Malgré une stabilisation de la prévalence chez les enfants et les adolescents au cours des dernières années, des efforts restent à faire pour pouvoir inverser la courbe des tendances. L'objectif de cette étude qualitative était de comprendre chez des adolescents suivis en milieu spécialisé, quels sont les facteurs d'adhésion à la prise en charge de leur problème de poids. Matériel et méthode : nous avons réalisé une étude qualitative avec entretiens individuels auprès de neuf adolescents suivis à l'Union Méditerranéenne de Nutrition, à Marseille. Le matériel a été recueilli à partir d'un guide d'entretien explorant leur vécu personnel et leur expérience de prise en charge du problème de poids. Une analyse par théorisation ancrée a permis de faire émerger les facteurs influençant la prise en charge de leur obésité. Résultats : les adolescents vivent les conséquences de leur surcharge pondérale sur le plan physique, psychologique et social. Ils présentent des difficultés à parler de leur problème de poids. Le déclic, qui est un phénomène d'ordre psychosocial, survient plus ou moins rapidement en fonction du vécu personnel des adolescents. Il est essentiel selon eux pour envisager une prise en charge. Celle-ci est dépendante de la motivation de l'adolescent, qui n'est pas constante. La perte des premiers kilos et la peur de reprendre du poids poussent l'adolescent à continuer les efforts entrepris. Le suivi en groupe augmente l'adhésion à la prise en charge. Le cercle familial, avec au centre de celui-ci la mère, joue un rôle essentiel dans les différentes étapes traversées par les adolescents. Les freins et défauts d'adhésion à la prise en charge mis en évidence sont les difficultés de communication autour du problème de poids, le manque motivation personnelle de l'adolescent et le manque de confiance en ses capacités. L'existence d'un surpoids plutôt qu'une obésité retarde la compréhension du problème et la prise en charge.L'adolescent a des attentes précises vis-à-vis des différents intervenants de la prise en charge. Pour pouvoir lui venir en aide, il est important d'apprendre à le connaitre et de lui donner des conseils concrets et adaptés à son vécu personnel. Conclusion : l'adolescent est un individu à part entière. Il fait ses propres choix concernant la prise en charge de son problème de poids, mais il a besoin de la cohésion des personnes qui l'entourent. Parler de son problème est difficile. Ainsi, les différents professionnels de santé doivent anticiper ses attentes afin qu'il reprenne confiance en lui pour trouver la motivation nécessaire à la prise en charge.
