Book excerpt: Le parcours patient hôpital-ville après un passage au service d'accueil des urgences est un parcours simple et souvent bref. Il peut néanmoins être le début d'une prise en charge plus longue et complexe, où le médecin traitant doit pouvoir reprendre le fil du suivi de manière efficace et coordonnée. Il est intéressant d'en comprendre l'utilisation par les usagers pour améliorer l'efficience des soins coordonnés. L'objectif principal de cette étude est d'analyser le taux de consultations chez le médecin traitant lorsqu'une réévaluation est indiquée par l'urgentiste. Les objectifs secondaires sont de chercher les facteurs influençant cette réévaluation et d'en déduire des pistes possibles d'amélioration. Nous avons mis en place une étude épidémiologique observationnelle descriptive prospective et monocentrique du 21/02/2022 au 01/04/2022 au service d'accueil des urgences de La Ciotat (13). Nous avons contacté ces patients 15 jours après leur sortie pour un bref questionnaire téléphonique. Le critère de jugement principal était le « devenir-patient » (consultation réalisée par le médecin traitant dans les 15 jours). Sur les 404 patients rappelés, 178 ont pu être joints et inclus dans l'analyse. Le taux de consultation brut à 15 jours est de 65%, celui chez le médecin traitant de 47% (soit 72% des consultants). La moitié des patients qui ne re consultent pas disent « aller mieux ». Une part non négligeable de patients attend encore un rendez-vous 15 jours après la sortie des urgences (imagerie ou médecin). Les facteurs positifs significatifs de réévaluation sont : l'âge et l'impression d'un compte rendu de passage aux urgences (+20% de réévaluation). Les patients respectent globalement les consignes de surveillance données lors de la sortie mais n'appréhendent pas complètement le fonctionnement du parcours patient coordonné. La valeur ajoutée du compte rendu de sortie du patient est probablement sous-estimée et son impression devrait être plus systématique. L'optimisation informatique doit permettre une amélioration de la production et de la transmission d'information. Les réseaux de santé locaux doivent améliorer la connaissance du parcours de leurs patients et des patients de leur parcours.

Book excerpt: INTRODUCTION : En France, le nombre de consultations aux urgences s'accroit inexorablement alors que le nombre de lits d'hospitalisation ne cesse de diminuer. C'est ce qui rend nécessaire une optimisation de la gestion du flux des patients aux urgences. La proposition de consultations post-urgences est l'une des pistes possibles. Notre objectif principal était d'évaluer le taux de nouvelles consultations aux urgences et la prévalence de nouvelles hospitalisations chez des patients ayant eu une consultation post-urgences. METHODE: Une étude analytique, rétrospective et monocentrique a été réalisée dans le service d'accueil des urgences de l'hôpital de Versailles de novembre 2020 à avril 2021. Tous les patients de plus de 18 ans ayant bénéficié d'une consultation post-urgences suite à une première consultation aux urgences pour un motif médical et relevant d'une prise en charge ambulatoire a été inclus dans l'étude. Les données ont été recueillies via les logiciels URQUAL et ALIZEE. L'analyse a porté sur des données épidémiologiques et cliniques ; le diagnostic médical aux urgences puis en consultations post-urgences ; la présence d'une adaptation des thérapeutiques ; le devenir du patient ; le délai de prise de rendez-vous pour la consultation post-urgences. RESULTATS: Sur 54 dossiers de patients initialement inclus, 50 ont pu être analysés. L'âge moyen était de 40,63 ans. Après la consultation post-urgences, 92 % des patients sont rentrés au domicile, 8 % ont été hospitalisés et 2,9 % des patients ont à nouveau consulté aux urgences pour le même motif. Les principales pathologies prises en charge en consultation post-urgences étaient : urinaires, sexuelles, cutanées, rhumatologiques (polyarthralgies) ou en lien avec des maladies systémiques non étiquetées. En moyenne, 4,9 jours étaient nécessaires pour la prise d'un rendez-vous. 15% des médecins du service ont répondu à un questionnaire permettant d'analyser les 6 premiers mois de fonctionnement du circuit-court de consultations post-urgences. 100% en était satisfaisait, même si des axes d'amélioration ont été individualisés. DISCUSSION : La mise en place d'un circuit-court de consultation post-urgences réduit les réadmissions aux urgences pour des motifs identiques ainsi que les hospitalisations secondaires. Au vu d'une politique réductrice des coûts et des lits d'hospitalisation, ces résultats ne peuvent que nous conforter dans le développement de ces consultations post-urgences dans nos hôpitaux. Le rôle de ces consultations post-urgences pourraient être prépondérant lorsque celles-ci sont organisées au niveau des zones sous-dotées en médecins de notre territoire.
