The influence of overtime and extended work shifts on worker health and safety, as well as on worker errors, is gaining increased attention from the scientific community, labor representatives, and industry. U.S. hours of service limits have been regulated for the transportation sector for many years. In recent years, a number of states have been considering legislation to limit mandatory overtime for health care workers. The volume of legislative activity seen nationwide indicates a heightened level of societal concern and the timeliness of the issue. This document summarizes recent scientific findings concerning the relationship between overtime and extended work shifts on worker health and safety. This report provides an integrative review of 52 recently published research reports that examine the associations between long working hours and illnesses, injuries, health behaviors, and performance. The report is restricted to a description of the findings and methods and is not intended as an exhaustive discussion of all important issues related to long working hours. Findings and methods are summarized as reported by the original authors, and the study methods are not critically evaluated for quality.

Book excerpt: Mandatory overtime has been a standard for most healthcare organizations, and was brought into the spotlight by the Institute of Medicine in the 2004 report Keeping patients safe: Transforming the work environment of nurses (Monahan, 2012). The report showed extended work hours resulted in slowed reaction times, lapses in attention, reduced motivation, compromised problem solving, and decreased energy (Monahan, 2012). The focus of this paper is to identify how extended work hours relate to patient care errors. One facilities mandatory oncall program puts nurses at a high volume of work hours. This facilities on-call shifts are identified as a potential problem. A solution to change the call program and limit work hours is examined as a way to potentially reduce patient related errors. The rational for the solution is the direct correlation that has been seen in extended work hours and fatigue. The implementation of the solution is carefully detailed with step-by-step plans. Important steps that are discussed are the approval process, the implementation of a committee, and the logistics of the implementation process. How the data will be analyzed using the difference-in-differences approach is explained. Variables that need to be considered include causes of the incidents, quality of the report, and organizational factors. Finally how the information is shared with the stakeholders, nursing community, and other facilities is described.

Mandatory overtime and extended hours are constantly being utilized to improve improper staffing. Due to this, errors and near errors are increasing, as well as adverse health concerns increasing to those that care for them. A review of literature was done that supports the theory that the amount of time one works directly affects the errors and health conditions of the nurse. Change theory will be used in order to implement the required knowledge needed to change the status quo. The dissemination plan and evaluation plan will enable monitoring of the effects of proper staffing and its direct effects on patient safety as well as the individual nurse.
