Some individuals with low levels of risk factors succumb early to disease while many others with a high risk profile do not. CHD clusters in families and is manyfold increased in first degree relatives of persons with an early onset of the disease. Such studies provide compelling evidence of the high degree of heritability of CHD and its risk factors. This book consolidates the available evidence for the roles of genetics in atherosclerosis, its correlates and its sequelae. It presents and discusses the methodology currently used to elucidate the role of genetics. Separate parts focus on evidence of familial aggregation and ethnic variability of the disease and on monogenic and polygenic inheritance modes including all the recent findings and innovation. The book also contains chapters on the genetic aspects of vessel wall processes, such as early structural findings in histological studies and the variability of coronary anatomic patterns. Polymorphisms at the DNA level (RFLP) are detailed and reviewed. Directions for future research in the exciting and fast developing realm of genetic epidemiology are outlined and the major preventive and public health implications are discussed. Cardiovascular disease results from complications of a silent and chronic arterial disease: atherosclerosis. The challenge for the practitioner is adapting diagnostic and therapeutic responses to prevent this common and complex disease. Dyslipidemia, are disorders of the metabolism of soluble transporters of lipids in extracellular spaces of the human body (including blood), called lipoproteins. They are major cardiovascular risk factors, causally related with atherosclerosis and are themselves multifactorial diseases, resulting from interactions between genetic and environmental factors. The study of genetic factors has recently taken a new path with the study of DNA as an experimental object. More than fifty genes of lipoprotein metabolism have been identified in both their physiological actions and their contribution to the pathogenesis of human dyslipidemia. The diversity of observations has refined our current knowledge of the control of lipid metabolism and energy homeostasis in living organisms beyond the limits of the cardiovascular system (e.g., brain, immune system, and development). These studies have given way to a shake-up of former phenotypic classifications, distinguishing new entities, defining targeted therapeutic strategies, providing a basis for different patterns of disease distribution in human populations.

The atherosclerotic process begins early in life, progresses during the middle years, and usually culminates in clinical disease towards the later years of the life span. Since atherosclerosis is a multifactorial disease, and many of the "risk factors" are time- and age related, it has been difficult to sort out intrinsic aging from environmental factors that operate over many years. Furthermore, the role of genetic factors remains unknown. This workshop has produced much worthwhile information that is helping elucidate the impact of age on atherogenesis. Important strides have been made in understanding the role of changes in the arterial wall and of lipoproteins, platelets, and monocyte-derived macrophages in the disease process. In parallel, our understanding of the biology of aging has increased sufficiently so that these two areas of interest can now profitably intersect. The proceedings of this successful workshop emphasize that there is much to be gained by continued interaction between those scientists interested in the biology of aging at all levels and those interested in the atherosclerotic process. Hopefully, we may eventually progress in our understanding and reach the stage when atherosclerosis will no longer be an inexorable concomitant of human aging. Edwin L. Bierman, M. D.
