Ce guide est un outil de préparation à la décision. Il propose une analyse selon deux axes - Une première partie schématise les 22 questions clés qui constituent le modèle de l'accord mutuel librement consenti et propose des exercices d'entraînement pour chacune d'entre elles. - Une seconde partie présente des cas exemplaires ainsi que les conduites possibles les concernant, en commentant chaque attitude. Les variantes de ces cas permettent de disposer de plus de 80 situations cliniques soulevant des difficultés manifestes. Ce guide s'adresse donc autant aux formateurs qui souhaitent disposer d'un outil pratique permettant l'élaboration de véritables ateliers interactifs d'apprentissage du raisonnement médical, qu'à chaque soignant qui souhaite effectuer une formation individuelle encadrée. Devant l'évolution de la médecine mobilisant des connaissances pléthoriques mises à jour de manière perpétuelle, le médecin généraliste se doit d'être performant et polyvalent. Cette évolution du soin s'accompagne d'une informatisation des cabinets de médecine générale et de l'apparition de nombreux d'outils d'aide à la décision. Quelle utilisation réelle de ces outils fait le médecin généraliste dans sa pratique quotidienne ? Méthode. Une étude qualitative, semi dirigée de septembre 2020 à janvier 2021 a été réalisée auprès des médecins généralistes de la Loire. Résultat. Les outils d'aide à la décision sont utilisés par les médecins généralistes comme outils de partage avec le patient et également comme justificatifs lors de décisions divergentes avec d'autres professionnels de santé. Ces outils sont une manière pour les médecins de mettre à jour leurs connaissances. L'accès immédiat et continu à une information de qualité simplifie le quotidien du praticien, grâce à leur fluidité ils s'intègrent aisément dans la consultation s'ils sont bien maitrisés. La décision partagée qu'ils offrent avec le patient améliore la relation perçue soigné-soignant. L'angoisse du défaut de connaissance est mise à distance et le médecin s'en trouve plus à même de se centrer sur le patient et la relation de qualité avec celui-ci. Discussion. L'usage des outils d'aide à la décision est d'autant plus pertinent que la médecine générale est une spécialité aux connaissances multiples et variées. Ces outils limitent l'incertitude en consultation et contribuent à la formation continue des médecins. Ils ne peuvent en rien remplacer le médecin, impliqué dans une relation forte et singulière avec les patients. Plus avant, au service du médecin, les outils font aujourd'hui partie intégrante des consultations de médecine générale et peuvent se mettre au service d'une décision partagée avec le patient.
