in a cyber attack makes the ability to deter that much harder. Similar questions arise in deterrence against the asymmetrical threat of terrorism as it is difficult to identify interests and groups against which a response can be legitimately targeted. The Committee is calling on the MoD to set out how it can make clear that both cyber and terrorist attack will elicit an appropriate and determined response. Looking at the nuclear deterrent, the Committee points out that the UK's ability to effect a nuclear response is not credible in dealing with all threats, and so strong conventional deterrence is also required. And given the importance of communication to the concept of deterrence, investment in diplomatic and intelligence assets must be integral to the UK's security apparatus. The Committee concludes that it would be naive to assume that a decision not to invest in the nuclear deterrent would release substantial funds for investment in other forms of security. The Committee believes that the decision on the retention of the nuclear deterrent, should be made on its own merits.

The Committee are pleased to endorse the appointment of Dr Alison Evans to this post, but recommend that at least one of the existing Commissioners be reappointed for a further term to ensure continuity, and that one of the Commissioners be an audit professional. The selection process used resulted in an unranked list of four candidates deemed "appointable" being presented to the Secretary of State for consideration. This puts too much power in the hands of the Secretary of State for an independent scrutiny post and threatens to undermine the candidate in the eyes of the public who may assume that the candidate most sympathetic to DFID was chosen. The Committee recommend that panels for ICAI Commissioner appointments should be invited to rank candidates or otherwise advise the Secretary of State as they see fit. In the longer term, it is recommended that the Committee be able to choose the Chief Commissioner from the list of candidates.

Book excerpt: Army 2020 is an ambitious programme of change and restructuring which responds to the Government's need to reduce public spending, including on defence. It seeks, for the first time, to integrate fully a regular Army of 82,500 with a larger and more frequently used Army Reserve of 30,000. The Department did not test feasibility, or adequately consult the Army, before deciding to reduce the regular Army and increase the Army Reserve. We recognise that the decision to reduce the size of the Army was driven by the need to make financial savings in a time of austerity. However, it is remarkable that the Chief of the General Staff was not involved in all stages of the decision-making process given the magnitude and importance of the change required, and its impact on the service which he commands. We were also surprised to learn that the Department did not test the feasibility of recruiting and training the number of reserve soldiers it needs by 2019. The Department is confident that it can still recruit and train the required number of reserves by 2019, but we remain to be convinced given that its confidence is based on unevidenced assumptions. For future significant reviews of the armed forces, the Department should involve relevant stakeholders fully in the decision-making process, and ensure adequate testing of the feasibility for proposed actions.

Book excerpt: Urgent action is required to establish the numbers of primates kept in captivity by private individuals across the UK and to improve their welfare. Little is known about the types and numbers of primates being kept or traded by private individuals in the UK and about the manner in which they are being kept. A ban is not ruled out, but such a step must be based on solid evidence and cannot be imposed before every opportunity to improve the operation of our existing framework has been exhausted. Defra should commission independent research to establish the extent of the problems in this area and to report with a plan of action within six months of receiving the research results. The Government must ensure that standards for primates kept in private match those required in zoos. The quality of care that these animals enjoy should be the same whether they are kept in a circus, a pet shop, a private home or a zoo. If the Committee's suggested changes prove insufficient and the evidence for doing so proves compelling, then a ban on the trade and keeping of privately kept primates should remain an option for the future. The report also calls for the Government to: ensure that legislation governing pet sales is fit for the internet age; boost public awareness of existing regulatory controls and guidance; increase the utility and efficacy of the Primate Code; expedite the publication of guidance to local authorities on the Dangerous Wild Animals Act.

Turnout for the 2010 general election was 65.1% higher than the previous two general elections, but still the third lowest since the introduction of universal suffrage. Turnout at the last general election was also low compared with turnout at the last parliamentary elections in other European Union countries. There is also evidence that a significant number of people in the UK are not registered to vote, with the most recent estimates indicating that the electoral register was between 85 and 87% complete. This would mean that approximately 6.5 million people are missing from the electoral register. In light of this, the Political and Constitutional Reform Committee agreed to conduct an inquiry into voter registration and turnout in the UK.

The Government and Research Council UK should reconsider their preference for Gold open access during the five year transition period, and give due regard to the evidence of the vital role that Green open access and repositories have to play as the UK moves towards full open access. (Authors opting for Green open access publish in any subscription journal, and then make their peer-reviewed final draft freely accessible online by self-archiving or depositing the article in a repository (either institutional or disciplinary) upon acceptance for publication.) Other recommendations include: promotion of standardisation and compliance across subject and institutional repositories; mitigation against the impact on universities of paying Article Processing Charges out of their own reserves; introduce a reduced VAT rate for e-journals; non-disclosure clauses should not be used in publishing contracts that include the use of public funds; BIS must review its consultation processes to ensure that lessons are learned from the lack of involvement of businesses, particularly SMEs, in the formation of open access policy
