Book excerpt: Introduction : les difficultés de communication entre médecins généralistes (MG) et médecins hospitaliers (MH) ont souvent des répercussions négatives sur la prise en charge des patients. Matériels et méthodes : une étude descriptive observationnelle transversale a été réalisée en Île de France du 28/03 au 31/05/2018. Son objectif était d'établir un état des lieux de la communication via messagerie sécurisée (MS) entre MH et MG, et de décrire les freins ressentis et les souhaits d'amélioration des MG les concernant. L'objectif secondaire était d'évaluer si le fait d'être équipé d'une MS avait un impact sur les usages. Résultats : les réponses de 141 médecins représentatifs des MG d'Ile de France ont été analysées. 85% étaient équipés d'une MS, majoritairement par Apicrypt (24,1%) et MS Santé (24%) ou les deux (24,8%). Ils l'utilisaient principalement pour la réception d'examens complémentaires (EC) (74,4%) et les échanges avec les MH (71,1%). Les MG utilisent leur MS sur une base quasi quotidienne (72,7%). Les principaux freins à l'utilisation sont le manque de formation (36,4%) et le manque d'interlocuteurs (27,3%). Les MG souhaitaient des outils sécurisés, simples d'utilisation et réactifs qu'ils utiliseraient pour la réception des comptes rendus hospitaliers (CRH) (30,5%), la réception des EC (24,1%) et l'échange avec les MH (21,3%). Ainsi la coordination des soins entre les acteurs de soins primaires, que sont les médecins généralistes, et le CHOV se doit d'être la meilleure possible dans l'intérêt du patient. La communication ville-hôpital est un élément primordial de cette coordination. Un questionnaire, afin d'évaluer qualitativement le ressenti des médecins généralistes vis-à-vis de la communication avec l'hôpital, a été soumis à 59 praticiens exerçant dans le territoire sanitaire du CHOV. 24 d'entre eux ont répondu à ce questionnaire (41 %). Il apparait que près de la moitié des médecins ne se sentent et ne sont pas suffisamment informés de l'offre de soins proposée par le CHOV. Il ressort également une grande difficulté à effectuer la réalisation d'hospitalisation directe dans les différents services de médecine pour la majorité des médecins interrogés. Malgré un temps d'attente jugé satisfaisant par 15 médecins (62,5 %) avant d'être mis en relation avec un praticien hospitalier via le standard téléphonique des établissements, la quasi-totalité des répondeurs (22 ; 92 %) seraient favorables à la mise en place d'un numéro direct. Une autre grande attente des médecins généralistes est la réception des différents comptes-rendus médicaux par messagerie électronique sécurisée que la quasi-totalité des praticiens utilisent (21 ; 87 %). Ainsi 3 grands axes d'amélioration ont pu être mis en évidence : faciliter la communication avec les praticiens hospitaliers, la réalisation d'hospitalisation directe et instaurer l'envoi électronique des différents comptes-rendus.
