Book excerpt: Background and PurposeTissue perfusion during Basic Life Support (BLS) resuscitation depends on hemodynamic support provided by chest compressions. However, it remains unclear how the ischaemia/reperfusion phenomena influences hemodynamic stability during prolonged BLS. We aimed to study the arterial pressures and brain tissue perfusion in a porcine model of cardiac arrest (CA) and BLS.MethodsSix female pigs (30u00b1 Kg) underwent an 8-minute non-treated ventricular fibrillation followed by BLS resuscitation for 15 (n=3) and 30 minutes (n=3). Chest compressions were provided with a LUCAS machine and respirations (FiO2 0.21) with bag valve mask at 30:2 ratio, respectively. Invasive systolic, mean and diastolic arterial pressures (SAP, MAP and DAP) and brain SrO2 were recorded every minute from baseline until the end of the experiment. ResultsBriefly, arterial pressures rapidly dropped during 8-minutes untreated cardiac arrest and rapidly increased when BLS manoeuvres were started by LUCAS. SAP reached its peak at 3 minute and then was observed to gradually drop over-time. MAP and DAP were maintained low during BLS with also tendency to drop. Brain SrO2 during BLS did not correlate with the evolution of SAP, not showing any improvement once BLS manoeuvres began.ConclusionsIn a large-animal model of CA, arterial pressures drop over-time during the BLS despite mechanical chest compressions. Brain tissue perfusion does not follow arterial pressure pattern during BLS.

Book excerpt: Background: Myocardial ischemic contracture compromises the hemodynamic effectiveness of cardiopulmonary resuscitation (CPR). Its most severe form, referred to as stone heart, is considered to be beyond the limit of resuscitation. However, accurate assessment of the ischemic contracture severity is challenging during CPR. We investigated, in a swine model, whether hemodynamic parameters, including end-tidal carbon dioxide (ETCO2), are predictive of the ischemic contracture severity during consistent quality CPR.Methods: We retrospectively analyzed 96 datasets for ETCO2, arterial pressure, left ventricular (LV) wall thickness, and the percent of measured end-diastolic volume (%EDV) relative to EDV at the onset of ventricular fibrillation from eight pigs. Animals underwent advanced cardiovascular life support based on resuscitation guidelines.Results: During CPR, LV wall thickness progressively increased while %EDV progressively decreased. Systolic and diastolic arterial pressure and ETCO2 were significantly correlated with LV wall thickness and %EDV. Linear mixed effect models revealed that, after adjustment for baseline covariates, systolic and diastolic arterial pressure were not predictive factors for LV wall thickness or %EDV. ETCO2 was a significant predictive factor for LV wall thickness (P = 0.014) and %EDV (P = 0.013). However, it could explain only 6.1% and 14.2% of the total variance of LV wall thickness and %EDV in our models, respectively.Conclusions: ETCO2 was a significant predictive factor for the severity of ischemic contracture. However, it explained only a small proportion of the variance of ischemic contracture severity.
