Book excerpt: L'information a beaucoup changé ces cinquante dernières années et tout particulièrement dans le domaine de la médecine et de l'information médicale délivrée au patient. L'objectif principal de cette étude est d'évaluer la qualité de l'information délivrée à la sortie du service des urgences ainsi que la compréhension de cette dernière par les patients. Il s'agit d'une étude épidémiologique descriptive prospective réalisée à la sortie des urgences adultes du J.Monod de mai à novembre 2016. Les horaires de recueil ont été effectués de façon aléatoire. Les patients inclus sont majeurs sortant des urgences le jour même de leur consultation. L'information est évaluée suivant 6 items précis : le diagnostic, le traitement de sortie, les examens complémentaires, les effets secondaires, les consignes de surveillances et l'évolution de la maladie. Pour cela, un unique enquêteur interroge le patient à l'aide d'un questionnaire standardisé afin de recueillir si ce dernier pense avoir été informé et avoir compris l'information. Ensuite, il lui fait retranscrire ses connaissances afin d'évaluer l'assimilation des informations délivrées. La cotation de ces questions est Oui, Moyen, Non. 109 patients ont été inclus dans l'étude. Parmi ces patients, 92 % déclarent être informés sur le diagnostic et 76% l'ont assimilé. Concernant le traitement, 96% déclarent être informés et 75% l'ont assimilé. Concernant les examens complémentaires, 76% déclarent être informés et 60% les ont assimilés. Pour les effets secondaires, 24% déclarent être informés. 31% des patients répondent avoir été bien informés quant aux consignes de surveillance et 20% les ont assimilées. Enfin, concernant l'évolution de la maladie 52% déclarent être informés et 40% l'ont assimilée. Les patients semblent globalement très bien informés concernant leur diagnostic et leur traitement et bien informés concernant leurs examens complémentaires. Toutefois les trois critères concernant les effets secondaires, les consignes de surveillance et l'évolution prévisible de la maladie semblent être moins bien assimilées, cela très certainement dû à un défaut de délivrance. Il serait intéressant de réaliser d'autres études concernant l'information écrite remise au patient afin d'évaluer l'impact de celles-ci sur l'assimilation des informations essentielles à la bonne prise en charge.

Les objectifs secondaires ont été de déterminer qu'elles sont les facteurs qui intervenaient dans la compréhension des patients ainsi que d'évaluer la compréhension ressentie des patients. Matériel et méthode : Nous avons réalisé une étude transversale, prospective, monocentrique aux urgences de l'hôpital Louis Domergue en Martinique pendant un mois nous avons assisté aux entretiens de sortie. Dans un second temps nous leur avons fait passer une entrevue. Les domaines de compréhensions évalués ont été les soins reçus, les examens complémentaires, diagnostic, l'ordonnance et consignes de sortie, les examens complémentaires à réaliser en externe, et le courrier de sortie. Résultats : Nous avons inclus 178 patients dans notre essai. Le niveau de compréhension totale n'est que de 20% pour l'ensemble des domaines. La très grande majorité des patients n'a qu'une compréhension partielle, 44% d'entre eux ont présenté un défaut de compréhension dans au moins 3 domaines. Les consignes de sortie, et le diagnostic font partie des domaines les moins bien perçus. Les acteurs ayant une corrélation statistique significative sur la compréhension des patients sont l'âge, le niveau d'étude, le fait de poser des questions, et la langue. Environ 50% des patients ne sont pas conscients de leur manque de compréhension. Conclusion : Le niveau de compréhension des patients sortant du service des urgences est faible et cela en dépit d'une grande satisfaction des patients vis-à-vis de leurs échanges avec les médecins (note moyenne 8,4/10). Une attention particulière devrait être apporté aux patients créolophones d'autant plus s'ils sont âgés et qu'ils ne posent pas de question.

Or plusieurs études ont démontré que la compréhension des patients aux urgences est faible. Méthode : Etude descriptive, prospective, monocentrique réalisée en 2011 dont l’objectif principal était d’évaluer la compréhension des patients d’un service d’accueil des urgences. Les objectifs secondaires étaient la recherche de facteurs influençant la compréhension, et la recherche d’une corrélation entre la compréhension objective et l’avis du patient et du médecin sur celle-ci. 129 patients ont été inclus. Les patients étaient interrogés à l’aide d’un questionnaire par un enquêteur à leur sortie des urgences. Le médecin donnait son avis sur la compréhension. Résultats : Le diagnostic était compris par 74.4% des patients (IC : 66.89-81.95), le traitement par 49% (IC : 39.08-58.88) et les consignes de sortie par 65.9% (IC : 56-75.81). Des facteurs de mauvaise compréhension du diagnostic ont pu être identifiés : un niveau scolaire bas (p=0.0006), l’absence de maitrise de la langue française (p=0.02), l’âge élevé (p=0.0007). Pour le traitement, les facteurs de mauvaise compréhension étaient : l’absence de médecin traitant (p=0.009), la situation de précarité évaluée par le score EPICES (p=0.04), l’existence d’une souffrance psychique (p=0.04), l’âge élevé (p=0.04), ne pas connaître le statut de l’informateur (p=0.02), avoir consulté en période de garde (p=0.01), ne pas avoir posé de questions (p=0.001). Pour la compréhension des consignes : l’âge élevé (p=0.002), un faible niveau scolaire (p=0.009), l’absence de consignes orales (p=9.109), ne pas connaître le statut de son interlocuteur (p=1.105), ne pas avoir posé de questions (p=0.001). Il n’y a pas de corrélation entre la compréhension objective et l’avis du patient (pour le diagnostic p=0.0004 ; pour le traitement p=2.108 ; pour les consignes = 1.1011). Il n’y a pas non plus de corrélation entre la compréhension et l’avis du médecin (p=6.109 pour le diagnostic, p=0.002 pour le traitement). Conclusion : Notre étude a permis de confirmer que la compréhension des patients aux urgences est mauvaise. Des populations « à risque » de mauvaise compréhension ont été identifiées. Des moyens simples peuvent être mis en place pour améliorer cette compréhension : information orale, inciter le patient à poser de questions, améliorer la compréhension du statut des médecins. Nous avons démontré que le médecin devait vérifier la compréhension et ne pas se fier à son jugement ou à celui du patient.

L’objectif de ce travail était de préciser les motifs de ces consultations et d’évaluer la transmission de l’information médicale. Matériel et Méthodes : Les médecins de 143 patients sortant des urgences de Beauvais ont été contactés par téléphone afin de répondre à un questionnaire renseignant les motifs de consultation des patients et l’évaluation quantitative et qualitative du courrier des urgences. Résultats : La prévalence des consultations était de 39,2%. Les motifs de ces consultations étaient la persistance de la douleur, les arrêts de travail, puis les consultations de principe. Un compte rendu de passage aux urgences a été donné à 32,2% des patients et est parvenu au médecin traitant dans 58,5% des cas. Les médecins évoquent principalement une absence d’exhaustivité de ces comptes rendus et une communication insuffisante avec les services d’urgences. Discussion : Il faut optimiser en charge de la douleur et l’évaluation des arrêts de travail aux urgences. Le patient doit être guidé dès sa sortie afin qu’il suive le bon parcours de soins et ne consulte pas par excès. Il est impératif d’optimiser la transmission de l’information médicale tant sur le plan quantitatif que qualitatif. Une perspective d’avenir pourrait être la transmission du dossier et des données paracliniques par voie électronique. Conclusion : Au final, il semble possible de faire l’économie de nombreuses consultations médicales. Il ne faut pas retirer au médecin référent son rôle de coordinateur des soins et donc lui fournir les informations indispensables à sa pratique.

Ils répondaient à un questionnaire mesurant leur taux de compréhension (diagnostic, organe atteint, pronostic, traitement reçu et consignes de surveillance), leurs désirs en termes d'information, et leurs signes d'anxiété et de dépression selon l'échelle HAD. Le médecin fournissait des renseignements concernant le mode informatif. Résultats : 15,9% des patients comprenaient globalement (4 ou 5 items sur 5) l'information. Ils étaient satisfaits de l'information fournie dans 92,5% des cas. 23% des patients présentaient une symptomatologie dépressive, et 48% des traits de personnalité anxieuse. En univarié, le seul critère favorisant statistiquement la compréhension était le fait que le médecin demande au patient s'il avait des questions à poser (p=0,0ll). D'autres facteurs modifiables liés au mode informatif semblaient améliorer la compréhension, mais de manière non significative. En multivarié, le taux de compréhension était plus faible lorsque le médecin pensait que son patient était dépressif (p=0,03). Conclusion : Les patients comprennent mal les explications fournies au cours de leur hospitalisation en UHCD. La compréhension est altérée lorsque le patient est considéré comme dépressif par le médecin. Un moyen simple d'améliorer cette compréhension est d'inviter le patient à poser des questions.

Il existe une dissociation entre l'impression d'être informé qui est globalement bonne et la réalité de cette information. La transmission de l'information est complexe, fait intervenir de nombreux facteurs et ne peut se résumer à un transfert de données pour se protéger d'une éventuelle poursuite. Il est important de former les médecins aux méthodes de communication et de valoriser le temps de consultation qui devient une composante majeure de l'acte médical.

Un deuxième questionnaire était complété par téléphone à J+7 de la sortie des urgences par le même Interne. 181 patients ont répondu au deuxième questionnaire. Le critère de jugement principal a été évalué sur la notation (de 0 à 10) de la satisfaction concernant le délai de prise en charge médicale. Résultats. Les satisfactions moyennes concernant le délai de prise en charge médicale et la durée de prise en charge sont respectivement de 6,77 et 6,08/10. Les délais de prise en charge moyens envisagé, acceptable, réel et ressenti étaient respectivement de 63, 62, 74 et 86 minutes. 67 % des patients auraient mieux vécu leur attente si on leur avait annoncé le délai prévisionnel de prise en charge médicale dès l’arrivée aux Urgences. Seulement 10 % des patients interrogés ont correctement estimé leurs délais de prise en charge médicale. Les examens biologiques, la réalisation de scanner ou la demande d'avis spécialisés allongent la durée de prise en charge de manière significative. Conclusion. L’obtention de l’information concernant le délai de prise en charge médicale prévisionnel après évaluation par l’infirmière d'accueil pourrait être un moyen d'améliorer la satisfaction des patients consultant aux urgences.

With smartphones in hand, we are no longer beholden to an impersonal and paternalistic system in which "doctor knows best." Medicine has been digitized, Topol argues; now it will be democratized. Computers will replace physicians for many diagnostic tasks, citizen science will give rise to citizen medicine, and enormous data sets will give us new means to attack conditions that have long been incurable. Massive, open, online medicine, where diagnostics are done by Facebook-like comparisons of medical profiles, will enable real-time, real-world research on massive populations. There's no doubt the path forward will be complicated: the medical establishment will resist these changes, and digitized medicine inevitably raises serious issues surrounding privacy. Nevertheless, the result-better, cheaper, and more human health care-will be worth it. The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
