Pour évaluer la place des médecins généralistes du Val d'Oise (95), nous avons réalisé une enquête auprès de 200 d'entre eux choisi par tirage au sort. Les résultats de notre enquête montrent qu'un tiers des médecins généralistes font des frottis au cabinet; l'obstacle essentiel à la non-réalisation étant la non-formation à ce geste simple. De plus, beaucoup d'entre eux prescrivent des frottis à faire dans des laboratoires. On note également qu'après incitation auprès des patientes (relation médecin-patiente et documentation), la couverture du dépistage peut être élargie de façon importante.

Objectif principal: décrire les modalités d'une consultation avec pratique du FCU en MG. Méthodes : étude observationnelle, descriptive, isolée et prospective. Sur 242 MSU accueillant des internes de MG, 171 ont participé. Recueil des données effectué par les internes de Mai 2015 à Mai 2016. Etude réalisée en simple aveugle. Les questionnaires étaient remplis par les internes après observations des consultations gynécologiques avec FCU réalisés par les MSU. Résultats : 87 MSU (65.52 % de femmes) ont réalisé des FCU. Dans 85.11 % des cas, le MSU était le médecin traitant de la patiente. 188 consultations ont été observées. Configuration du cabinet : espace dédié au déshabillage (31.03 %), table équipée (88.51 %). 88.30 % des patientes consultaient seules. Le MSU avait un rappel de date de FCU dans le dossier médical dans 55.32 % des cas. Les patientes consultaient spécifiquement pour un motif gynécologique dans 84.57 % des cas et plus particulièrement pour la réalisation du FCU dans 56.60 % des cas. Lorsque le motif n'était pas le FCU, il a été proposé dans 91.30 % des cas. Pour 90,43 % des patientes, un premier FCU avait déjà été réalisé et pour 65.88% d'entre elles, par le même médecin. Les explications pour la recherche HPV ont été données à 17.02 % patientes (dont 34 % avec demande d'accord). Information sur les modalités de communication des résultats du FCU faite pour 69.68 % des patientes. Consignes sur le déroulement du déshabillage données à 48.94 % patientes. Préparation oestrogénique prescrite en amont ou application de lubrifiant sur le speculum pour pallier la sécheresse vaginale pour 46.03 % des patientes ménopausées. Une discussion avec la patiente a eu lieu pendant la réalisation du FCU (95.21 %). Conclusion : caractéristiques associées à la pratique du FCU : cadre d'exercice adapté, proposition d'une consultation dédiée. Suivi gynécologique de la femme réalisé par le MG. L'accord préalable de la femme est rare pour la recherche HPV. Les modalités de communication des résultats diminuent l'anxiété. Indices semblant indispensables au bon déroulement du FCU : respect de l'intimité, prise en compte du confort de la femme ménopausée, écoute et échange lors de l'examen, utilisation de mots simples dans un climat de prévention.
