Book excerpt: Objectif : évaluer et comparer les méthodes de mesure de la densité du thrombus en Unités Hounsfield (UH) au scanner cérébral non injecté à la phase aigüe d’un accident vasculaire cérébral ischémique traité par fibrinolyse intraveineuse (IV). Matériel et méthodes : l’étude rétrospective a porté sur 100 patients traités par fibrinolyse IV entre janvier 2009 et décembre 2013 avec un thrombus visible sur le scanner initial. 3 méthodes de mesure de région d’intérêt (ROI) étaient testées: ROI circulaire de la plus petite taille mesurable, ROI ovale épousant les contours du thrombus, ROI circulaire du diamètre de l’artère occluse. La mesure était réalisée 3 fois par chaque observateur et rapportée au côté sain. Les résultats étaient comparés selon la méthode de Bland et Altman et des limites d’agrément acceptables définies pour la concordance entre observateurs. Nous avons recherché une association entre la densité du thrombus et la recanalisation après fibrinolyse, le score de Rankin à 3 mois et la cause de l’AVC. Résultats : la concordance entre observateurs a montré des limites d’agrément allant de -12 UH à +10 UH pour la mesure du thrombus et de -0,3 à +0,3 pour le ratio de densité rapporté au côté sain. Ces résultats étaient au-delà des bornes fixées. Il n’a pas été observé de liaison significative entre la densité du thrombus ou son ratio, et le score de Rankin à 3 mois, la visualisation d’une recanalisation sur l’imagerie de contrôle, ou la cause de l’AVC. Conclusion : la mesure de la densité du thrombus n’est pas un outil reproductible d’un observateur à l’autre quelle que soit la technique utilisée. Elle ne doit pas être considérée comme un facteur pronostique de l’AVC.
