Book excerpt: "African American (AA) women in Mississippi have a higher burden of infant mortality, especially those with low income and educational levels, as compared to other regions of the country. However, it is unknown if abnormal gestation weight gain (GWG) is contributing to this increased burden. This quantitative cross-sectional study examined the association between GWG (25-35lbs), level of education, income, and infant mortality among AA women with normal prepregnancy body weight who were between 30 to 47 years old in the state of Mississippi from 2016 to 2018. Biopsychosocial and ecosocial models were used as the theoretical foundation to design the study and assess the findings. Secondary data were obtained from the Mississippi Pregnancy Risk Assessment Monitoring System and binary logistic regression analysis was used for inferential analysis. The findings revealed no statistically significant association between normal GWG (25–35 lbs) and infant mortality when compared to AA women with GWG ≥36 lbs. Income was not statistically significant in predicting GWG. In addition, education levels were not statistically significant in predicting infant mortality. Although the study results were not significant, the findings of this research will contribute to positive social change by providing knowledge to inform the following public health agencies in the state. This study encourages public health researchers to search for additional factors that contribute to abnormal GWG as a known risk factor for an increased risk of infant mortality and inform efforts to reduce infant mortality in the state of Mississippi."--Abstract.

It builds on the conceptual framework that underscored the 1990 weight gain guidelines and addresses the need to update them through a comprehensive review of the literature and independent analyses of existing databases. The book explores relationships between weight gain during pregnancy and a variety of factors (e.g., the mother's weight and height before pregnancy) and places this in the context of the health of the infant and the mother, presenting specific, updated target ranges for weight gain during pregnancy and guidelines for proper measurement. Given the unprecedented environment in the United States in which two-thirds of the adult population meets the criteria for being overweight or obese, the implications for women in the reproductive age period are unique in the history of the country. The concerns for maternal and infant health are real. The questions and answers tackled by committee members and workshop participants were not easy. Nevertheless, having an opportunity to explore what is known, examine the gaps in knowledge, and explore what to do now and in the future build a pathway for further inquiry and action. This report summarizes the workshop proceedings and highlights key themes that deserve further attention. The participants in this workshop describe what is known about recent trends in maternal weight gain and the impact of maternal weight during pregnancy on the health of mothers and their children. The workshop provided a valuable opportunity to assess trends that have occurred since the publication of an earlier study by the Institute of Medicine (IOM), which included guidelines for recommended weight gain during pregnancy.

Book excerpt: Abstract: Objective: To describe African American and White women's perceptions of weight gain, physical activity, and nutrition during pregnancy and to explore differences in perceptions by race. Design: Qualitative interview study. Setting: Two Ob/Gyn clinics in South Carolina, USA. Participants: Thirty pregnant women (15 African American, 15 White) between 20 and 30 weeks gestation, equally represented across pre-pregnancy BMI categories (10 normal weight, 10 overweight, and 10 obese). Findings: White women more frequently described intentions to meet weight gain, physical activity, and dietary guidelines in pregnancy than African American women. African American women were more concerned with inadequate weight gain while White women more commonly expressed concerns about excessive weight gain. More White women discussed the importance of physical activity for weight management. Regardless of race, few women described risks of excessive weight gain or benefits of physical activity as it relates to the baby's health. The primary cited barrier of healthy eating was the high cost of fresh produce. Key conclusions and implications for practice: Several knowledge gaps as well as race differences were identified in women's perceptions and intentions toward weight gain, physical activity, and nutrition during pregnancy. Future interventions should seek to educate women about common misperceptions. It may be necessary to culturally tailor gestational weight gain interventions to optimise health outcomes. Highlights: Knowledge gaps exist in pregnant women's perceptions of weight-related behaviours. Perceptions of weight-related behaviours during pregnancy differ by race. Interventions should educate pregnant women about common misperceptions.

Book excerpt: This study sought to analyze the relationship between diabetes, hypertension, and low birth weight (LBW) amongst non-Latinx African American women. The Oregon Health Assessment Tool (OPHAT) served as a data base for the sample population. The sample population consisted of African American women between the ages of 25-39 that gave birth between 2013-2014. Relative risk ratios were calculated to determine the likelihood of delivering a LBW infant. The exposures for the risk ratios were: Prepregnancy diabetes (DM), gestational diabetes (GM), prepregnancy hypertension (HTN), and gestational hypertension (GHTN). An analysis of conditional factors that impact birth weight was also conducted. The percentage of LBW that occurred after exposure to the conditional factors was calculated. The conditional factors in this study included: Level of education mother received, amount of prenatal care received (PNC), mother's insurance, mother's weight gain during pregnancy, and the use of tobacco during the first trimester of pregnancy. Relative risk ratios revealed a risk ratio of 11.9% for women that experienced GDM, and a risk ratio of 7.3% for women that had DM. Additionally, the relative risk ratios revealed a risk ratio of 12.5% for women that experienced GHTN and a risk ratio of 12.3% for women that had HTN. This data indicates that GDM and GHTN are higher risk factors for delivering a LBW infant. However, LBW amongst non-Latinx African American is a complex issue to address due to the numerous conditional factors that influence the likelihood of developing chronic disease and delivering a LBW infant.
