Book excerpt: Contexte : Les accidents vasculaires cérébraux (AVC) sont des pathologies fréquentes et graves. Leur prévalence est en constante augmentation en particulier chez le sujet âgé. Les situations de stress pro-inflammatoire, pro-oxydant sont des situations fréquentes dans la population âgée et présentes dans les maladies cardio-vasculaires. L'hospitalisation en soi, peut être définie comme une situation de stress aiguë. L'objectif de ce travail est de mesurer l'importance du risque d'AVC lié à l'hospitalisation et sa durée dans le temps. Méthode : Il s'agit d'une étude observationnelle rétrospective, cas-croisés dont les données ont été extraites de la base de données nationale française du programme de médicalisation des systèmes d'information (PMSI) de janvier 2007 à Décembre 2013.Tous les AVC ischémiques ont été inclus. L'exposition était l'hospitalisation. La période d'exposition a été divisée en plusieurs intervalles de 30 jours dans la période cas et la période contrôle. L'outil « IT-CARES » a été utilisé. La régression logistique conditionnelle a été utilisée pour évaluer l'association entre l'hospitalisation et le sur risque d'AVC. Résultats : Pour toutes les classes d'âge, le risque d'AVC était très élevé les 30 jours suivants l'hospitalisation : OR 3.82 IC95% [3.75 3.9]. Ce risque décroissait au fil du temps. Chez le sujet âgé, les tendances étaient similaires avec un risque élevé les 30 jours suivants l'hospitalisation : OR 3.17 IC95% [3.1 3.26]. Cependant ce sur risque lié à l'hospitalisation perdure plus longtemps dans le temps jusqu'à 150jours en population gériatrique. Conclusion : Le risque d'AVC ischémique est élevé après une hospitalisation notamment les 30 premiers jours suivants l'hospitalisation. Il décroit au fil du temps. Il a été montré dans notre étude que ce sur risque persiste de façon prolongée chez le sujet âgé.
