A Water Safety Plan (WSP) aims to identify hazards to drinking water quality that can be introduced at multiple points from the source to the tap. The WSP does not, however, traditionally provide for identifying hazards that could compromise drinking water quality after it reaches the household tap, such as contamination associated with water collection, storage, and treatment practices within the home. A household survey can help researchers to understand the fate of water from the time it reaches the home to the point of consumption. It can provide valuable information about the quality and reliability of water reaching the home and changes to water quality through household storage and treatment. It can also provide information on the prevalence of water-related illnesses, community perceptions and concerns, alternate or supplemental water sources, and customer satisfaction, information that may fall outside the purview of a traditional Water Safety Plan. A household survey contributes to Module 2 (System Assessment) of the Water Safety Plan, upon which the subsequent steps of hazard identification, consideration of control measures, and development of corrective actions, monitoring, and verification plans are based. Thus, the survey provides valuable information for the WSP team as the team goes through the process of system evaluation and implementation of changes resulting from the Water Safety Plan. The aim of this manual is to provide guidance on conducting a household survey as part of a Water Safety Plan for organized piped water supply systems in resource-limited settings. Specific examples intended to guide the planner in designing the survey are provided in the appendices.

Book excerpt: Legionnaires' disease, a pneumonia caused by the Legionella bacterium, is the leading cause of reported waterborne disease outbreaks in the United States. Legionella occur naturally in water from many different environmental sources, but grow rapidly in the warm, stagnant conditions that can be found in engineered water systems such as cooling towers, building plumbing, and hot tubs. Humans are primarily exposed to Legionella through inhalation of contaminated aerosols into the respiratory system. Legionnaires' disease can be fatal, with between 3 and 33 percent of Legionella infections leading to death, and studies show the incidence of Legionnaires' disease in the United States increased five-fold from 2000 to 2017. Management of Legionella in Water Systems reviews the state of science on Legionella contamination of water systems, specifically the ecology and diagnosis. This report explores the process of transmission via water systems, quantification, prevention and control, and policy and training issues that affect the incidence of Legionnaires' disease. It also analyzes existing knowledge gaps and recommends research priorities moving forward.

Spanning almost 1 million miles in the United States, distribution systems represent the vast majority of physical infrastructure for water supplies, and thus constitute the primary management challenge from both an operational and public health standpoint. Recent data on waterborne disease outbreaks suggest that distribution systems remain a source of contamination that has yet to be fully addressed. This report evaluates approaches for risk characterization and recent data, and it identifies a variety of strategies that could be considered to reduce the risks posed by water-quality deteriorating events in distribution systems. Particular attention is given to backflow events via cross connections, the potential for contamination of the distribution system during construction and repair activities, maintenance of storage facilities, and the role of premise plumbing in public health risk. Book excerpt: The President's Commission on Critical Infrastructure Protection (PCCIP) conducted a study concluding that physical and cyber threats are a clear danger (risk) to all infrastructures. Among these critical infrastructures are the nation's water supply systems. In general, these systems were not designed to withstand sophisticated cyber intrusion or other physical terrorist attacks. The size, ubiquity, and complexity of water systems do not allow more traditional methods for quantifying the probability of attacks. In the summer of 2002, the EPA mandated that all community water utilities that serve more than 3,300 people complete vulnerability assessments by the end of June 2004. The EPA has not however estabilished a standard vulnerability metholdology other than to recommend 17 different agencies that can assist in the training of personnel. The EPA guidance as well as guidance from the major water supply associations takes the form of checklists and does not take on a holistic, systems approach to risk and vulnerability assessment. This report describes the effort to survey the water infrastructure community to develop an understanding of critical assets of US water supply systems and then to conduct analysis of methodologies for water infrastructure vulnerability assessment.
