Devant la sur- consommation des antibiotiques en France, il est important d'avoir une prescription adaptée dans cette pathologie. Objectif : L'objectif principal de cette étude était d'évaluer les facteurs qui entrainent une variation entre les prescriptions en antibiotiques des médecins et les recommandations. L'objectif secondaire était de connaître l'intérêt de la réalisation d'un parcours de soin coordonné dans la prise en charge de l'otite moyenne aiguë. Méthode : Il s'agit d'une étude quantitative descriptive observationnelle réalisée à l'aide d'un questionnaire composé de cas cliniques qui a été envoyé par e-mails aux médecins généralistes de Côte-d'Or durant la période de Juillet à décembre 2023. Résultats : Au total, 60 médecins ont répondu au questionnaire soit un taux de réponse de 2,7%. Les médecins généralistes ont une concordance de 52,24% entre leurs prescriptions et les recommandations. Les médecins semblent avoir des difficultés avec la prise en charge de otites moyennes aiguës perforées et celles chez les enfants de plus de deux ans. L'intérêt d'un parcours de soins coordonnés avec les ORL ne semble pas être un point important hormis en cas d'otite à répétition ou de doute diagnostic. Conclusion : La lutte contre la résistance en antibiotique est une priorité de l'OMS. La prise en charge thérapeutique des otites moyennes aiguës en médecine générale doit être améliorée. Plusieurs pistes peuvent permettre d'aider les médecins à la fois dans le diagnostic mais aussi dans la prescription ou non des antibiotiques via les CPTS et CRAtb.

Book excerpt: This book is a comprehensive introduction to text forming resources in English, along with practical procedures for analysing English texts and relating them to their contexts of use. It has been designed to complement functional grammars of English, building on the generation of discourse analysis inspired by Halliday and Hasan's Cohesion in English. The analyses presented were developed within three main theoretical and applied contexts: (i) educational linguistics (especially genre-based literacy programmes) (ii) critical linguistics (as manifested in the development of social semiotics) and (iii) computational linguistics (in dialogue with the various text generation projects based on systemic approaches to grammar and discourse). English Text's major contribution is to outline one way in which a rich semantically oriented functional grammar can be systematically related to a theory of discourse semantics, including deconstruction of contextual issues (i.e. register, genre and ideology). The chapters have been organized with the needs of undergraduate students in theoretical linguistics and postgraduate students in applied linguistics in mind.
