Book excerpt: The purpose of this study is to examine the effects of exercise training on tumor response to anthracycline-based chemotherapy. Eighty athymic Fischer 344 mice will be purchased at 21 days and at 26 days of age, MDA-MB-231 carcinoma cells (3x10EXP 6 cells prepared from donor tumors) will be subcutaneously implanted into the right flank of all animals. At 40 days of age, animals will be randomly assigned to one of four groups: (i) exercise alone (n=20), (ii) doxorubicin alone (n=20), (iii) exercise plus doxorubicin (n=20), and (iv) control (n=20). Animals assigned to doxorubicin alone and exercise plus doxorubicin will receive intravenous injections of doxorubicin at 5mg/kg every 7 days. The exercise group will be progressively trained to run at 22m/min at 0% grade for 45 minutes 5 days/week for 8 weeks. Forty eight hours after the final exercise session, all experimental animals will be sacrificed via carbon dioxide anesthetization and cervical dislocation. The primary tumor will be surgically removed, weighed and histologically processed. The primary outcome will be tumor volume measured in two dimensions. Secondary outcomes will be tumor growth delay calculated as the number of days for each individual animal tumor to reach 1000 cubic mm compared with the control group.

Book excerpt: Exercise is becoming readily accepted as a beneficial adjunct therapy to maintain or enhance quality of life in breast cancer patients during adjuvant chemotherapy. An essential precursor to these studies is to investigate whether exercise modulates the antitumor efficacy of chemotherapeutic agents. Athymic female mice were transplanted with MDA-MB-231 breast xenografts and randomly assigned to one of four groups (n= 21 per group): (i) control, (ii) exercise only, (iii) doxorubicin only, or (iv) exercise + doxorubicin. Exercise groups performed progressive treadmill running up to 18m/min at 0% grade for 45mins, 5 days/wk for 8 weeks. Tumor growth delay was significantly longer in the doxorubicin only and exercise + doxorubicin groups compared with the control (median 42 vs. 25 days, p=0.0082; 36 vs. 25 days, p=0.029, respectively) and exercise only groups (median 42 vs. 25 days, p=0.029; 36 vs. 25 days, p=0.080, respectively). There was no significant difference between doxorubicin only and exercise + doxorubicin groups (median 42 vs. 36 days, p=0.33) suggesting that moderate intensity exercise does not significantly influence doxorubicin-induced tumor growth delay.

Method: Using a two-armed, prospective, randomized design, potential participants will be identified and screened for eligibility via medical record review of patients scheduled for their primary neoadjuvant chemotherapy treatment consultation at DUMC. Following the successful completion of all baseline assessments participants will be randomly assigned to an exercise or control group. Participants assigned to combined exercise training and chemotherapy will perform an individualized exercise training program consisting of three cycle ergometry sessions per week at approximately 60-80% of VO2peak on nonconsecutive days for the duration of neoadjuvant chemotherapy (approximately 12 weeks). Most Significant Research Finding During Reporting Period: As per our statement of work, human ethnical approval was obtained (Task 1); all exercise testing / training procedures and clinical protocols were clarified (Task 2), and data collection has been initiated (Task 3). To date, all 3 patients in phase I of the study have been enrolled and no doselimiting toxicities have been identified with acceptable exercise adherence (>60%), thus phase II has been initiated.

SSI provides income assistance for disabled, blind, and aged people who have limited income and resources regardless of their prior participation in the labor force. Both programs share a common disability determination process administered by SSA and state agencies as well as a common definition of disability for adults: "the inability to engage in any substantial gainful activity by reason of any medically determinable physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than 12 months." Disabled workers might receive either SSDI benefits or SSI payments, or both, depending on their recent work history and current income and assets. Disabled workers might also receive benefits from other public programs such as workers' compensation, which insures against work-related illness or injuries occurring on the job, but those other programs have their own definitions and eligibility criteria. Book excerpt: In an era of promising advances in cancer research, there are considerable and even alarming gaps in the fundamental knowledge and understanding of ovarian cancer. Researchers now know that ovarian cancer is not a single disease-several distinct subtypes exist with different origins, risk factors, genetic mutations, biological behaviors, and prognoses. However, persistent questions have impeded progress toward improving the prevention, early detection, treatment, and management of ovarian cancers. Failure to significantly improve morbidity and mortality during the past several decades is likely due to several factors, including the lack of research being performed by specific disease subtype, lack of definitive knowledge of the cell of origin and disease progression, and incomplete understanding of genetic and non-genetic risk factors. Ovarian Cancers examines the state of the science in ovarian cancer research, identifies key gaps in the evidence base and the challenges to addressing those gaps, considers opportunities for advancing ovarian cancer research, and examines avenues for translation and dissemination of new findings and communication of new information to patients and others. This study makes recommendations for public- and private-sector efforts that could facilitate progress in reducing the incidence of morbidity and mortality from ovarian cancers.
