Book excerpt: In this report the Health Committee welcomes improvements in the performance of the Nursing and Midwifery Council (NMC) over the last year, but expresses continuing concern that the progress made so far remains fragile. The Committee emphasises that it is important to ensure that the new challenges facing the NMC do not become a distraction from the continuing requirement to improve its performance of its core functions. The report is the first example of a Health Committee review of a professional regulator which builds on the work of the Professional Standards Authority (PSA). The length of time the NMC takes to conclude its fitness to practise cases has been an enduring concern for the Committee. From 2015, the NMC proposes to toughen the target period for resolving fitness to practise cases to 15 months (eventually to 12 months). The NMC has announced plans to introduce a system of revalidation by the end of 2015 which is welcomed. The Francis Report into the failings at Mid Staffs examined the role of regulators, including the NMC, in detail. The report stresses the importance of ensuring firstly that registrants understand their professional obligation to raise concerns when they see evidence of poor patient care, and secondly that patients and public are made more aware of the role of the NMC as the regulator of professional and clinical standards. The NMC should take urgent steps to raise the profile of the NMC both among its registrants and among patients and public.

Book excerpt: Transport infrastructure in some parts of the UK may get left behind under the new system to be used from next year (2015) to share out central government money for local major transport schemes. The Government has again changed the system for distributing money to local areas for major transport projects, with much more emphasis now on competition for funding. This will not necessarily help regions get a fairer share of transport funding and could make the situation worse. The Government's focus on using competition to bring in private sector funding for projects could disadvantage the regions, where there tends to be less private sector money available compared with London. Those Local Enterprise Partnerships (LEPs) which are well organised and resourced will have an advantage in bidding for funds. Other areas may lose out as a result. In addition, with greater reliance on competitive bidding for funds, there will see more money wasted on failed bids. Strategically significant schemes such as access to ports must not get overlooked. The changes are set against a backdrop where far less money is spent on transport projects outside London than in the capital. Transport infrastructure spending is £2,500 per head in London compared with £5 per head in the north east. This inequality must change. The Committee calls for the new funding arrangements to be reviewed by the end of the next Parliament to ensure that they are efficient and effective in providing funding for the most urgent transport priorities.

In January 2014 the Civil Service Commission put out to consultation two proposals on expanding ministerial influence on the recruitment process. PASC has concluded that the first option - to formalise the recruitment panel's powers to seek, and take into account, the view of the relevant minister during the appointment process - should be adopted. The Committee has warned that the adoption of the Commission's second option - allowing the Prime Minster or Secretary of State to effectively appoint a permanent secretary by choosing between two candidates 'of equivalent merit' risks the appearance that the choice will be made on grounds other than merit alone

Continuing this theme, the Committee argues that as the NHS financial situation tightens, the challenge for Monitor in supporting trusts in financial difficulty is likely to increase. The MPs emphasise the importance of addressing pressures within individual providers in the context of the local health economy. The requirement for major change in the care model can only be delivered if individual providers, and Monitor as their regulator, look beyond preserving existing structures and address the need to develop different structures to meet changing needs. The Committee also expresses concern that Monitor has not done enough to reform the system of tariff payments for providers, arguing that the current tariff arrangements often create perverse incentives for providers and inhibit necessary service change. It recommends that Monitor and NHS England should initiate a formal joint process for a prioritised review of the NHS tariff arrangements with the objective of identifying and eliminating perverse incentives and introducing new tariff structures which incentivise necessary service change

The Committee calls for: clarity about the purpose and effectiveness of rail subsidies; a clear link between policy on rail and other aspects of transport policy; a strategic approach to policy-making which does not sacrifice democratic accountability, takes passenger interests more clearly into account, upholds safety standards and develops a strategy for improving the security of the rail network; greater transparency about the costs of rail to ensure that new investment, operator alliances, profit or wastage levels and various forms of franchise can be better compared and evaluated; more modern, flexible fares and ticketing options and a clear long-term policy on regulated fares that rules out even higher fares for commuters on peak time trains; a strong single economic regulator for the rail industry; effective industry leadership via the Rail Delivery Group, scrutinised closely by the regulator to ensure that this strategic body acts in the best interests of the farepayer and taxpayer, rather than simply of established rail interests; devolution for some rail franchises to local or regional bodies. The Committee recommends that the Rail Delivery Group, made up of senior industry leaders, should spearhead the swift implementation of innovative ticketing technology and work with Passenger Focus to develop a clear strategy for improving retail facilities on stations and trains.
