Book excerpt: Dans le cadre de l'évaluation du fonctionnement du Réseau Douleur de Franche-Comté, une évaluation des pratiques professionnelles a été réalisée entre septembre 2005 et avril 2006., Nous avons choisi d'évaluer l'impact des prises en charge sur l'intensité de la douleur et le niveau de la qualité de vie des patients suivis au sein du Réseau et présentant une douleur chronique en dehors de la maladie cancéreuse évolutive. Les objectifs sont de permettre une amélioration des pratiques et de la qualité de la prise en charge par des mesures correctrices. - À M0 et à M6 de la prise en charge, nous avons demandé à chaque nouveau patient de remplir un auto questionnaire comprenant 5 échelles : une échelle visuelle analogique (0-10) et une échelle numérique (0-10) de la douleur, une échelle de retentissement comportemental, l'Hospital anxiety and depression scale, et une échelle visuelle analogique de qualité de vie. Au total 166 patients ont été inclus. 100 ont répondu au questionnaire à 6 mois. La moyenne d'âge était de 50 ans. La durée d'évolution de la pathologie à l'origine des douleurs était en moyenne de 7 ans. - Résultats : À M0, 15% des patients connaissaient des difficultés sociales (aucun n'a été suivi par une assistante sociale), 84 % présentaient un score d'anxiété douteux ou certain, et 67% un score de dépression douteux ou certain (24% ont bénéficié d'une prise en charge psychothérapeutique). À M6, 38% des patients avaient une amélioration de l'intensité des douleurs d'au moins un point, plus fréquente en cas de céphalées et de douleurs neuropathiques ainsi qu'en l'absence de difficultés sociales. L'amélioration d'au moins 50% de la qualité de vie (36% des patients), l'amélioration de la marche (43% des patients) et du travail habituel à la maison (55% des patients) étaient plus fréquentes en cas de prise en charge en psychothérapie et en rééducation fonctionnelle. L'amélioration du temps de loisir, de la libido et du score d'anxiété et de dépression était plus fréquente en l'absence de difficultés sociales. - Conclusion : Nous avons montré que les prises en charge, notamment en rééducation fonctionnelle et en psychothérapie, sont nécessaires et insuffisamment mises en œuvre. Ce déficit est lié au nombre trop restreint de psychologues au sein du réseau, et à l'insuffisance du nombre de centres de rééducation capables de recevoir les patients douloureux chroniques. Nous avons vu, en outre, que l'absence de problèmes sociaux est un facteur majeur dans l'amélioration globale des patients. Ceci confirme l'importance du décloisonnement entre les domaines sanitaire et social.

The critique of ‘essentialism' (i.e. there is no specifically feminine essence) proposed by both gender theory and deconstruction is just one more twist in the ontological negation of the feminine. Contrary to all expectations, however, this ever more radical hollowing out of woman within intellectual movements supposed to protect her, this assimilation of woman to a ‘being nothing', clears the way for a new beginning. Let us now assume the thought of ‘woman' as an empty but resistant essence, an essence that is resistant precisely because it is empty, a resistance that strikes down the impossibility of its own disappearance once and for all. To ask what remains of woman after the sacrifice of her being is to signal a new era in the feminist struggle, changing the terms of the battle to go beyond both essentialism and anti-essentialism.
