Book excerpt: Problématique : la chimiothérapie néoadjuvante est devenue le traitement standard des cancers du sein localement avancés. L'identification de facteurs clinico-pathologiques prédictifs du risque de rechute locorégionale (RLR) serait utile, afin de personnaliser les indications de radiothérapie adjuvante. Le but de ce travail est, après une revue de la littérature, d'identifier ces facteurs à partir de l'analyse des données d'une étude de phase III multicentrique (EORTC 10994). Méthodes : les patientes ont été traitées par chimiothérapie néoadjuvante, chirurgie puis radiothérapie adjuvante. Certaines des patientes ayant un cancer HER2 (Human Epidermal growth factor Receptor 2) positif ont reçu un traitement adjuvant par trastuzumab. Une analyse multivariée a été réalisée afin d'isoler les facteurs prédictifs de RLR. Le critère de jugement principal était le temps jusqu'à la RLR (premier événement). Résultats : le suivi médian est de 4,8 ans. Parmi 1553 patientes éligibles, 76 RLR ont été observées, soit un taux de 4,9% à 5 ans (IC 95% [3,76 - 6,04]). Book excerpt: Introduction : La récidive locorégionale est associée à un risque de survenue de métastases à distance et de décès dans le cancer du sein. L'objectif de cette étude était d'évaluer l'impact du délai de récidive locorégionale sur la survie sans métastase dans une série de cancers du sein traités par chimiothérapie néoadjuvante. Méthodes : Entre 2002 et 2012, 1191 patientes de la cohorte NEOREP à l'institut Curie ont été incluses. Une analyse multivariée a été réalisée à l'aide d'un modèle statistique de type « Random Survival Forest ». Résultats : Le délai médian de suivi était de 59,8 mois. 7,1% des patientes ont présenté une récidive locorégionale. Le délai de récidive locorégionale et la récidive locorégionale étaient les facteurs les plus discriminants dans la survenue de métastase. Un délai de récidive locorégionale inférieur à 50 mois était associé à un mauvais pronostic en termes de survie sans métastase à 3 et 5 ans. L'analyse par sous-types moléculaires (Luminal, triple négatif et Her2 positif) retrouvait également un seuil pronostique à 3 ans de 50 mois pour les 3 sous-types. A 5 ans, le seuil restait de 50 mois pour les tumeurs triple négatives mais était de 75 mois pour les tumeurs luminales et HER2 positives. Conclusion : Une récidive locorégionale avant 50 mois est un facteur pronostique important dans la survenue de métastases dans le cancer du sein traité par chimiothérapie néoadjuvante. Passé ce seuil, la survie sans métastase est peu influencée par le délai de récidive.

L’objectif secondaire est l’étude du rôle et des indications de la radiothérapie après chimiothérapie néo-adjuvante et traitement chirurgical conservateur. Matériels et méthodes : Analyse rétrospective de 290 patientes prises en charge, de 1990 à 2004, à l’Institut Bergonié à Bordeaux, pour un cancer du sein non métastatique traité par une chimiothérapie première suivie d’un traitement chirurgical conservateur et d’une radiothérapie. Etude des taux de rechute locorégionale en fonction de plusieurs facteurs pronostiques potentiels. Résultats : L’analyse univariée a permis d’identifier cinq facteurs pronostiques de rechute locorégionale, statistiquement significatifs : l’âge au diagnostique inférieur à 40 ans (p = 5.69x10-5), la négativité des récepteurs hormonaux (p = 0.0001), l’envahissement ganglionnaire clinique initial (p = 0.006), l’envahissement ganglionnaire au curage axillaire, après chimiothérapie (p 0.0008), la présence de carcinome canalaire in situ dans la pièce opératoire (p = 0.0006). En analyse multivariée, trois de ces facteurs sont indépendants : la négativité des récepteurs hormonaux (RR = 4.015 IC 95% [2.057-7.837], l’âge initial inférieur à 40 ans (RR = 2.801 IC 95% [1.592-4.926]), et l’envahissement ganglionnaire au curage axillaire (RR = 2.302 IC 95% [1.248-4.246]). Conclusion. Dans notre série, la négativité des récepteurs hormonaux, l’âge inférieur à 40 ans et l’envahissement ganglionnaire au curage axillaire sont des facteurs pronostiques de rechute locorégionale après chimiothérapie première et chirurgie conservatrice. Cela soulève la question de la prise en charge thérapeutique optimale pour ces patientes à haut risque : faut-il proposer une mastectomie, ou peut-on intensifier le traitement locorégional et notamment la radiothérapie afin de limiter ce risque de rechute ?

Méthodes : Le nomogramme IGR/MDACC a été développé en population générale (i.e. incluant des tumeurs luminales, basales, HER2). Les données de 270 patientes traitées par trastuzumab au Centre de lutte Georges François Leclerc à Dijon et à l'Institut Curie à Paris ont été utilisées afin d'évaluer le nomogramme IGR/MDACC sur les tumeurs surexprimant/amplifiant HER2. Une analyse uni puis multivariée a été réalisée afin de rechercher des facteurs prédictifs indépendants au sein d'une population spécifique (tumeurs surexprimant/amplifiant HER2). Les données clinico-pathologiques associées à la pCR ont été combinées afin de créer un prédicteur basé un modèle de régression logistique. Résultats : L'évaluation du nomogramme IGR/MDACC sur notre population a montré un manque de discrimination (AUC=0,54, IC95% :0,51-0,58) et un manque de calibration (p=0,01) limitant son utilisation. Une analyse uni puis multivariée a permis de mettre en évidence des facteurs prédictifs de pCR spécifiques d'une population surexprimant/amplifiant HER2. On compare les caractéristiques morphologiques et histologiques obtenues sur la micro-biopsie initiale entre les patientes ayant une réponse histologique complète (RHC) et celles n'ayant pas une RHC. Résultats : 26 % des patientes ont eu une RHC. Une association d'anthracyclines-taxanes (A-T) entraîne plus de RHC que l'utilisation d'anthracyclines seules (p=0,03). Un Ki-67 > 20 % est prédictif de RHC. On retrouve une tendance entre un âge inférieur à 50 ans et RHC (p=0,08) mais également entre une embole vasculaire sur la biopsie et une RHC (p=0,06). Il y a également un bénéfice en terme de survie sans récidive (p=0,02) dans le groupe RHC. Conclusion : Les patientes ayant un CSTN pouvant espérer un bénéfice à une chimiothérapie néoadjuvante à base d'A-T sont les patientes jeunes sans embole et avec un Ki-67 élevé sur la biopsie.
