Moins fréquents, les mélanomes ont un mauvais pronostic au stade avancé. Le principal facteur de risque des cancers cutanés est l’exposition solaire. Selon certaines études, la population française s’estime bien informée sur les risques de cancer liés à l’exposition solaire. Malgré ce bon état de connaissance et la multiplication des messages de prévention, l’application des mesures de protection solaire reste insuffisante. Considérant toutes ces données, il est apparu important de comprendre les raisons à l’origine de la poursuite de ces comportements à risques. Objectifs : explorer les freins des patients à l’application des conseils en matière de prévention primaire des cancers cutanés. Identifier le rôle du médecin généraliste dans la lutte contre ces freins. Matériels et méthode : étude qualitative par entretiens semi-dirigés auprès de participants adultes résidant en Occitanie de l’Ouest. Analyse thématique avec triangulation des données à chaque étape de l’analyse et réalisation d’un schéma intégratif des résultats. Résultats : 10 entretiens ont été effectués en présentiel entre mars 2021 et mars 2022. L’exposition solaire était souvent associée à la période estivale chez les participants. Ils décrivaient dans l’ensemble un comportement adapté en cas d’exposition solaire programmée. A l’inverse, les situations non programmées étaient pourvoyeuses de manque de protection et donc décrites comme un frein. Les autres principaux freins étaient l’oubli, le manque d’automatisme, les bénéfices de l’exposition solaire, la volonté d’être bronzé, le manque de conscience du risque ou encore l’expérience désagréable liée à l’utilisation de crème solaire. Conclusions : cette étude a permis de mettre en évidence de nombreux freins à la protection solaire en population générale mais également d’envisager des perspectives d’amélioration de la prévention solaire, en orientant les campagnes de sensibilisation vers les populations les plus jeunes et en proposant à des multiples acteurs de s’impliquer dans cette démarche de prévention, dont un des premiers est le médecin généraliste.

Book excerpt: Importance : L'incidence du mélanome cutané est en augmentation à La Réunion rejoignant ainsi les plus fortes incidences mondiales. Le facteur de risque principal du mélanome est l'exposition solaire pendant l'enfance. Or la prévention solaire reste insuffisante dans les écoles à La Réunion. Objectif : Explorer les représentations, les freins et les motivations face à la protection solaire chez les principaux acteurs de la protection solaire des enfants : leurs parents, les enseignants et les enfants eux-mêmes. Méthode : Étude qualitative par entretiens individuels semi-dirigés chez 14 enfants d'école primaire à La Réunion, 13 parents d'élèves et 13 enseignants. Les données ont été collectées de septembre 2019 à novembre 2020. La suffisance théorique des données a été recherchée. Les entretiens ont été enregistrés, retranscrits, codés, triangulés, analysés puis modélisés utilisant le principe de théorisation ancrée. Résultats : L'ensemble des 40 participants ont exprimé leurs habitudes de protection qui variaient selon leur exposition solaire. La protection était moindre lors des activités quotidiennes y compris en période scolaire. Les contraintes pratiques et financières étaient identifiées par les parents. Les enseignants relevaient le manque de moyens avec des infrastructures inadaptées et une absence de formation. Par ailleurs, la responsabilité de la protection des enfants était un point de désaccord entre parents et enseignants. Les enfants limitaient l'application des mesures protectrices principalement par manque de confort. Conclusion : La protection solaire des enfants en milieu scolaire est sous la responsabilité à la fois scolaire, parentale et sociale. Une amélioration de la communication entre les acteurs scolaires et les parents est nécessaire. Des campagnes de prévention et un aménagement des lieux de vie scolaire pourraient permettre à une meilleure protection de la population infantile.
