Book excerpt: Emergency department healthcare workers are among the highest at risk groups for workplace violence. Research indicates the majority of emergency department nurses do not feel safe in their workplace. A quantitative cross sectional study using a self-administered survey tool was conducted to examine if emergency department nurses' perceptions of safety are affected by the physical environment of the emergency department. The study was conducted over a period of six weeks among a sampling of emergency department nurses from seven hospitals within a Midwestern regional area. Although there are various reasons for workplace violence, this study focused on emergency department registered nurses' perceptions of safety from workplace violence as they relate to the physical environment of the emergency department. Descriptive statistics revealed emergency department physical environmental safety factors vary from few to many among different hospitals. Descriptive statistics also showed most emergency department nurses feel the emergency department physical environment affects their safety; however, there was not enough evidence to support a statistical significant relationship between the respondent's answers to the physical environment questions and their answers to the nurses' perceptions of safety questions. The study found the type and amount of physical environmental safety factors had little effect on emergency department nurses' perceptions of safety.

Book excerpt: ABSTRACT Nursing physical fatigue is a critical issue that may lead to degradation of care delivery and ultimately result in medical errors. This issue is equally relevant due to the looming shortage of nurses, which has been linked to the physical demands and potential occupational hazards intrinsic to the profession; as well as to the graying of the nursing workforce who experiences gradual loss of strength and agility that accompanies aging as time in the career advances. In a hospital Emergency Department, the level of nursing physical fatigue can potentially reach its threshold in light of challenging workloads, scope of job assignments and demanding schedules. While in other safety-sensitive industries such as aviation and nuclear plants, similar concerns have been the object of systematic research and addressed with consequent regulations, just recently, the healthcare sector has been engaged in further investigations. This study proposed to explore the linkage between Emergency Department design-layout and nursing physical fatigue. It was expected that further understanding on this relationship would support evidence-based design propositions linking nursing wellness, job satisfaction, and performance to a higher quality of care and improved patient safety levels. To this end, data collection was performed during four weeks in a community-based hospital. A convenience sample of twenty-four eligible nurses was invited to participate in this two-part study. The first section consisted of the completion of a self-administered questionnaire, which assessed nurses' perception of the impact of working conditions on nursing physical fatigue. The second section included the monitoring, through the use of accelerometers, of nurses' actual activity intensity levels during three consecutive shifts. Among other findings, data demonstrated that nurses perceive several attributes or components of the built environment as potential contributors to physical fatigue. In addition, various operational practices and organizational protocols were linked to physical fatigue. Contrary to nurses' perception of physical fatigue, their actual physical activity levels fell mostly between sedentary or light intensity ranges. This paradox offers the opportunity for design interventions that, in alignment with operational practices and organizational protocols will enhance nurses' performance and improve nurses' retention.

The purpose of this Doctor of Nursing Project was two-fold. First to assess the emergency department nurse’s knowledge and perception of the role of athletic trainers for sports-related injuries through an on-line survey of more than 300 emergency nurses which demonstrated that knowledge deficits exist. The second purpose championed athletic trainers as valued members of the interprofessional healthcare team for enhanced sports injury care and seamless transition of care for both admission and discharge from the emergency department.

Book excerpt: The aim of this study was to determine the perceived competence of Emergency Department (ED) registered nurses in disaster preparedness and response. A determination of the current level of nurses' disaster preparedness competence is critical in the development of educational curriculum. This study used a quantitative, cross-sectional design to explore current perceptions of ED nurses regarding disaster preparedness. The Emergency Preparedness Information Questionnaire was used which allows participants to rate their perceived competence. Results showed that nurses felt most familiar with triage and basic first aid (M=3.9, SD=0.916) and least familiar with detection (M=2.7, SD=1.08). No subcategory received a median score of 5, nor a mean greater than 3.9 indicating low to neutral emergency preparedness familiarity. Findings from this study suggest that although nurses feel competent in some dimensions of emergency preparedness, further curriculum development is necessary to facilitate a comprehensive understanding of disaster response.

Book excerpt: Emergency departments (ED) are the entryway into the healthcare system during a disaster event and patients often arrive in the first thirty minutes after an event has occurred. This creates unique challenges for ED nurses. The purpose of the Quality Improvement (QI) project was to measure the impact of a Department Emergency Operations Plan (DEOP) on the ED nurse’s perception of disaster preparedness. This study included a convenience sample of 126 full time and part time registered nurses working in the ED at a Level 1 Trauma hospital. Institutional Review Board (IRB) approval was obtained by both Arkansas Tech University and the participating hospital. Informed consent was obtained prior to taking the pre- and post-education questionnaires. The questionnaires were created by the primary investigator with expert validation by the Emergency Nurses Association’s (ENA) 2018 Emergency Management and Preparedness Committee (EMPC). Measures included knowledge level questions based on the identified priority needs of the leadership team. Education of the DEOP was provided over thirty minutes during the January, 2019 staff meetings. Data was collected via REDCap for analysis. Thirty participants completed both questionnaires. Results indicate that the DEOP QI project significantly increased the perception of the ED’s nurse’s knowledge level regarding disaster response for each of the questions utilized from the pre-education questionnaire as compared to the post-education questionnaire. Additionally, 85% of participants noted that the education increased their comfort level with the DEOP.
