Book excerpt: In response to the COVID-19 outbreak, many governments recommended or mandated the use of personal protective equipment (PPE) in public and private settings to prevent the spread of the virus, resulting in a sharp surge in demand. However, PPE manufacturers were not able to increase production fast enough to meet demands. For this reason, many govern-ments worldwide failed to secure enough PPE to protect their frontline workers in hospitals and clinics. This study aimed to compare the procurement, production, and distribu-tion challenges that PPE manufacturers, distributors, and governments faced during the 2020 PPE shortage and the strategies that governments in China, Germany, and the United States applied to secure their PPE supply. The purpose of this study was further to derive recommendations for public decision makers to secure medical supplies during health emergencies sim-ilar to COVID-19. An exploratory case study of these three countries was conducted based on 12 semi-structured interviews with supply chain management and healthcare experts, PPE manufacturers and distributors, and one govern-ment representative between September and October 2020. Additionally, secondary data from newspaper articles, governments, press releases, and websites were used to enrich the research through data triangulation. This study's findings revealed many challenges, such as fierce competition, the time needed for acquiring new suppliers, lack of workforce, raw mate-rials, and machines, limited transportation capacities, and export re-strictions, making it challenging to secure enough PPE. Additionally, the results show that the observed governments actively engaged in the healthcare supply chain by financing, securing, and distributing PPE while pursuing different strategies. The conducted research contributes to the discussion on the disaster man-agement literature by presenting challenges and strategies that emerged in this crisis. Furthermore, it provides recommendations for public decision makers to improve their preparedness and responsiveness for future health emergencies.

Book excerpt: This study investigates the forces that contributed to severe shortages in personal protective equipment in the US during the COVID-19 crisis. Problems from a dysfunctional costing model in hospital operating systems were magnified by a very large demand shock triggered by acute need in healthcare and panicked marketplace behavior that depleted domestic PPE inventories. The lack of appropriate action on the part of the federal government to maintain and distribute domestic inventories, as well as severe disruptions to the PPE global supply chain, amplified the problem. Analysis of trade data shows that the US is the world's largest importer of face masks, eye protection, and medical gloves, making it highly vulnerable to disruptions in exports of medical supplies. We conclude that market prices are not appropriate mechanisms for rationing inputs to health because health is a public good. Removing the profit motive for purchasing PPE in hospital costing models and pursuing strategic industrial policy to reduce the US dependence on imported PPE will both help to better protect healthcare workers with adequate supplies of PPE.

During a time of global pandemic, society attempted to slow the spread of disease before vaccinations were developed through techniques such as social distancing and the use of Personal Protective Equipment (PPE). As a result, the demand for PPE surged to historical records, and supply was not able to meet demand. This resulted in worldwide shortages for all different types of PPE, and even healthcare providers struggled to obtain the necessary PPE to protect themselves while treating COVID-19 patients. Therefore, the intent of this research is to provide a list of best practices and recommendations for contingency planning after COVID-19, specifically for the PPE industry. Current literature was reviewed and analyzed, and best practices were garnered from the literature. Three industry executives were also interviewed about their thoughts and experiences with PPE during the COVID-19 pandemic. Their opinions and recommendations were compared and contrasted with the best practices from literature, and ultimately a list of holistic best practices was developed and recommended for contingency planning. Best practices for contingency planning include enhancing the Strategic National Stockpile infrastructure, bringing more PPE manufacturing onshore in the U.S., improving end-to-end PPE inventory monitoring, filtering out misinformation on social media platforms, establishing clear guidelines on the reuse of PPE, and developing innovative ways for consumers to dispose of masks in order to mitigate environmental damage.

Book excerpt: Early in the COVID-19 pandemic, a global shortage of hospital gowns, gloves, surgical masks, and respirators caused policymakers around the world to panic. This paper examines international trade in this personal protective equipment (PPE) during the crisis, with a focus on China, the European Union, and the United States. As the pandemic first hit, China increased imports and decreased exports of PPE, removing considerable quantities of supplies from global markets. For the European Union and United States, the decrease in their imports from China was not immediately replaced by increased trade from other foreign suppliers. Early shortages led to EU and US export controls on their own, domestically produced PPE and other extraordinary policy actions, including a US effort to reserve for itself supplies manufactured in China by a US-headquartered multinational. By April 2020 China's exports had mostly resumed, and over the rest of 2020 its export volumes of some products surged, more than doubling compared to pre-pandemic levels. But China's export prices also skyrocketed and remained elevated through 2020, reflecting severe and continued shortages. This paper documents these facts. It also explores these and other government actions, such as US trade war tariffs and the emergence of over $1 billion of US industrial policy to build out its domestic PPE supply chain, as well as potential lessons for future pandemic preparedness and international policy cooperation.

The original Blue Book was a comprehensive publication used widely in health-care centers and government agencies to assist in the adoption of national guidance. It also provided support to committed medical directors and managers to make improvements and presented practical information on waste-management techniques for medical staff and waste workers. It has been more than ten years since the first edition of the Blue Book. During the intervening period, the requirements on generators of health-care wastes have evolved and new methods have become available. Consequently, WHO recognized that it was an appropriate time to update the original text. The purpose of the second edition is to expand and update the practical information in the original Blue Book. The new Blue Book is designed to continue to be a source of impartial health-care information and guidance on safe waste-management practices. The editors' intention has been to keep the best of the original publication and supplement it with the latest relevant information. The audience for the Blue Book has expanded. Initially, the publication was intended for those directly involved in the creation and handling of health-care wastes: medical staff, health-care facility directors, ancillary health workers, infection-control officers and waste workers. This is no longer the situation. A wider range of people and organizations now have an active interest in the safe management of health-care wastes: regulators, policy-makers, development organizations, voluntary groups, environmental bodies, environmental health practitioners, advisers, researchers and students. They should also find the new Blue Book of benefit to their activities. Chapters 2 and 3 explain the various types of waste produced from health-care facilities, their typical characteristics and the hazards these wastes pose to patients, staff and the general environment. Chapters 4 and 5 introduce the guiding regulatory principles for developing local or national approaches to tackling health-care waste management and transposing these into practical plans for regions and individual health-care facilities. Specific methods and technologies are described for waste minimization, segregation and treatment of health-care wastes in Chapters 6, 7 and 8. These chapters introduce the basic features of each technology and the operational and environmental characteristics required to be achieved, followed by information on the potential advantages and disadvantages of each system. To reflect concerns about the difficulties of handling health-care wastewaters, Chapter 9 is an expanded chapter with new guidance on the various sources of wastewater and wastewater treatment options for places not connected to central sewerage systems. Further chapters address issues on economics (Chapter 10), occupational safety (Chapter 11), hygiene and infection control (Chapter 12), and staff training and public awareness (Chapter 13).
