Book excerpt: he starting point for this guideline is the point at which a woman has learnt that she is living with HIV and it therefore covers key issues for providing comprehensive sexual and reproductive health and rights-related services and support for women living with HIV. As women living with HIV face unique challenges and human rights violations related to their sexuality and reproduction within their families and communities as well as from the health-care institutions where they seek care particular emphasis is placed on the creation of an enabling environment to support more effective health interventions and better health outcomes. This guideline is meant to help countries to more effectively and efficiently plan develop and monitor programmes and services that promote gender equality and human rights and hence are more acceptable and appropriate for women living with HIV taking into account the national and local epidemiological context. It discusses implementation issues that health interventions and service delivery must address to achieve gender equality and support human rights.

The book also reviews nonexperimental techniques, including a critical examination of evaluation methods that are observational rather than experimentalâ€"a necessity when randomized experiments are infeasible.

Book excerpt: The ‘migration-development’ nexus has emerged as an important area of both research and policy over the last ten years. However, most of the interest has focused on the potential that migration holds for poverty alleviation. Relatively little attention has been paid to the relationship between migration and inequality, particularly on inequality as a consequence of migration. This is unfortunate, given that inequality is emerging as an important area of inquiry within development studies. This edited collection explores the relationship between migration and inequality in Africa, Asia and Latin America by taking into account economic and social inequalities. While the focus on inequality as opposed to poverty is in itself original, the book offers additional points of interest. First, it combines chapters on internal and international migration, thereby challenging the current focus in the migration literature that focuses almost exclusively on cross-border migration. Internal migration greatly outnumbers cross-border moves. Yet policy-makers as well as most studies focus on cross-border international migration. We are only just beginning to unravel the relationship between internal and cross-border migration. Second, the theme of inequality complements the existing focus in the migration-development nexus on issues of poverty. Third, the chapters focus on both economic and social inequalities, often combining an analysis of different types of inequalities. The book also covers governance and migrants’ rights; gender and intersectionality; and health. The chapters in this edited volume make an original contribution to debates on the migration-development nexus as well as the literature on inequality, which often tends to focus on economic measurements of inequality at the expense of including a thorough analysis of social inequality.

Book excerpt: Within the continuum of reproductive health care, antenatal care provides a platform for important health-care functions, including health promotion, screening and diagnosis, and disease prevention. It has been established that, by implementing timely and appropriate evidence-based practices, antenatal care can save lives. Endorsed by the United Nations Secretary-General, this is a comprehensive WHO guideline on routine antenatal care for pregnant women and adolescent girls. It aims to complement existing WHO guidelines on the management of specific pregnancy-related complications. The guidance captures the complex nature of the antenatal care issues surrounding healthcare practices and delivery, and prioritizes person-centered health and well-being --- not only the prevention of death and morbidity --- in accordance with a human rights-based approach.

Available in PDF, EPUB and Kindle. Book excerpt: When working with countries to measure and compare health systems functioning, it is important to strike a good balance between avoiding blueprints that do not allow for country contexts and specificities while also encouraging a degree of standardization that enables comparisons within and between countries as well as over time. Standardized indicators allow comparisons between countries and can help mutual learning, including the identification of bottlenecks and the sharing of lessons learned. This handbook does not attempt to cover all components of the health system or deal with the various monitoring and evaluation frameworks. Instead, it is structured around the WHO framework that describes health systems in terms of six core components or "building blocks": service delivery, health workforce, health information systems, medical products, vaccines and technologies, financing and leadership/governance. The selection of indicators was guided by the need to detect change and show progress in health systems strengthening. Indicators relate to both the level and distribution of inputs and outputs. While the focus is on low- and middle-income countries, experiences from high-income countries are also used to guide the development of measurement systems. Each section has proposed core indicators that all countries are encouraged to collect, plus a wider set of indicators that users can choose or modify as needed. It is anticipated that the core indicators will enable the production of country "dashboards" that contain the instruments by which health systems trends can be regularly monitored and compared. Countries should integrate new indicators with existing indicators of their health sector and statistical strategies and plans. Health systems monitoring should also be seen in the context of the indicators' impact on access to priority health services and their contribution to reaching the Millennium Development Goals (MDGs). The handbook is divided into six sections, each of which covers one health system component or building block and is set out along the following lines: -introduction to the component and related indicators; -description of possible sources of information and available measurement strategies; -proposed "core indicators", supplemented, where necessary, by additional indicators that may be used depending on the country health system attributes and needs.

Book excerpt: The U.S. government supports programs to combat global HIV/AIDS through an initiative that is known as the President's Emergency Plan for AIDS Relief (PEPFAR). This initiative was originally authorized in the U.S. Leadership Against HIV/AIDS, Tuberculosis, and Malaria Act of 2003 and focused on an emergency response to the HIV/AIDS pandemic to deliver lifesaving care and treatment in low- and middle-income countries (LMICs) with the highest burdens of disease. It was subsequently reauthorized in the Tom Lantos and Henry J. Hyde U.S. Global Leadership Against HIV/AIDS, Tuberculosis, and Malaria Reauthorization Act of 2008 (the Lantos-Hyde Act). Evaluation of PEPFAR makes recommendations for improving the U.S. government's bilateral programs as part of the U.S. response to global HIV/AIDS. The overall aim of this evaluation is a forward-looking approach to track and anticipate the evolution of the U.S. response to global HIV to be positioned to inform the ability of the U.S. government to address key issues under consideration at the time of the report release.

Book excerpt: In 2003, Congress passed the United States Leadership Against HIV/AIDS, Tuberculosis, and Malaria Act, which established a 5-year, $15 billion initiative to help countries around the world respond to their AIDS epidemics. The initiative is generally referred to by the title of the 5-year strategy required by the act-PEPFAR, or the President's Emergency Plan for AIDS Relief. PEPFAR Implementation evaluates this initiative's progress and concludes that although PEPFAR has made a promising start, U.S. leadership is still needed in the effort to respond to the HIV/AIDS pandemic. The book recommends that the program transition from its focus on emergency relief to an emphasis on the long-term strategic planning and capacity building necessary for a sustainable response. PEPFAR Implementation will be of interest to policy makers, health care professionals, special interest groups, and others interested in global AIDS relief.

This book addresses the quantitative evaluation of HIV prevention programs, assessing for the first time several different quantitative methods of evaluation. The authors of the book include behavioral scientists, biologists, economists, epidemiologists, health service researchers, operations researchers, policy makers, and statisticians. Book excerpt: Health interventions that are effective in developed countries may not be as effective in developing countries given the differing social, economic, cultural, and infrastructure factors that may affect a program's implementation and outcomes. This monograph is intended to promote an understanding of why program evaluation is a critical component of any health intervention and to stimulate discussion on ways to make evaluation of health interventions in developing countries more rigorous.
