But why should liberals favor lockdowns, when all the social science research shows that those who suffer most are the economically disadvantaged, without access to good internet or jobs that can be done remotely; that the young will pay the price of the pandemic in future taxes, job prospects, and erosion of public services, when they are already disadvantaged in comparison in terms of pension prospects, paying university fees, and state benefits; and that Covid's impact on the Global South is catastrophic, with the UN predicting potentially tens of millions of deaths from hunger and declaring that decades of work in health and education is being reversed. Toby Green analyses the contradictions emerging through this response as part of a broader crisis in Western thought, where conservative thought is also riven by contradictions, with lockdown policies creating just the sort of big state that it abhors. These contradictions mirror underlying irreconcilable beliefs in society that are now bursting into the open, with devastating consequences for the global poor.

The economic consequences for society at large are potentially catastrophic. The health and social care sectors have reacted by providing emergency care on an unprecedented scale, while the scientific community has focused on developing new treatments and a vaccine to prevent future waves of the pandemic. Evidence is emerging to suggest that certain conditions, such as obesity and hypertension, predispose some individuals to a worse outcome if they become infected, and that women may be less likely to die from COVID-19 than men. It is also currently believed that pregnant women are at no greater risk than the general population. There is an urgent need to determine whether these early observations are correct. Furthermore, we need to be sure that pregnancy outcomes are not affected by COVID-19 and that SARS-CoV-2 is not transmitted to the fetus during pregnancy or labour, nor to the infant through breast milk. There are so many questions that need to be answered to optimise care, avoid harm, reduce anxiety amongst women and their families, and inform health professionals and policymakers. We also need to understand the unintended consequences of the global lockdown on women’s health in general. For example, have rates of domestic violence risen; to what extent has women’s mental health been affected and have women successfully adapted or devised new coping mechanisms; have women been denied access to gynaecological treatments during the lockdown, including safe abortion and, if so, with what impact on their health and wellbeing; has the female work-force suffered disproportionately in economic terms; have national and international recommendations and policies been sufficiently gender neutral; have breastfeeding rates been adversely affected; will COVID-19 make attainment of the UN SDGs more difficult, etc.? In keeping with the Scope & Mission of Frontiers in Global Women’s Health, this Research Topic aims to provide a multi-disciplinary platform to answer important COVID-19 related questions that specifically impact upon women’s health and wellbeing, particular in resource-poor settings. The Topic Editors welcome a broad range of contributions including Original Research, Reviews, Commentaries, Study Protocols and Systematic Reviews. Book excerpt: The ‘novel coronavirus disease’ (COVID-19) has caused significant global morbidity, mortality and economic damage on a scale similar to the influenza pandemic of 1918. The causative ‘severe acute respiratory syndrome virus 2’ (SARS-CoV-2) is a RNA virus which is evolving rapidly, accumulating mutations, and existing as a cloud of variants with quasispecies diversity. We have tens of thousands of variants, of which currently two are ‘variants of interest’ (Lambda, Mu) and another four ‘variants of concern’ (Alpha, Beta, Gamma, Delta, Omicron) to the World Health Organization (WHO). Variants of interest often emerge as variants under investigation in one or more countries, as seen with the Omicron (initially denoted as Nu in South Africa). They become variants of concern when there is evidence of increased transmissibility, greater case severity, loss of neutralization by antibodies, and reduced effectiveness of countermeasures. Some SARS-CoV-2 variants can infect new hosts (for instance, the Alpha, Beta and Gamma variants of concern can infect wild type mice which is a species that cannot be vaccinated or controlled easily), and/or reduce the effectiveness of vaccines and therapies (especially Beta and Delta); diagnostic tests have also been reported to be affected. Currently, we do not yet have any ‘variant of high consequence’ with confirmed attributes such as widespread diagnostic test failures, significant reduction in the effectiveness of vaccines and/or therapies, with greater disease severity and deaths. It remains to be seen if Omicron would be the first to be declared by the WHO as a variant of high consequence; more variants of concern are also likely to be declared by the WHO over the next couple of years. Therefore, our ability to control and conquer this pandemic over the long run is dependent on predicting and keeping up with the evolution of this virus, its new hosts, as well as understanding and mitigating its impact on diagnostics, vaccines and therapies.
