Book excerpt: Background: Spinal cord injury (SCI) can be a devastating event causing a significant burden to individuals, their family members and society. In the United States (US), it is estimated 276,000 individuals are living with a SCI and it is projected that 12,500 new SCI cases are reported each year. Individuals with a physical impairment such as SCI often develop a pressure ulcer (PU), which may be prevented. There is a correlation to the development of PU's from poor nutritional status and poor nutritional intake. The purpose of this study was to examine the relationships between macronutrient and micronutrient intake, anthropometric measurements, psychosocial, disability, and nutritional status measurements among community dwelling non-ambulatory persons with a (SCI) with and without (PU's). Methods: Participants were SCI individuals living in the community with and without a PU and were recruited from home health agencies and SCI rehabilitation facilities. Data was collected using structured questionnaires, nutritional assessment tools, skin assessment, and anthropometric measurement over 4 days consisting of one initial visit and a second visit on day four. Macro and micronutrient intake data was collected using a 3-day self-reported food log. Psychosocial factors were measured via structured questionnaire administered during the initial home visit. Nutritional status measurements included the Mini Nutritional Assessment-Short Form.Disability status measurements included World Health Organization Assessment Schedule (WHODAS) 2.0, and Spinal Cord Injury-Secondary Condition Scale (SCI-SCS). The sample included 80 participants, 40 with and 40 without PUs. Results: This study found significant and clinical differences in micro and macronutrient intake among participants with and without PU indicating persons with SCI are at some nutritional risk. Conclusions: Participants consumed inadequate dietary intake in both quantity and quality. Participants with PU are not getting enough protein. Calories are within the recommended daily allowances; however, those calories are coming from cholesterol, carbohydrates, saturated fat, and can lead to decrease in wound healing and increase their risk for secondary condition such as cardiovascular disease.

Book excerpt: Background: The primary aim of this study was to display descriptive data about HEI scores, average dietary intake and food insecurity in a sample of individuals with spinal cord injury. Secondary aims were to investigate whether the malnutrition criteria proposed by the Academy of Nutrition and Dietetics (AND) and the American Society of Enteral and Parenteral Nutrition (ASPEN) could be used to assess the spinal cord injury population. Methods: Twenty-three spinal cord injury subjects completed a Vioscreen food frequency questionnaire where nutrient data and Healthy Eating Index-2010 (HEI) scores were calculated. Anthropometric data and pressure injury history were collected during the study visit. Food security status was assessed with a validated two-item screening tool. Malnutrition status was assessed using the proposed 2012 AND/ASPEN malnutrition criteria. Results: Mean HEI-2010 score of the sample population was 65 out of a total 100 points. This is 112% of the 2011-12 NHANES mean of 58.27 points. Nine subjects consumed less than 75% of RDA for vitamin C. Four subjects consumed less than 75% of RDA for zinc. Nine subjects consumed less than 75% of calculated protein needs. One subject consumed less than 75% of RDA for vitamin A. No subjects consumed less than 75% of RDA for selenium. Three subjects were screened at risk for food insecurity. Two subjects met the 2012 AND/ASPEN criteria for malnutrition.

Book excerpt: Pressure ulcers are defined by the National Pressure Ulcer Advisory Panel (NPUAP) as “localized injury to the skin and/or underlying tissue usually over a bony prominence, as a result of pressure, or pressure in combination with shear and/or friction.” A number of risk factors are associated with increased risk of pressure ulcer development, including older age, black race, lower body weight, physical or cognitive impairment, poor nutritional status, incontinence, and specific medical comorbidities that affect circulation such as diabetes or peripheral vascular disease. Pressure ulcers are often associated with pain and can contribute to decreased function or lead to complications such as infection. In some cases, pressure ulcers may be difficult to successfully treat despite surgical and other invasive treatments. In the inpatient setting, pressure ulcers are associated with increased length of hospitalization and delayed return to function. In addition, the presence of pressure ulcers is associated with poorer general prognosis and may contribute to mortality risk. Recommended prevention strategies for pressure ulcers generally involve use of risk assessment tools to identify people at higher risk for developing ulcers in conjunction with interventions for preventing ulcers. A variety of diverse interventions are available for the prevention of pressure ulcers. Categories of preventive interventions include support surfaces (including mattresses, integrated bed systems, overlays, and cushions), repositioning, skin care (including lotions, dressings, and management of incontinence), and nutritional support. Each of these broad categories encompasses a variety of interventions. The purpose of this report is to review the comparative clinical utility and diagnostic accuracy of risk-assessment instruments for evaluating risk of pressure ulcers and to evaluate the benefits and harms of preventive interventions for pressure ulcers in different settings and patient populations. The following Key Questions are the focus of this report: KQ1. For adults in various settings, is the use of any risk-assessment tool effective in reducing the incidence or severity of pressure ulcers compared with other risk-assessment tools, clinical judgment alone, and/or usual care? KQ1a. Do the effectiveness and comparative effectiveness of risk-assessment tools differ according to setting? KeQ1b. Do the effectiveness and comparative effectiveness of risk-assessment tools differ according to patient characteristics and other known risk factors for pressure ulcers, such as nutritional status or incontinence? KQ2. How do various risk-assessment tools compare with one another in their ability to predict the incidence of pressure ulcers? KQ2a. Does the predictive validity of various risk-assessment tools differ according to setting? KQ2b. Does the predictive validity of various risk-assessment tools differ according to patient characteristics? KQ3. In patients at increased risk of developing pressure ulcers, what are the effectiveness and comparative effectiveness of preventive interventions in reducing the incidence or severity of pressure ulcers?
