" Tout ce qui siffle n'est pas asthme " ; d'autres maladies, dont la plupart commencent à se manifester dès les premières années de vie, peuvent donner les mêmes symptômes. L'objectif de notre travail était d'isoler, chez l'enfant de moins de six ans, les éléments cliniques et para-cliniques évocateurs d'un diagnostic différentiel, à partir de dossiers médicaux et de l'étude de différents articles de la littérature médicale, afin de définir une démarche à suivre pour ne pas méconnaître ces maladies. Nous avons rétrospectivement choisi sept cas d'enfants de moins de six ans initialement considérés comme asthmatiques mais chez qui finalement une autre maladie en rapport avec leurs sifflements a été diagnostiquée, rencontrés au cours de l'expérience pneumo-pédiatrique de l'unité de Pédiatrie du centre hospitalier et universitaire de Nantes et du service de Pédiatrie de l'Hôpital de Saint-Nazaire entre novembre 2000 et novembre 2005. Ces maladies étaient une pneumocystose dans le cadre d'un déficit immunitaire congénital combiné sévère, un emphysème lobaire congénital, des séquelles de virose, une dyskinésie trachéo-bronchique, une cardiopathie congénitale, une mucoviscidose, et un double arc aortique. Dans tous les dossiers, des éléments cliniques permettaient d'envisager un diagnostic différentiel : résistance aux traitements anti-asthmatiques, retard staturo-pondéral, stridor associé. D'autres paramètres évocateurs ont été définis ainsi que la liste complète des diagnostics différentiels de l'asthme dans cette classe d'âge, à partir de différents articles médicaux. Dans six dossiers sur sept, ce fut la radiographie thoracique qui a suggéré l'existence d'un diagnostic différentiel ; dans le septième dossier, l'aspect des épreuves fonctionnelles respiratoires était déjà évocateur. Les indications des autres examens complémentaires utiles dans ce contexte ont été détaillées. Nous recommandons la réalisation d'une radiographie thoracique de face en inspiration et si possible en expiration, au moindre doute au terme de l'évaluation d'un enfant asthmatique de moins de six ans. Les épreuves fonctionnelles respiratoires sont réalisables en routine et fiables dès trois ans, leur généralisation à ces âges serait une aide précieuse notamment pour le diagnostic différentiel.
