Book excerpt: The French collaboration ClaRyS is developing fast detectors for online ion range monitoring to reduce treatment uncertainties in hadron therapy. Prompt gamma imaging (PGI), based on the detection of prompt gamma rays following nuclear reactions along the ion track, is a promising technique for the real time verification of ion range, especially when time of flight (TOF) is used to discriminate the prompt gamma rays issued from the patient from the large background of secondary radiation [1].A Compton Camera prototype has been developed by CLaRyS for PGI. Using Compton kinematics, the photon emission point is reconstructed using either an iterative process, or, if the camera is used in conjunction with a beam-tagging hodoscope, an analytic process involving the intersection of a line and a cone. The iterative reconstruction yielded a precision of 2 mm in the measured range for 160 MeV protons incident on a PMMA phantom. Whilst much quicker than the iterative reconstruction, the analytic reconstruction is less precise, but may be improved using TOF with 100 ps resolution or less. This has been studied by means of a Monte Carlo simulation.CLaRyS has recently acquired a fast photon counting Temporal camera developed by Damavan Imaging [2]. Although originally conceived for standalone nuclear dismantling operations, the custom Temporal Compton camera readout will be synchronized with the readout of a beam hodoscope. The camera is undergoing characterisation to investigate its suitability to be used as an online range monitor in hadron therapy. Results from the two studies will be presented and discussed.[1] J. Book excerpt: Protontherapy is a promising technique for tumor treatment that is becoming more and more widespread. The sharply peaked profile of the dose and the finite particle range allow for very conformal treatment and better sparing of healthy tissue beyond the tumor, but he precise delivery also proves to be the biggest challenge of the technique. Errors in range are a considerable risk in proton therapy and no range monitoring method is currently systematically used for quality control. In this manuscript, an indirect method of measuring the dose distribution, via the detection of secondary prompt gamma radiation emitted along the beam path, is explored. Two different one-dimensional collimated camera concepts, a multi-parallel-slit camera and a knife-edge slit camera are compared with regards to their potential use. Both systems are optimized via Monte Carlo simulation and measurements are presented for validation. The comparison is made on the basis of the precision with which a shift in the prompt gamma profile falloff edge can be retrieved by comparison with a reference profile as well as the spatial resolution, the cost, weight and bulkiness of the system and guidelines are given for choosing the best configuration for different requirements. Similar values can be obtained for both concepts, reaching a precision for the retrieval of the falloff edge of around 2 mm for a single pencil beam spot of 5×107 protons. This study concludes with an outlook on future developments and areas of investigation with the goal of reaching clinical applicability of a prompt gamma detection system.

Book excerpt: In vivo range verification is desirable to understandthe range uncertainties, minimizing beam delivery errors duringhadron therapy. The aim of this project is to develop a novelprompt gamma-ray imaging (PGI) prototype detector for theabsolute and relative range verification of hadron therapy, whichcan be used in clinical tests. The detection system consists ofarrays of scintillator LFS (Luteium Fine Silicate) crystals coupledto silicon photomultiplier (SiPM) arrays. A passive tungstenmulti-slit collimator has been constructed in-house and used forthe PG profile along the depth of serval phantoms. In themeantime, the proton dosimetry is estimated using a microprobethat is inserted into the phantom. The correlated measurementsfrom both detectors will be used for the determination of therelationship between u201cBragg peaku201d (BP) and PG peak. We willpresent preliminary results from in-beam measurements usingthe MC40 cyclotron at Birmingham with a proton beam at 38MeV . Time-of-flight (ToF) resolution, energy response andgamma-ray profiles will be shown for an energy windowcorresponding to 3 u2013 7MeV gamma rays. The results are alsocompared to a Monte Carlo (MC) Geant4 simulation model thatwas developed within this project.

Book excerpt: Hadrontherapy uses a beam of charged particles (protons or carbon ions) to destroy tumour cells. Compared to conventional radiotherapy hadrontherapy represents a promising technique to treat cancer because of its concentrated dose delivery within the Bragg peak. Monitoring of dose delivery can thus be addressed by verifying the range of the beam particles.CLaRyS, which stands for online control of hadrontherapy by secondary radiation, is a project regrouping several laboratories that aims at improving dose delivery control by imaging the emission of prompt-gamma rays issued from inelastic collisions of the beam particles with target nuclei. This requires to detect prompt-gamma rays using either a mechanical or the electronic collimation resulting from the Compton interactions of the gamma rays in a scatterer. As part of this development, a high-performance and compact Back-End (BE) electronics was developed at CPPM to perform data acquisition from the detectors through 34 optical links of 3 Gb/s. Data are packed and sent to a PC on a 1 Gb/s Ethernet link, guaranteeing 300 Mb/s throughput as specified for the Compton camera by Monte Carlo simulations for a beam intensity of 10 power of 8 proton/s. My thesis work comprised the development of the FPGA firmware for the BE electronics of CLaRyS, while contributing to the development and evaluation of its detection system. Measurements with the collimated camera and beam hodoscope were performed on the 65 MeV proton beam line of the Mediterranean Protontherapy Institute in Nice.

Book excerpt: This thesis work has been carried out within the CLaRyS French collaboration, which is involved in the development of a multi-collimated gamma camera and a Compton camera for the application in ion beam therapy monitoring through prompt-gamma detection and in nuclear medicine examinations. The main goal of the thesis was the complete characterization of the camera detector components, and the test of the whole systems on clinical ion beams. In parallel, simulation works have been performed to assess the performance of the Compton camera for the measurement of ion range during proton and carbon therapy, and for single photon emission computed tomography application in the nuclear medicine field.

Book excerpt: Proton Therapy Physics goes beyond current books on proton therapy to provide an in-depth overview of the physics aspects of this radiation therapy modality, eliminating the need to dig through information scattered in the medical physics literature. After tracing the history of proton therapy, the book summarizes the atomic and nuclear physics background necessary for understanding proton interactions with tissue. It describes the physics of proton accelerators, the parameters of clinical proton beams, and the mechanisms to generate a conformal dose distribution in a patient. The text then covers detector systems and measuring techniques for reference dosimetry, outlines basic quality assurance and commissioning guidelines, and gives examples of Monte Carlo simulations in proton therapy. The book moves on to discussions of treatment planning for single- and multiple-field uniform doses, dose calculation concepts and algorithms, and precision and uncertainties for nonmoving and moving targets. It also examines computerized treatment plan optimization, methods for in vivo dose or beam range verification, the safety of patients and operating personnel, and the biological implications of using protons from a physics perspective. The final chapter illustrates the use of risk models for common tissue complications in treatment optimization. Along with exploring quality assurance issues and biological considerations, this practical guide collects the latest clinical studies on the use of protons in treatment planning and radiation monitoring. Suitable for both newcomers in medical physics and more seasoned specialists in radiation oncology, the book helps readers understand the uncertainties and limitations of precisely shaped dose distribution.

Book excerpt: The Gamma Ray Imaging (GRI+) Compton camera at the University of Liverpool is investigated for its suitability for range and dose verification of Proton Therapy (PT). The verification is done by measuring the prompt gamma emissions (PG) during treatment; providing the advantage of online treatment verification[1]. The simulation will be tested against measurements at Clatterbridge Cancer Centre (CCC), where 60 MeV proton beam is used for treatment of eye cancer, and is aimed to be applicable at higher proton beam energies.[2]The PG emitted following inelastic scattering of the proton off target nuclei are in the high energy region ranging from 2 to 10 MeV. Optimisation of the GRI+ set up for the measurement of high energy gamma rays is being performed by using simulation and laboratory measurements prior to clinical measurements at the CCC. This poster will report on the characterisation of the GRI+ Compton Camera using Monte Carlo simulation. A code written in the NPTool framework based on the Geant4 and Root packages is used. Protons and carbon ions are used in several centers all over the world for the treatment of different solid tumors. Typical indications are ocular malignancies, tumors of the base of the skull, hepatocellular carcinomas and various sarcomas. The physical characteristics of the charged particles (Bragg peak) allow sparing of much more normal tissues than it is possible using conventional X-rays, and for this reason all pediatric tumors are considered eligible for protontherapy. Ions heavier than protons also display special radiobiological characteristics, which make them effective against radioresistant and hypoxic tumors. On the other hand, protons and ions with high charge (Z) and energy (HZE particles) represent a major risk for human space exploration. The main late effect of radiation exposure is cancer induction, and at the moment the dose limits for astronauts are based on cancer mortality risk. The Mars Science Laboratory (MSL) measured the dose on the route to Mars and on the planet’s surface, suggesting that a human exploration missions will exceed the radiation risk limits. Notwithstanding many studies on carcinogenesis induced by protons and heavy ions, the risk uncertainty remains very high. In this research topic we aim at gathering the experiences and opinions of scientists dealing with high-energy charged particles either for cancer treatment or for space radiation protection. Clinical results with protons and heavy ions, as well as research in medical physics and pre-clinical radiobiology are reported. In addition, ground-based and spaceflight studies on the effects of space radiation are included in this book. Particularly relevant for space studies are the clinical results on normal tissue complications and second cancers. The eBook nicely demonstrates that particle therapy in oncology and protection of astronauts from space radiation share many common topics, and can learn from each other.

Book excerpt: The Department of Defense, through the Assembled Chemical Weapons Alternatives program, is currently in the process of constructing two full-scale pilot plants at the Pueblo Chemical Depot in Colorado and the Blue Grass Army Depot in Kentucky to destroy the last two remaining inventories of chemical weapons in the U.S. stockpile. These two storage sites together account for about 10 percent of the original U.S. chemical agent stockpile that is in the process of being destroyed in accordance with the international Chemical Weapons Convention treaty. Unlike their predecessors, these facilities will use neutralization technologies to destroy agents contained within rockets, projectiles, and mortar rounds, requiring the use of specially designed equipment. As part of its focus on safe operation of the planned facilities, the Program Manager for Assembled Chemical Weapons Alternatives asked the National Research Council (NRC) to conduct a study to offer guidance on the application of process safety metrics at the Pueblo Chemical Depot and Blue Grass Army Depot. Process safety is a disciplined framework for managing the integrity of operating systems, processes and personnel handling hazardous substances, and operations by applying good design principles, engineering, and operating practices. Process Safety Metrics at the Blue Grass and Pueblo Chemical Agent Destruction Pilot Plants discusses the use of leading and lagging process safety metrics that could provide feedback on the effectiveness of controls to mitigate risks and minimize consequences of potential incidents. The book makes several recommendations that will facilitate the development and application of process safety metrics at both sites.
